Emergency procedures on the descending thoracic aorta in the endovascular era.

BACKGROUND: Thoracic endovascular aortic repair (TEVAR), initially developed for the treatment of degenerative aneurysms of the descending thoracic aorta, has been applied to the entire spectrum of descending thoracic aortic pathology in both the elective and emergent settings. This single center study evaluates the effectiveness of TEVAR for the treatment of acute surgical emergencies involving the descending thoracic aorta, including traumatic aortic disruption (TAD), ruptured descending thoracic aneurysm (RDTA), and acute complicated Type B dissection (cTBD). METHODS: A retrospective revie

Heparinless left heart bypass for resection of thoracic aortic aneurysms.

Our clinical experience employing heparinless left heart bypass for the resection of twenty-three thoracic aneurysms is presented. In our current technic of heparinless left heart bypass, plastic tubing coated with nonthrombogenic polyurethane-polyvinyl-graphite material and a conventional roller pump are employed. The reduced size of the aneurysm below the aortic clamp during bypass facilitates careful dissection of the aneurysm and the aorta. Twenty-two of twenty-three patients underwent successful thoracic aneurysmectomy with this technic. None had subsequent paraplegia and the postoperativ

Surgical treatment of aortobronchial fistula after thoracic endograft failure.

Endovascular stent grafting has been recently considered as a less invasive alternative to either medical therapy or open surgical treatment for many patients with descending thoracic aortic disease. Late complications are rarely described in literature. Herein, we described the occurrence of an aorto-bronchial fistula and a retro-A dissection in a 73-year-old man after stent-grafting for a penetrating atherosclerotic ulcer (PAU) of the descending thoracic aorta and the successful surgical technique adopted in order to remove the stent-graft.

Deterioration of vascular dementia caused by recurrent multiple small emboli from thoracic aortic atheroma.

We report a case of a 77-year-old man with deteriorating dementia caused by repeated multiple small cerebral embolisms from a thoracic aortic atheroma. Multiple small embolisms were confirmed by diffusion-weighted magnetic resonance imaging (DWI). The patient ultimately died due to aortic dissection. Pathological examinations revealed that no causative embolic source for multiple embolisms could be detected other than severe atheromatous ulcer in thoracic aorta. This case demonstrates that severe aortic atheroma has the potential to precipitate deterioration of vascular dementia.

Treatment with Aortic Stent Graft Placement for Stanford B-Type Aortic Dissection in a Patient with an Aberrant Right Subclavian Artery.

A 71-year-old man visited our hospital with the chief complaint of back pain and was diagnosed with acute aortic dissection (Debakey type III, Stanford type B). He was found to have a variant branching pattern in which the right subclavian artery was the fourth branch of the aorta. We performed conservative management for uncomplicated Stanford type B aortic dissection, and the patient was discharged. An ulcer-like projection (ULP) was discovered during outpatient follow-up. Complicated type B aortic dissection was suspected, and we performed thoracic endovascular aortic repair (TEVAR). The ai

Long-term predictors of descending aorta aneurysmal change in patients with aortic dissection.

OBJECTIVES: We sought to demonstrate the long-term natural course of descending aorta dilation after acute aortic dissection (AD) and identify early predictors for late aneurysmal change. BACKGROUND: Aneurysmal dilation of the aorta is a critical late complication in AD patients. METHODS: Contrast-enhanced computed tomography (CT) was performed during the acute phase in 100 AD patients, comprising 51 type 1 who underwent ascending aorta surgery and 49 type 3 AD patients. Clinical observation was conducted for 53 +/- 26 months, and CT was repeated for 31 +/- 27 months. RESULTS: Aneurysm (dia

Physician-modified fenestrated endograft for postdissection thoracoabdominal aortic aneurysm following provisional extension to induce complete attachment and renal artery stenting.

We have described a patient with a history of type A-11 dissection repair, who subsequently underwent thoracic endovascular aortic repair with distal bare metal dissection stents (provisional extension to induce complete attachment) and renal artery stenting for malperfusion. During the next 3 years, the thoracoabdominal aorta had continued to enlarge to 6.9 cm, despite false lumen embolization and thoracic endovascular aortic repair extension. Given the continued aortic enlargement, physician-modified fenestrated endovascular aortic repair was performed within the prior aortic and renal stent

KIF6 Trp719Arg Genetic Variant Increases Risk for Thoracic Aortic Dissection.

BACKGROUND: KIF6 (kinesin family member 6), a protein coded by the KIF6 gene, serves an important intracellular function to transport organelles along microtubules. In a pilot study, we found that a common KIF6 Trp719Arg variant increased the propensity of thoracic aortic aneurysms (TAA) to suffer dissection (AD). The present study aims for a definite investigation of the predictive ability of KIF6 719Arg vis à vis AD. Confirmatory findings would enhance natural history prediction in TAA. METHODS: 1108 subjects (899 aneurysm and 209 dissection patients) had KIF6 719Arg variant status determin

Time-dependent mechanical behaviour of the aortic chronic dissection flap.

OBJECTIVES: The transition of aortic dissection from acute to chronic is poorly understood. We examined time-dependent mechanical behaviour and biochemical properties of chronic dissection tissues. METHODS: Aorta samples were obtained from 14 patients with mixed aetiology who were undergoing elective surgery for chronic dissected aneurysms, ranging from 3 months to 15 years post-dissection. The tissue elastic modulus and tissue deformation following application of loading for 5 h were measured for the false lumen (FL), true lumen (TL) and flap (FP) tissues with a custom-indentation technique.

Expanding perigraft seroma after ascending aorta replacement.

