Treatment of SCFE in a healthy 5-year-old child: case report and review.

Slipped capital femoral epiphysis is a common adolescent hip disorder and when patients present at an age younger than 10 years, it is atypical and there is often some identifiable associated metabolic or endocrinologic abnormality. We present the case of a 5-year-old boy with idiopathic bilateral slipped capital femoral epiphysis. This patient underwent staged bilateral pinning in situ using a uniquely modified screw where the distal threads were machined-off for smooth fixation across the physis. The patient had an unremarkable postoperative course and showed evidence of remodeling and paten

Slipped capital femoral epiphysis: an ultrastructural study before and after osteosynthesis.

BACKGROUND AND PURPOSE: Several different theories have been proposed to explain the pathogenesis of slipped capital femoral epiphysis (SCFE). Using transmission electron microscopy (TEM), we carried out an ultrastructural study of core biopsy specimens of the physis at various stages of the disease. METHODS: Core biopsies were performed in 6 patients with different forms of SCFE during the first operation, and in 3 of them when removing the osteosynthesis material before physeal closure. The specimens were prepared for TEM examination. RESULTS: In 6 specimens obtained at first surgery, a ma

Body mass index in patients with slipped capital femoral epiphysis.

Approximately 20% of children with idiopathic slipped capital femoral epiphysis (SCFE) have bilateral disease. Predicting which patients will develop problems with both hips remains difficult. This is the first study to evaluate the relationship between body mass index (BMI) and unilateral and bilateral SCFEs. Height and weight measurements of patients presenting with SCFE to our institution were obtained and used to calculate the BMI. Of the 54 patients enrolled in the study, 16 ultimately had bilateral disease. The mean BMI of patients with bilateral disease was significantly greater than th

Arthroscopic retrieval of a broken guidewire fragment from the hip joint after cannulated screw fixation of slipped capital femoral epiphysis.

The use of cannulated screws for internal fixation of slipped capital femoral epiphysis (SCFE) is recognized as the standard method of treatment and has fewer complications compared with previous methods such as pins or tri-flanged nails. Some complications related to guidewires have been reported in the treatment SCFE. The most dangerous complication is inadvertent advance of the guidewire into the pelvic cavity. Guidewire breakage is frequent and maybe under-reported. Articular migration of a guidewire fragment has potentially devastating effects and implies a second surgical procedure. Open

Reliable X-ray diagnosis of slipped capital femoral epiphysis by combining the conventional and a new simplified geometrical method.

BACKGROUND: The diagnosis of subtle slipped capital femoral epiphysis (SCFE), for example in the contralateral asymptomatic hip, may require use of an exactly defined and reproducible lateral view where the slipping angle can be measured. OBJECTIVE: To test a simplified geometrical method and compare it to the conventional method (AP and frogleg views). MATERIALS AND METHODS: The two methods were compared in 95 normal children 9-14 years old and 100 children 9-20 years old with SCFE. RESULTS: The slipping angle (SA) measurements had very high reproducibility, and the new method was diagnost

Biomechanical analysis of compression screw fixation versus standard in situ pinning in slipped capital femoral epiphysis.

A slipped capital femoral epiphysis was created in 12 matched pairs of immature bovine femora using an anterior-to-posterior-directed shear force. All soft tissues, with the exception of the perichondrial ring, were removed before testing. One specimen from each pair was fixed with a single cannulated screw in standard fashion, whereas the contralateral specimen was fixed with a single screw that compressed the physis. The amount of compression achieved was quantified using Fuji film. Standard fixation yielded 1.4 MPa of pressure across the physis; compression fixation yielded 3.2 MPa, a 2.3-f

Deficiency of a parathyroid hormone fragment containing the midportion and 1,25-dihydroxyvitamin D in serum of patients with slipped capital femoral epiphysis.

The endocrine abnormality that causes slipped capital femoral epiphysis (SCFE) has not been revealed. Recent studies have shown that parathyroid hormone (PTH) and 1,25-dihydroxyvitamin D [1,25-(OH)2D] are involved in growth-plate chondrogenesis and matrix mineralization. Thus we examined in 13 patients with SCFE the serum levels of three immunoreactive forms of PTH (iPTH): the whole peptide [(1-84)PTH], the fragment containing the COOH-terminal portion (C-PTH), and the midportion (M-PTH). Additionally, serum levels of 25-hydroxyvitamin D [25-(OH)D] and 1,25-(OH)2D were measured. We found that

Bilateral epiphysiolysis of the femoral heads in two dogs.

