INTRODUCTION: In the treatment of avulsion fractures of the posterior calcaneal tuberosity, open reduction and internal fixation are prone to several complications. We describe a new treatment using an Ilizarov external fixator, which can minimise the complications and achieve sufficient stability of the displaced fragment.
CASE PRESENTATION: A 55-year-old woman sustained an avulsion fracture of the calcaneus. Examination revealed the development of bruising with extremely taut skin over the posterior prominence of the displaced bone. Radiographs demonstrated grossly proximal displacement of

OBJECTIVE: To investigate the effectiveness of hinged external fixator with mini-plate to treat terrible triad of the elbow.
METHODS: Between August 2008 and June 2011, 11 patients with terrible triad of the elbow were treated with hinged external fixator combined with mini-plate. There were 9 males and 2 females, aged 22-56 years (mean, 34 years). The injuries were caused by falling from height in 8 cases and traffic accident in 3 cases. All patients were closed injury. The time from injury to operation varied from 8 hours to 7 days (mean, 3.7 days). According to Mason classification standar

BACKGROUND: Knee dislocation is a severe but relatively uncommon injury caused by violent trauma that can result in long-term complications, such as arthrofibrosis, stiffness, instability, and pain. Perhaps owing in part to its rarity, treatment of this injury is controversial. We therefore describe a treatment approach for these complex cases involving a novel dynamic knee external fixator.
DESCRIPTION OF TECHNIQUE: We performed open PCL reconstruction when possible and/or repair of other associated lesions. At the end of the surgical procedure, the surgeon applied an external fixator that r

Charcot neuroarthropathy of the foot and ankle is a devastating neuropathic complication that can eventually lead to a lower extremity amputation in the presence of an ulceration or infection. Current surgical approaches for the management of the diabetic Charcot foot and ankle deformities are largely based on expert opinions in various fixation methods attempting to avoid major postoperative complications. The goal of this article is to discuss the advantages and disadvantages of various internal, external, or combined fixation methods as they relate to the inherent challenges in the manageme

BACKGROUND: Certain complex foot deformities can be corrected surgically with a U-osteotomy. This osteotomy is indicated for patients with a uniform deformity of the entire foot relative to the tibia, preexisting stiffness and/or fusion of the subtalar joint, and a pain-free ankle joint. The goal is to create a plantigrade foot through gradual osseous repositioning of the entire foot relative to the tibia by means of external fixation. If needed, foot height can be increased simultaneously.
METHODS: Fifteen complex multiplanar foot deformities in fifteen patients were treated with a U-osteoto

Five cats with caudal jaw injuries including mandibular ramus fractures, temporomandibular luxation/subluxation and temporal bone fractures were managed with external skeletal fixation to provide open-mouth maxillomandibular fixation. Three of five cats were able to eat orally during the period of fixation, whereas two cats with jaws fixed in a suboptimal position were dependent on oesophagostomy tube nutrition. Fixation was well tolerated and was maintained for 21 to 42 days. All cats were eating normally and had good jaw function at follow-up (mean 39 months, range 7 to 71 months).

BACKGROUND: We conducted a study evaluating the clinical and radiologic results of the open tibial shaft fractures using an external fixator as definitive treatment.
METHODS: Clinical, observational, descriptive, prospective and longitudinal study. Forty-six patients were included, 40 males (87%) and 6 females (13%), whose age was 31.02 +/- 14.62 years; the time elapsed from the accident to admission in the Emergency Room was 1-16 hours, with a mean of 5.1 +/- 3.35 hours. Ten patients (21.74%) had a Gustilo grade I open fracture, and 36 patients (78.26%) a Gustilo grade II fracture. They were

BACKGROUND: Nonunion of the supracondylar area of the humerus is a very difficult area to treat. The Ilizarov method has been shown to be effective in the treatment of nonunion of the humeral diaphysis. However, there is little in the literature regarding the treatment of nonunion of the supracondylar area especially in post-infection cases.
METHODS: Eight patients with a mean age of 45.73 ± 11.42 years were treated for post-infection nonunion of the supracondylar area of the humerus with the Ilizarov method and followed up for 3 years. All had undergone at least 2 previous failed operations.

