Clinical signs of orbital wall fractures as a function of anatomic location.

OBJECTIVE: The objective of this study was to see whether clinical signs of medial orbital wall fractures distinguished these fractures from fractures of the lateral orbital wall and the orbital floor. STUDY DESIGN: The orbital fractures of 424 patients were analyzed. The patients were divided into 2 groups: (1) patients with orbital fractures with a medial orbital wall component and (2) patients with orbital fractures without a medial orbital wall component. RESULTS: Orbital fractures with involvement of the medial orbital wall showed a significantly higher incidence (P =.001) of diplopia a…

[Surgical treatment of post-contusion orbital hematoma in the late phase].

OBJECTIVE: The object of the study was to present successfully performed decompression of the orbit in orbital hematoma. MATERIAL AND METHODS: Male, 51 years of age sustained the contusion injury of the right eye during his fall on the bath tap. It caused the development of a large orbital hematoma with protrusion, restricted movement of the eyeball, pain, diplopia, folding of the retina due to hemtoma compression to bulbus oculi and retinal hemorrhages, higher intraocular pressure and impaired visual acuity to 2/60. He was treated by medicaments for a month, but the results were not favorabl…

Doppler ultrasonography to aid diagnosis of orbital capillary haemangioma in neonates.

PURPOSE: To illustrate the use of Colour Doppler Ultrasonography to aid the differential diagnosis of orbital masses in neonates. METHODS: Three cases are reported of neonates presenting with undiagnosed orbital masses as isolated findings, causing lid swelling and proptosis and prompting a differential diagnosis of an orbital vascular lesion, rhabdomyosarcoma or other tumour. Colour Doppler Ultrasonography in expert hands using a small footprint linear 10-5 transducer proved an atraumatic, rapid and accurate way of confirming a vascular lesion at the first clinic visit. This reassured the fa…

Endoscopically assisted transconjunctival approach in orbital medial wall fractures.

Full exposure of the medial orbital wall for fracture repair poses difficulty with traditional approaches except coronal incision, especially in cases of wide fracture. The endoscopic-assisted approach with limited incision has been introduced. The authors used the endoscopically assisted transconjunctival approach in 21 cases: 15 isolated medial orbital wall fractures and 6 cases of concomitant floor fractures. Through the medial transconjunctival slit incision, repair of the fracture using calvarial bone graft was undertaken with the aid of an endoscope. All patients recovered without any ey…

Pterional orbital decompression in diseases with acute increase of intraorbital pressure.

BACKGROUND: Orbital cellulitis as well as traumatic or spontaneous intraorbital hemorrhages can lead to an acute rise of intraorbital pressure, threatening the function of orbital vascular, muscular and neural structures. The treatment of this rare entity with extended pterional orbital decompression is described in comparison with other methods of orbital decompression. PATIENTS AND METHODS: Between 1991 and 2000 the authors treated 16 patients with sight-threatening acute elevations of intraorbital pressure due to orbital cellulitis (3 patients) and spontaneous (3 patients), postoperative (…

Orbital granulocytic sarcoma: an unusual presentation of acute myelocytic leukemia.

Granulocytic sarcoma is an unusual manifestation of acute myelogenous leukemia in children and presents a diagnostic dilemma when it precedes the development of systemic disease. We present CT and MRI findings of an extraconal mass proven to be granulocytic sarcoma in a 6-year-old otherwise healthy boy with several months' history of worsening unilateral proptosis. This case is unique in providing exquisite CT and MRI correlation and in demonstrating rapid response to therapy. Further, as cytogenetics were positive for the t(8,21) translocation, this case provides opportunity for discussion of

Orbital blowout fractures: experimental evidence for the pure hydraulic theory.

BACKGROUND: The mechanism of injury and the underlying biomechanics of orbital blowout fractures remain controversial. The "hydraulic" theory proposes that a generalized increased orbital content pressure results in direct compression and fracturing of the thin orbital bone. OBJECTIVE: To examine the pure hydraulic mechanism of injury by eliminating the factor of globe-to-wall contact and its possible contribution to fracture thresholds and patterns. MATERIALS AND METHODS: Five fresh human cadaver specimens were used for the study. In each cadaver head, 1 orbit was prepared to mimic the norm…

Monostotic fronto-orbital fibrous dysplasia with convulsion--case report.

A 28-year-old man presented with monostotic fronto-orbital fibrous dysplasia associated with convulsions. Signs of meningeal irritation were observed. Computed tomography (CT) showed right frontal sinusitis, and destruction from the inner to outer table with expansion of the diploic space. T1- and T2-weighted magnetic resonance imaging showed an abnormal low-intensity mass, with heterogeneous gadolinium enhancement. Although the meningitis resolved, signs of infection continued for 2 months due to sinusitis. Treatment of the right frontal sinusitis was undertaken, accompanied by open biopsy. T…

Orbital involvement in sinus pathology: often without ocular pain.

Many and diverse sinus pathologies can present with an orbital involvement. Contrary to popular belief, uncomplicated rhinosinusitis causes rarely orbital pain or headaches. However, especially in children, acute ethmoidal rhinosinusitis can be complicated by an extension of the infection into the orbit. Also, isolated sphenoid sinusitis is often associated with significant ophthalmologic or neurological complications. Sinus surgery is another possible source of orbital complications associated with pain. The occurrence of orbital pain in these and other sinus pathologies is discussed, since e…

Orbital infection as a complication of sinusitis: are diagnostic and treatment trends changing?

Orbital infection has long been the most common complication of sinusitis. In light of our increased knowledge of sinusitis, improved diagnostic tools, and new pharmacologic and surgical treatments, we investigated whether trends in diagnosis and treatment are changing. We reviewed the charts of all 43 patients who had been referred to our institution with orbital complications of sinusitis between Jan. 1, 1985, and Dec. 31, 1999. Nine of the 43 patients had been diagnosed between Jan. 1, 1985, and Dec. 31, 1990 (mean: 1.5 patients/yr) and 34 had been diagnosed between Jan. 1, 1991, and Dec. 3…

Orbital venous-lymphatic malformations (lymphangiomas) mimicking cavernous hemangiomas.

PURPOSE: To illustrate that orbital venous-lymphatic malformations (lymphangiomas) may rarely simulate cavernous hemangiomas. METHODS: Retrospective case review. RESULTS: Five patients were identified from a series of 85 patients with venous-lymphatic malformations. The age range was 21 to 69 years, and all cases presented with a history of slowly progressive or long-standing proptosis. Computerized tomography revealed relatively homogeneous intraconal masses that were well defined anteriorly. Two of the cases had expansion of the orbit, and one had focal calcification. The three who had mag…

Subgaleal versus subperiosteal dissection in fronto-orbital advancement procedures.

Extensive cranioorbital surgery for craniosynostosis in young children can produce excessive blood loss. Thus, to overcome the disadvantage of unappropriated blood loss and translocation of the osteosynthesis material, we performed a subgaleal dissection in infants and compared the subperiosteal with the subgaleal surgical approach in 29 children who underwent surgery for craniosynostosis. The treatment goal of correction of the deformity was accomplished in each case. Fronto-orbital deformities were significantly improved in all patients. Blood loss was less in the group with subgaleal elevat…