Two Shetland Sheepdogs that did not have a history of trauma were referred because of a gradual onset of lameness in the hind limbs. Bilateral slipped capital femoral epiphysis was diagnosed. Separation of the proximal femoral epiphysis that is not associated with trauma is recognized as a distinct clinical syndrome in adolescent human beings and swine, causing a condition called epiphysiolysis. The precise cause of this type of injury is unknown. Histologic lesions observed in the growth plates could have been the result of an abnormally high mechanical load imposed by obesity. It is not know

Biomechanical comparison of three internal fixations for treatment of slipped capital femoral epiphysis in immature dogs.

The biomechanical strength and stiffness of 3 fixation techniques used to repair acute slipped capital femoral epiphysis were evaluated in bone specimens from immature dogs. A servohydraulic testing machine was used to create slipped capital femoral epiphysis in 9 pairs of femurs by shearing the capital femoral epiphysis along the physis in a craniocaudal direction. The slip was reduced and repaired with 1, 2, or 3 double-pointed, 1.6-mm (0.062-inch) smooth pin(s) and retested. The strength and stiffness of each intact femur (which served as the control) and repaired femur were compared. Resul

[Lengthening of long bones by the method of slow lysis of the epiphyses. Preliminary report].

The authors have presented a preliminary report on the results of elongation of the lower extremities by the bloodless method of slow disruption of the long bones epiphyses based on the observation of 6 cases. The treatment was applied in the children just before bone growth has been finished. Bone rebuilding at the site of elongation was obtained at 4 to 6 months after the start of distraction. In that period, union of the epiphyseal cartilage was present in 5 from 6 treated patients. The authors believe that the presented method of elongation of the lower extremities can be recommended.

The evolving slope of the proximal femoral growth plate relationship to slipped capital femoral epiphysis.

We measured the physeal angle (slope) of the proximal femoral physis on the standard anteroposterior (AP) radiographs. Controls consisted of 307 hips in children aged 1-18 years. Similar measurements were made on the affected (slip) and unaffected (nonslip) sides of 107 children with manifest unilateral slipped capital femoral epiphysis. An age-related increase of 14 degrees occurs in the slope of the proximal femoral physis between ages 1 and 18 years, with maximal increase between ages 9 and 12 years. Increasing obliquity of the proximal femoral physis may be yet another factor contributing

[The painful hip--diagnostic possibilities of sonography].

205 patients with a painful hip joint have been investigated prospectively. The examination technique and the sonoanatomy of the hip joint are illustrated. An intraarticular effusion could be detected in 105 patients. Depending on the aetiology and the duration of disease, different echostructures of the fluid collections were noted. Additionally, the reaction of the synovia and of the articular capsule allowed the discrimination of transient synovitis and septic arthritis. Extraarticular alterations were found in 11, changes of the bone contour in 40 patients (osteomyelitis, slipping femoral

Bilaterality in slipped upper femoral epiphysis.

The frequency of slipping and osteoarthritis of the contralateral hip was recorded in 260 patients with slipped upper femoral epiphysis between 1910 and 1960. Twenty-three of these patients (9%) had primary bilateral slipping, 32 (12%) had a contralateral slip diagnosed later during adolescence and a further 104 (40%) had signs of contralateral slipping at follow-up 16 to 66 years later, giving a total of 159 cases (61%) with bilateral slips. Of the 104 slippings diagnosed at follow-up, 25% showed osteoarthritis. It is concluded that, with a slipped epiphysis, prophylactic contralateral pinnin

Growth of children with physiolysis of the hip.

Growth during adolescence was analyzed in 40 children with slipped capital femoral epiphysis using the Infancy Childhood Puberty growth model. The advantage of this model is that reference values for height can be adjusted for the individual age at pubertal maturation. In both sexes an above average height was found before the onset of the pubertal growth spurt. However, the pubertal gain in height was less than normal, and at maturity the heights were only slightly above the reference mean values. Most of the children had their first hip symptoms during the period of increasing gain in height
