BMP10 reflects pre-capillary pulmonary hemodynamics: association of biomarkers and hemodynamic parameters in pulmonary hypertension.

BACKGROUND AND AIMS: The role of biomarkers in diagnosing pulmonary hypertension (PH) and distinguishing between pre- and post-capillary PH remains poorly understood. We aimed to identify biomarkers with a strong association with mean pulmonary arterial pressure, mPAP (PH diagnosis) and pulmonary vascular resistance, PVR (pre-capillary component), but not with pulmonary arterial wedge pressure, PAWP (post-capillary component). METHODS: Blood samples were collected in patients undergoing right heart catheterization within a prospective cross-sectional study. Biomarkers measured included BMP10,

Diagnosis and treatment guidelines for aberrant portal hemodynamics: The Aberrant Portal Hemodynamics Study Group supported by the Ministry of Health, Labor and Welfare of Japan.

Idiopathic portal hypertension (IPH), causing aberrant portal hemodynamics, is a disease with an as yet unidentified cause and no established treatment protocol. The Japanese research group on IPH in Japan was set up in 1975 by the Ministry of Health, Labor and Welfare. Extrahepatic portal obstruction and Budd-Chiari syndrome (BCS) have since been added to the group's research subjects. The aims of the research group are to accurately evaluate the current status of the three diseases in Japan, elucidate their etiology and pathogenesis, and develop new treatments. Due to the long-term efforts o…

The effects of chronic treatment with octreotide versus octreotide plus midodrine on systemic hemodynamics and renal hemodynamics and function in nonazotemic cirrhotic patients with ascites.

OBJECTIVES: The adrenergic agonist midodrine improved circulatory and renal dysfunction when acutely administered in nonazotemic cirrhotic patients with ascites while its combination with octreotide has recently been proposed as an effective treatment of type 1 hepatorenal syndrome (HRS). However, the effects of octreotide on systemic hemodynamics and renal function in cirrhotic patients are controversial. This study evaluated the effects of chronic treatment with octreotide versus octreotide plus midodrine on systemic hemodynamics and renal hemodynamics, and function in nonazotemic cirrhotic …

Intradialytic non-invasive hemodynamic monitoring: usefulness of cardiac preload values in predicting hemodynamic instability.

INTRODUCTION: Adequate control of patient blood volume in hemodialysis (HD) is essential as a modifiable risk factor for morbidity and mortality. In this study, we propose continuous non-invasive hemodynamic monitoring using bioreactance (Starling SV.Baxter) and real-time characterization of cardiac preload data to aid in the accurate assessment of volume status and improvement of tolerance in HD. METHODOLOGY: Observational and prospective study on the relationship between cardiac preload data and intradialytic hemodynamic instability. Forty-six stable HD patients were recruited. Clinical, an

A Novel Invasive Hemodynamic Parameter (V') to Predict Hemodynamic Response to Mitral Transcatheter Edge-to-Edge Repair.

BACKGROUND: Left atrial (LA) V-wave amplitude can be higher with elevated left ventricular end-diastolic pressure, mitral regurgitation (MR), or both. This may cause hemodynamic and echocardiographic discordance when assessing MR. Identifying more specific hemodynamic correlates to MR severity helps in the setting of discordance. We examined the amplitude of the V-wave relative to the LA mean pressure following the reduction in MR after mitral transcatheter edge-to-edge repair (M-TEER). METHODS: A total of 231 patients who underwent M-TEER between 2019 and 2022 with reliable intraprocedural i

Validation of the Hemodynamic Classification System for Hemodynamic-Related Events in Patients With Durable Left Ventricular Assist Devices.

BACKGROUND: Durable left ventricular assist device (dLVAD) physiology predisposes to right heart failure (RHF) and aortic insufficiency (AI), recently labeled hemodynamic-related events (HDREs). We sought to validate the recently proposed dLVAD Hemodynamic Classification System for HDREs. METHODS: RHF and AI HDREs were adjudicated, staged, and phenotyped on the basis of the Hemodynamic Classification System assessed at 6 months after dLVAD implantation. The primary outcome was a composite of HF hospitalization or cardiovascular death between 6 and 24 months after dLVAD implantation. RESULTS:

Heterogeneity of hemodynamic parameters in untreated primary hypertension, and individualization of antihypertensive therapy based on noninvasive hemodynamic measurements.

