Effect of scar and His-Purkinje and myocardium conduction on response to conduction system pacing.

INTRODUCTION: Conduction system pacing (CSP), in the form of His bundle pacing (HBP) or left bundle branch pacing (LBBP), is emerging as a valuable cardiac resynchronization therapy (CRT) delivery method. However, patient selection and therapy personalization for CSP delivery remain poorly characterized. We aim to compare pacing-induced electrical synchrony during CRT, HBP, LBBP, HBP with left ventricular (LV) epicardial lead (His-optimized CRT [HOT-CRT]), and LBBP with LV epicardial lead (LBBP-optimized CRT [LOT-CRT]) in patients with different conduction disease presentations using computati

Atrio-hisian fibers anatomy and electrophysiology.

INTRODUCTION: In this case, electrophysiology and histology could be studied in the same heart. Clinical investigation, clinical electrophysiology, and postmortem serial histological sections of the septum were analyzed. METHODS: A patient with repeated seizures and a short PR interval with narrow QRS complex underwent electrophysiologic studies. RESULTS: The patient died while experiencing a very rapid supraventricular tachycardia and histologic examination showed a atrio-hisian bypass tract. CONCLUSION: In our study, the lack of lengthening of the PR interval in spite of progressively pre

Successful radiofrequency ablation of a true paraHissian accessory pathway.

Radiofrequency catheter ablation seems to become a first-line therapy for patients with paroxysmal tachycardias using an accessory pathway. We report the case of a 31-year-old woman who suffered from drug-resistant recurrent reciprocating tachycardia, using an uncommon accessory pathway. The 12-lead electrocardiogram in sinus rhythm showed the following characteristics: a positive delta waves in leads I, II, aVL, aVF, QRS axis = 0 degrees, R/S in lead V1 < 1, and a special QRS pattern in the precordial leads (QS or Qr in V1-V3 and QRS transition > V3). A deep septal parahissian accessory pathw

Response of recurrent sustained ventricular tachycardia to verapamil.

A 28-year-old man is described with no demonstrable organic heart disease and recurrent paroxysmal attacks of sustained ventricular tachycardia. Lignocaine and ajmaline failed to terminate the first attack but a bolus infection of verapamil succeeded. This drug was subsequently successful on six more occasions. During electrophysiological study of the eighth attack, slow intravenous administration of verapamil significantly reduced the rate of the tachycardia and prevented its subsequent reinitiation by pacing. Two mechanisms are postulated to explain both the arrhythmia and the beneficial eff

The sick sinus syndrome. Clinical and electrocardiographic spectrum of nine patients.

The paper presents a report on the clinical and electrographic spectrum of nine patients with the sick sinus syndrome with severe symptoms of clinical disturbances and serious disorders of impulse formation and conduction. Seven patients had syncope associated with sinus bradycardia, sinoatrial block or atrial fibrillation with slow ventricular rate. Prolonged episodes of atrial and ventricular asystole occurred and were interrupted by junctional escape beats. Paroxysmal atrial fibrillation and flutter were foun in three patients and infrahisian block in four. Atropine induced a slight acceler

Stylet-Driven Lead Vs. Lumenless Lead for Left Bundle Branch Area Pacing: Systematic Literature Review and Meta-Analysis.

BACKGROUND: Left bundle branch area pacing (LBBAP) offers cardiac synchrony benefits over conventional ventricular pacing. Although many studies on LBBAP have used lumenless pacing leads (LLLs), stylet-driven pacing leads (SDLs) can also be used. In this study, we compared LLLs and SDLs for LBBAP through a systematic review and meta-analysis of the literature. METHODS: The PubMed, Embase, and Cochrane Library databases were searched for full-text articles on LBBAP from their respective inception dates to April 9, 2024. The studies comparing LLLs and SDLs were extracted, and electrophysiologic

[ICD electrode implantation for left bundle branch area pacing].

We are entering a new era of physiological pacing with left bundle branch area pacing (LBBAP), which is becoming an increasingly common in clinical routine. LBBAP has clear advantages over right ventricular (RV) and biventricular (BVP) pacing. Another option is to combine LBBAP and BVP, known as left bundle branch optimized (LOT) cardiac resynchronization therapy (CRT), which shortens the QRS duration more effectively than LBBAP and BVP alone. Implanting LOT CRT in patients with an ICD indication is particularly complicated. In this case, four leads are generally required. Additionally, the IC

Successful ablation of a right concealed epicardial accessory pathway using ethanol infusion.

INTRODUCTION: This study describes a rare case of concealed epicardial accessory pathway (AP) successfully ablated using ethanol infusion (EI) through a variant vessel connecting the right atrium (RA) and the right ventricle (RV) surface. METHODS AND RESULTS: A 58-year-old male referred to our hospital for prior failed AP ablation. Cardiac-enhanced computerized tomography scan showed there was a variant vessel at the tip of right atrial appendage and a pulmonary artery (PA)-RA fistula at the roof of RA. The earliest activation was present at the site of the PA-RA fistula. A selective angiogra

Evaluation of Criteria for Left Bundle Branch Capture.

Left bundle branch pacing (LBBP) provides electrical and mechanical synchrony at low and stable pacing output and effectively corrects distal conduction system disease. The criteria for differentiating LBBP from LV septal pacing has not been validated in large trials. There are several electrocardiography-based and intracardiac electrogram-based criteria to confirm LBB capture. In this section, the authors review these criteria and their overall accuracy.

A Continuous Pacing and Recording Technique for Differentiating Left Bundle Branch Pacing From Left Ventricular Septal Pacing: Electrophysiologic Evidence From an Intrapatient-Controlled Study.

BACKGROUND: Left bundle branch pacing (LBBP) is a promising approach for achieving near-physiologic pacing. However, differentiating LBBP from left ventricular septal endocardial pacing (LVS(e)P) remains a challenge. This study aimed to establish a simple and effective method for differentiating LBBP from LVS(e)P and to evaluate their electrophysiologic characteristics. METHODS: LBBP, using continuous uninterrupted pacing and real-time monitoring of electrocardiograms along with intracardiac electrograms, was performed in 97 consecutive patients. We evaluated the electrophysiologic characteri

Physiologic Variants of Cardiac Conduction (Aberration, Gap, Supernormal Conduction).

Wide QRS complexes during supraventricular rhythms can be caused by fixed bundle branch block, functional (intermittent) bundle branch block, preexcitation, or toxic/metabolic causes. Functional bundle branch block can be caused by long-short aberrancy (usually physiologic), or acceleration/deceleration dependent aberrancy (usually pathologic). Electrocardiogram criteria have been proposed to differentiate aberration from ventricular tachycardia; they are not always accurate. The gap phenomenon "paradox" is that with increasingly premature extrastimuli, progressive proximal conduction delay al

Is the left bundle branch pacing a choice to conquer the right bundle branch block?-A case report.

In patients with preserved ejection fraction or right bundle branch block (RBBB) pattern requiring a high percentage of ventricular pacing, His-bundle pacing (HBP) might be an alternative to biventricular pacing, although the high threshold occasionally occurs. We provided a case of the intrinsic RBBB correction by capturing intra-Hisian left bundle branch (LBB) or distal His-bundle with different output settings. LBB pacing had the advantage of a much lower threshold while remained most synchrony as HBP. LBB pacing might be a promisingly safe and effective procedure for patients with high-gra