BACKGROUND: Perigraft seroma is a persistent and sterile fluid confined within a fibrous pseudomembrane surrounding a graft that develops after graft replacement. Development of perigraft seroma is an uncommon complication that occurs after the surgical repair of the thoracic aorta using woven polyester grafts. mechanism underlying perigraft seroma formation remains unclear. CASE PRESENTATION: Herein, we describe the case of 77-year-old man who underwent repeat sternotomy for the treatment of large perigraft seroma 1 year after ascending aorta replacement for acute type A dissection. Lack of evidence for the involvement of non-canonical pyroptosis in a mouse model of thoracic aortic dissection.

Thoracic aortic dissection (TAD) is a severe aortic disease with high mortality. One of its pathological hallmarks is aortic smooth muscle cell death, but the death modalities are not fully known. Recent studies suggest an association of pyroptosis with aortic diseases, but whether it contributes to TAD remains unknown. Here, we modeled TAD by feeding 3-week-old mice with β-aminopropionitrile (BAPN), a lysyl oxidase inhibitor, for 28 days. By means of global knockout of Caspase11 and Gsdmd in mice, two key knots in non-canonical pyroptosis pathway, we showed that neither Caspase11-/- nor Gsdmd

H2S Click Delivery: Responsive Release for Multidimensional Therapy of Thoracic Aortic Dissection.

Thoracic aortic dissection (TAD), a lethal cardiovascular emergency, lacks effective pharmacological interventions. Endothelial dysfunction and hydrogen sulfide (H2S) deficiency drive TAD progression, but existing H2S donors exhibit uncontrolled release and cytotoxicity. Herein, we developed a GSH-responsive H2S-releasing polymer, named PSG12, using a gas-click polymerization strategy under mild conditions. PSG12 achieved tunable H2S release (4.27-fold higher at 2 mm than 0.5 mm GSH) and sustained plasma levels (more than 650 nm for 12 h post-injection). In TNF-α-challenged endothelial cells,

[Surgical treatment of a patient with complications after endovascular procedure for type B aortic dissection (a clinical case report)].

The incidence of aortic dissection varies from 3 to 10 cases per 100.000 people. According to the International Registry of Acute Aortic Dissection (IRAD), type B aortic dissection accounts for 33%. The mean age of these patients is 63 years, with two thirds of them being males. The past two decades have witnessed an increase in the use of thoracic endovascular aortic repair (TEVAR) and hybrid methods of treatment from 7 to 31%, with a decrease in the proportion of open operations from 17 to 8%. In the current guidelines of the European Association for Cardio-Thoracic Surgery (EACTS) and the S

Standing tall after DeBakey Type I aortic dissection extending to left iliac artery.

This report describes DeBakey Type I aortic dissection in a middle-aged hypertensive female who had undergone mitral tissue valve replacement a decade previously. The patient had severe abrupt onset tearing pain in her throat, back, and chest, for which she got admitted in a community hospital, where because of no changes in her ECG and biomarkers, the dissection of aorta was missed. She was subjected to coronary angiography more than 6 weeks later for pain in her left shoulder, which demonstrated normal vessels. She then underwent multi-detector computerised tomography aortogram (MD CTA) that

Medically Managed Type A Thoracic Intramural Hematoma and Penetrating Aortic Ulcer.

Intramural hematoma (IMH) and a penetrating aortic ulcer (PAU) are included in a larger category of disorders termed acute aortic syndromes. These disorders typically involve the thoracic aorta, abdominal aorta, or both, and often require emergent evaluation and treatment. Both IMH and PAU, much like aortic dissection, are classified using the Stanford and DeBakey systems to indicate the aortic area involved, with Stanford type A (DeBakey type I and II) necessitating surgical intervention, and Stanford type B sufficing with medical management of blood pressure. While IMH and PAU share many cha

Sequential ruptures of penetrating atherosclerotic ulcers of ascending aorta, aortic arch and descending thoracic aorta.

BACKGROUND: Penetrating ulcers of aorta, aortic dissections and intramural hematomas all come under acute aortic syndromes and have important similarities and differences. CASE REPORT: We report a 67 year old man with rupture of a large penetrating ulcer of the distal ascending aorta with hemopericardium and left hemothorax. He underwent interposition graft replacement of ascending aorta and hemi-arch with a 30 mm Gelweave Vascutek graft but represented 6 months later with development of a penetrating ulcer which ruptured into a huge 14 cm pseudoaneurysm. This was repaired with a 28 mm Vascut

[A simultaneous operation of aortic root, arch replacement, and sternal elevation for Marfan's syndrome--a case report].

A 42-year-old male was transferred to our institution by his family doctor because of suspected type A aortic dissection with cardiac tamponade. His physical constitution gave the appearance of Marfan's syndrome. Contrast CT revealed DeBaky type 1 aortic dissection. Angiography detected an Annulo-aortic ectasia complicated by an aortic regurgitation (AR) grade IV. He also suffered from a severe funnel chest. We performed simultaneous procedures of aortic root, arch replacement, and sternal elevation. Upon operation, a staged aortic clamp technique was employed to reduce the period of cardiac a

In Situ Needle Fenestration during Thoracic Endovascular Aneurysm Repair: Successful Fenestration of Two Overlapping Thoracic Stent Grafts.

Endovascular stent grafting is becoming more common in treating complex thoracic aortic aneurysms and dissections. When it becomes necessary to cover the supra-aortic vessels, maintaining blood supply through the supra-aortic branches can be achieved by performing in situ needle fenestration. We present a case of a 65-year-old man with a type B aortic dissection that extended from the origin of the left subclavian artery. A stent graft was inserted into the thoracic aorta distally of the origin of the left common carotid artery. Due to the stent graft moving distally and not adequately sealing