Most of the evidence to date on the Ilizarov method in the management of complex foot and ankle deformities in children is based on expert opinion and retrospective case series. Often the technique is used as a salvage option where conventional techniques are inappropriate or have failed. The decision to use the Ilizarov external fixator to an alternative technique depends on several issues: complexity of the pathology, patient compliance, surgeon skills, and the capacity of the institution to manage patients with multidisciplinary requirements. Nevertheless, the Ilizarov method has proved to

We retrospectively review 84 cases of diaphyseal humeral fractures (24 type A, 38 type B, 22 type C of the AO/OTA classification) treated with external fixation (Hoffmann II frame) between 1995 and 2007. Six of these fractures were complicated with radial nerve palsy. Four cases were open fractures. All reductions were achieved closely or through minimal open approaches. All fractures achieved consolidation with an average of 95 days (range 58-140). The six radial nerve palsies had complete spontaneous recovery. According to the Constant score excellent shoulder function was recorded in 54.6%

Salvage of nonunion and malunion of trauma of the foot and ankle is often challenging surgery. Internal fixation provides the mainstay of most orthopedic surgeons' treatment of these conditions. Patient systemic factors, local factors, and the complex nature of these often multiplanar deformities may make external fixation a more viable option. This article provides an overview of the principles and results of the use of external fixation.

Arthrogryposis is a term used to describe the presence of multiple joint contractures that are present at birth. Reports that describe the experience of a rehabilitation program for adult patients with arthrogryposis are rare. We describe the treatment of an adult patient who had not received previous surgery or rehabilitation. At home, she walked on her knees and elsewhere used a wheelchair. She was submitted to surgery and Ilizarov external fixator to correct the deformities, followed by intensive rehabilitation to permit upright standing and walking with assistive devices. After 4 mos, the

OBJECTIVE: To design a kind of adjustable triangle external fixator, and to use it for treating calcaneal fractures.
METHODS: The external fixator was installed into extremity near the both sides of three Kirschner wires crossing the calcaneal tubercle, the lower end of the tibia and the first cuneiform. The support rod length was adjusted to ensure the Kirschner wires affecting the calcaneal tubercle, so the Böhler angle and anteroposterior diameter of the calcaneus were recovered. From April in 2003 to April in 2008, the external fixators were used to treat 34 patients with calcaneal fractu…

Diabetic patients, and especially elderly patients in a low-nutritional or immunocompromised state, are prone to a variety of opportunistic infections. 'Myiasis' is a term that refers to non-iatrogenic infestation of tissues by larvae, commonly known as maggots, of dipterous flies. Myiasis as a complication of fracture treatment by means of external fixation of long bones has not been reported. We present three diabetic patients, who suffered maggot infestation of their external fixation pin holes, and their outcome. Diabetes, immobilization, poor hygiene and low immune status are predisposing

BACKGROUND: Fracture of the distal radius is a common injury. A surgical treatment is external fixation, where metal pins inserted into bone on either side of the fracture are then fixed to an external frame.
OBJECTIVES: To evaluate the evidence from randomised controlled trials comparing different methods of external fixation for distal radial fractures in adults.
SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (June 2007), the Cochrane Central Register of Controlled Trials, MEDLINE, EMBASE and other databases, conference proceedings and re

There are serious problems with existing methods of treating pilon fractures of the tibia caused by high-energy trauma. The method chosen to treat these fractures should not raise the risk of infection while effectively restoring the joint surface. We successfully treated a 42-year-old male patient with a pilon fracture caused by high-energy trauma using an arthroscopy-assisted unilateral external fixator and minimally invasive internal osteosynthesis. We used arthroscopy to reposition the fracture fragments and restore the joint surface. The fracture fragments were fixed with screws immediate

BACKGROUND: Fracture of the distal radius ('broken wrist') is a common clinical problem. It can be treated conservatively, usually involving wrist immobilisation in a plaster cast, or surgically. A key method of surgical fixation is external fixation.
OBJECTIVES: To evaluate the evidence from randomised controlled trials comparing external fixation with conservative treatment for fractures of the distal radius in adults.
SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (September 2006), the Cochrane Central Register of Controlled Trials, MEDL
