The atypical pneumonias: a diagnostic and therapeutic approach.

Mycoplasmal pneumonia, tularemic pneumonia, Q fever pneumonia, psittacosis, and Legionnaires' disease are the most frequently encountered treatable atypical pneumonias. Mycoplasmal pneumonia, the most common, is often accompanied by nonexudative pharyngitis, conjunctivitis, or otitis. The nonproductive cough is characteristic. Tularemic pneumonia is characterized by substernal chest pain, bloody pleural effusion, and bilateral hilar adenopathy. Although the clinical presentation is mild, roentgenographic findings are impressive. Q fever pneumonia resembles psittacosis but is less serious; it m

External diseases of the eye.

The child presenting a red, tearing eye can pose a dilemma for the pediatrician. A good history and a careful systematic examination using good light and magnification should lead the physician to the proper diagnosis. The child with no complaints other than discharge probably has a conjunctivitis, while the child with photophobia and a scratchy sensation should make one search for either a foreign body or a corneal problem. Fluorescein dye is invaluable in outlining a corneal scratch, abrasion, dendritic ulcer, or bacterial ulcer; and the use of smears and cultures can help pinpoint the exact

Effect of Copper Ion Sterilization on Bacterial Community in a Freshwater Recirculating Aquaculture System.

The study aimed to evaluate the safety of copper ion sterilization based on copper ion residues in zebrafish (Brachydanio rerio), as well as bacterial community structure and diversity in recirculating aquaculture systems (RASs). The copper ion content was determined using national food safety standard GB 5009.13-2017. Bacterial community structures and alpha and beta diversity indexes were examined using the 16S rRNA gene sequences produced by Illumina HiSeq sequencing. The results revealed no significant copper ion enrichment in B. rerio when the copper ion concentration was 0.15 mg/L. The r

[Trend survey of ocular infections with bacteria at Toyama University Hospital over the past six years--from the standpoint of laboratory examination].

Specimens of bacterial ocular infections are frequently received in the clinical laboratory. However, a comprehensive trend survey of ocular infections with bacteria is very rare. Our objective is to understand the current tendency of ocular infections with bacteria in patients at Toyama University Hospital from the standpoint of laboratory examination. We studied 263 cases of ocular infection with bacteria diagnosed at Toyama University Hospital from January 2006 to December 2011. 123 were male and 140 were female, with a mean age of 61.2(0-98) years. Specimens were subjected to direct micros

[Gonococcal conjunctivitis complicated by perforating corneal abscess in an adult].

We present a case of bilateral purulent conjunctivitis complicated by ocular perforation of the right eye secondary to fulminant corneal melt in a 29-year-old man. He developed urethritis after a sexual contact with a prostitute 3 weeks previously. Microbiological analyses of conjunctival and urinary cultures were positive for Neisseria gonorrhoeae resistant to penicillins, tetracyclines, and fluoroquinolones. Progression was favorable with a 15-day course of high doses of parenterally administered antibiotics associating imipenem and fosfomycin. Keratoplasty was done after 3 months. This obse

Neonatal meningococcal conjunctivitis associated with meningococcal meningitis.

Two infants are described in whom identical strains of meningococcus were isolated from both the eyes and the cerebrospinal fluid. This suggests that the eye may be a portal of entry in at least some cases of perinatally acquired neonatal meningococcal disease and has important implications for the management of purulent conjunctivitis in the newborn.

Zinc-solubilizing Bacillus spp. in conjunction with chemical fertilizers enhance growth, yield, nutrient content, and zinc biofortification in wheat crop.

Micronutrient deficiency is a serious health issue in resource-poor human populations worldwide, which is responsible for the death of millions of women and underage children in most developing countries. Zinc (Zn) malnutrition in middle- and lower-class families is rampant when daily calorie intake of staple cereals contains extremely low concentrations of micronutrients, especially Zn and Fe. Looking at the importance of the problem, the present investigation aimed to enhance the growth, yield, nutrient status, and biofortification of wheat crop by inoculation of native zinc-solubilizing Bac

Urocanate as a potential signaling molecule for bacterial recognition of eukaryotic hosts.

Host recognition is the crucial first step in infectious disease pathogenesis. Recognition allows pathogenic bacteria to identify suitable niches and deploy appropriate phenotypes for successful colonization and immune evasion. However, the mechanisms underlying host recognition remain largely unknown. Mounting evidence suggests that urocanate-an intermediate of the histidine degradation pathway-accumulates in tissues, such as skin, and acts as a molecule that promotes bacterial infection via molecular interaction with the bacterial regulatory protein HutC. In Gram-negative bacteria, HutC has

Eliminating biofilm from ureteral stents: the Holy Grail.