Noninvasive measurement of hemodynamic parameter was undertaken in 240 patients with untreated primary hypertension using impedance cardiography (ICG) in outpatient clinics. High output was defined as a cardiac index (CI) >3.6 L/minute/m(2) and high resistance was defined as the total peripheral resistance index (TPRI) >2700 dyne·s·m(2)/cm(5). Of all patients, 67% had high-resistance hypertension (high TPRI with normal or low CI), and 16% had high-output hypertension (high CI with normal TPRI). Treatment with β-blockers for high-output hypertension and with calcium channel blockers for high-re

Precision hemodynamics in the NICU: A phenotype-based functional echocardiography framework.

Functional echocardiography (fECHO) has become a key bedside tool for real-time assessment of neonatal hemodynamics. Unlike conventional monitoring, which relies on indirect surrogates such as blood pressure, fECHO enables direct evaluation of preload, afterload, ventricular performance, cardiac output, and shunt physiology, providing a comprehensive physiologic profile of circulatory status. Neonatal hemodynamic instability is inherently heterogeneous, with similar clinical presentations arising from distinct underlying mechanisms, including preload limitation, myocardial dysfunction, altered…

Association of stenosis and arterial remodeling with cerebral hemodynamics and cognition.

BackgroundAssociations between intracranial stenosis, arterial remodeling, cerebral hemodynamics, including cerebral blood flow (CBF) and arterial transit time (ATT), white matter hyperintensities (WMH), and cognition remain unclear.ObjectiveThis study investigates the relationships between MRA-derived vascular parameters, cerebral hemodynamics, WMH burden, and cognition. We hypothesized that greater stenosis or remodeling is associated with impaired hemodynamics, increased WMH burden, and poorer cognition.MethodsCross-sectional study of 452 memory clinic patients (median age 74 [IQR 68-78], 5…

Repeatability and reliability of retinal arterial hemodynamics measurement using Doppler holography.

SIGNIFICANCE: Reliable quantification of retinal arterial blood flow is important for diagnosing and monitoring ocular and systemic diseases. Existing techniques are limited by invasiveness, motion artifacts, or a lack of quantitative flow estimation. AIM: The aim is to assess the repeatability, reproducibility, and robustness of laser Doppler holography (LDH) for measuring retinal arterial hemodynamics. APPROACH: We acquired LDH data at 67 kHz in healthy volunteers (14 eyes intra-day and 4 eyes inter-day) and quantified blood volume rate, resistivity index (RI), and vessel diameter. Additio…

Sound of nurturing: Advanced photoacoustic and ultrasound imaging of placental hemodynamics.

The placenta is a transient yet vital organ that regulates maternal-fetal exchange and profoundly influences pregnancy outcomes. Placental hemodynamic dysfunction is central to many major obstetric complications, including preeclampsia and fetal growth restriction. Despite advances in obstetric imaging, existing modalities such as Doppler ultrasound, magnetic resonance imaging, and X-Ray computed tomography remain limited in their ability to resolve placental microvasculature or quantify hemodynamics in vivo. Recent developments in photoacoustic (PA) and advanced ultrasound imaging have enable…

Vascular Aging Across the Cardiovascular-Alzheimer's Continuum: Hemodynamics and Artificial Intelligence.

Predicting health trajectories and accurately measuring aging processes across the human lifespan remain profound scientific challenges. Assessing the effectiveness and impact of interventions targeting aging is even more elusive, largely due to the intricate, multidimensional nature of aging-a process that defies simple quantification. Traditional biomarkers offer only partial perspectives, capturing limited aspects of the aging landscape. Yet, over the past decade, groundbreaking advancements have emerged. Epigenetic clocks, derived from DNA methylation patterns, have established themselves

Hemodynamics-guided therapy for hypertensive disorders of pregnancy: a systematic review.

OBJECTIVE: Despite on-going research into the underlying pathology of hypertensive disorders in pregnancy, maternal mortality is hardly decreasing. Current antihypertensive therapy is aimed at controlling symptoms and preventing severe courses of pregnancy. Traditional management focuses on blood pressure (BP) control, but does not consider individual hemodynamic variations. Hemodynamically guided therapy offers a personalized approach that can improve BP control and outcomes by treating the underlying pathophysiology. The aim of this review is to provide an overview of hemodynamically guided