PURPOSE OF REVIEW: Biofilms continue to be a major limiting factor in the long-term use of ureteral stents, promoting the development of chronic infections and antibiotic resistance and encrustation. Apart from stent removal in conjunction with antibiotic therapy, there is currently no treatment proven successful for completely eradicating a biofilm-related infection, highlighting the need for continued research in this area. RECENT FINDINGS: Research continues to focus mainly on biofilm prevention, specifically the development of novel coatings comprising antibacterial, antifouling or bacter

In-111 labeled leukocytes: a review of problems in image interpretation.

Leukocyte suspensions labeled with In-111 oxine or tropolone were administered intravenously to 150 patients for the detection of suspected foci of bacterial infection by gamma camera imaging. The results were correlated with other imaging modalities, and clinical, laboratory, and surgical findings after a minimum follow-up period of six months. Twenty-five of 29 foci of bacterial infection were demonstrated on the leukocyte-labeled images (sensitivity of detection = 86%). Three of the four missed lesions were chronic active osteomyelitis. The specificity of detection proved difficult to defin

Retrospective Review of Mycobacterial Conjunctivitis in Cockatiels (Nymphicus hollandicus).

The etiologic disease organism responsible for causing mycobacteriosis in avian species is an acid-fast gram-positive bacterium. This bacterium causes granulomatous disease in various internal organs, but in cockatiels (Nymphicus hollandicus) it has been commonly identified within the conjunctival tissues. Twenty-six cases of mycobacterial conjunctivitis in cockatiels were diagnosed through histopathologic assessment of diseased tissue samples, Fite acid-fast staining, and polymerase chain reaction in this retrospective study. Clinicians who saw these cases were contacted, and information was

Is Chlamydophila felis a significant zoonotic pathogen?

PROBLEM: Chlamydophila felis is a common cause of conjunctivitis in cats and in some textbooks is listed as an important zoonotic pathogen. However, there are no published comprehensive reviews assessing the evidence supporting this. SEARCH AND ANALYSIS: Bibliographic databases and bibliographies of papers and textbooks were searched for all published cases of zoonotic disease associated with chlamydiosis in cats. All published case reports were reviewed to establish the quality of the evidence supporting the association between C. felis and zoonotic disease. RESULTS: There are only seven cas

Mycoplasmal conjunctivitis in a European starling.

Bilateral conjunctivitis and episcleritis were identified in an adult European starling (Sturnus vulgaris). A novel mycoplasma species, Mycoplasma sturni, was isolated in pure culture from the conjunctiva of both eyes. The clinical presentation was similar to that of conjunctivitis in house finches (Carpodacus mexicanus) caused by Mycoplasma gallisepticum. However, the histologic lesions were distinct, by the presence of ulceration and by the absence of epithelial hyperplasia and lymphoplasmacytic infiltration [corrected]. Mycoplasma sturni ferments glucose, does not hemadsorb or hemagglutinat

[Cat-scratch disease associated with osteomyelitis].

A case of cat-scratch disease in an 8 year-old girl is reported. The inoculation injury followed by regional lymphadenopathy was typical of Parinaud syndrome. This patient also had vertebral lesions on scintigraphy, x-rays and CT scan. While the oculoglandular localization spontaneously improved within a few weeks, osteomyelitic lesions remained grossly unchanged after almost 2 years of follow-up.

Importance of reinfection in the pathogenesis of trachoma.

The authors' epidemiologic studies of trachoma on Taiwan and experimental monkey eye infections with and without Chlamydia trachomatis immunization are reviewed for the data they provide on the pathogenesis of trachoma. These studies indicate that trachoma is an immunopathologic disease in which the more severe progressive trachoma infections with pannus and scar formation occur only after reinfection. This hypothesis is supported by a 10-year study of 32 family households that were followed with repeated clinical and laboratory observations. Although most cases of active trachoma healed spont

Fatal genitourinary mucormycosis in a patient with undiagnosed diabetes.

We present what we believe is the first report in the world literature of penile necrosis due to mucormycosis, a rare and often fatal fungal infection. This case of rhizopus mucormycosis began with a penile lesion in a 27-year-old patient with undiagnosed diabetes; it led to necrosis of the phallus, lower urinary tract, rectum, and pelvic musculature and finally to death. Despite repeated aggressive surgical debridement in conjunction with medical therapy, we were unable to halt the progression of the fungal and synergistic bacterial infections.

A MAPS Vaccine Induces Multipronged Systemic and Tissue-Resident Cellular Responses and Protects Mice against Mycobacterium tuberculosis.

Tuberculosis (TB) remains a leading cause of morbidity and mortality worldwide. To date, the mainstay of vaccination involves the use of Mycobacterium bovis bacillus Calmette-Guérin (BCG), a live-attenuated vaccine that confers protection against extrapulmonary disease in infants and children but not against lung disease. Thus, there is an urgent need for novel vaccines. Here, we show that a multicomponent acellular vaccine (TB-MAPS) induces robust antibody responses and long-lived systemic and tissue-resident memory Th1, Th17, and cytotoxic CD4+ and CD8+ T cells, and promotes trained innate i
