Surgical repair of prolapsed stoma via the buttonpexy approach: a case series.

Stoma prolapse is a frequent complication following stoma formation in pediatric patients and often necessitates surgical intervention. The buttonpexy technique offers a minimally invasive, low-cost, and anesthesia-sparing alternative to formal stoma revision. This case series highlights its safety and effectiveness in managing pediatric stoma prolapse. Five pediatric patients with stoma prolapse secondary to various underlying conditions, including anorectal malformation, Hirschsprung's disease, and Currarino syndrome, were managed using the buttonpexy technique. In each case, the prolapsed b

The risk of postoperative nausea and vomiting (PONV) in pediatric patients under dental and oral surgery.

BACKGROUND: Postoperative nausea and vomiting (PONV) is a relatively common complication of general anesthesia. We encountered PONV in pediatric patients after dental and oral maxillofacial surgeries under general anesthesia. In particular, there is a higher incidence of PONV due to bleeding of the oral cavity and aspiration of gastric contents. In this study, we retrospectively investigated the risk factors of PONV on the first postoperative day in pediatric patients. METHODS: This study was approved by the Kyushu University Clinical Research Ethics Committee and was conducted retrospectivel

Incidence of Phrenic Nerve Palsy in Pediatric Costoclavicular Brachial Plexus Block.

The costoclavicular brachial plexus block has gained relevance as a safe and effective regional anesthesia technique for upper limb orthopedic surgery in adults, but data in pediatric populations remain limited. This study aimed to evaluate the incidence of phrenic nerve palsy associated with CBPB in pediatric patients. We conducted a descriptive observational study in 30 children undergoing upper limb orthopedic surgery. Diaphragmatic excursion was assessed before and after the block using M-mode ultrasound. No cases of phrenic nerve palsy were observed. Postoperative analgesia was effective,

Determination of the 90% Effective Dose of Remimazolam Continuous Infusion for Preventing Emergence Delirium in Pediatric Patients Undergoing Adenoidectomy or Adenotonsillectomy with Sevoflurane: A Biased Coin Up-and-Down Sequential Trial Design.

BACKGROUND: Emergence delirium (ED) is a common postoperative complication during the recovery period in pediatric anesthesia. Continuous intravenous infusion of remimazolam can effectively prevent the occurrence of ED. However, the optimal dose for preventing ED in pediatric patients remains unclear. This study aimed to determine the 90% effective dose (ED90) of remimazolam during continuous infusion for preventing ED in children. METHODS: This prospective, single-center double-blind trial recruited 52 children aged 3-7 years scheduled to undergo elective adenoidectomy or adenotonsillectomy …

It's time to stop using nitrous oxide for pediatric mask induction.

BACKGROUND: Mask induction of anesthesia for pediatric patients has included the use of nitrous oxide since the inception of pediatric anesthesia. However, the use of nitrous oxide precludes adequate preoxygenation. Additionally, pediatric physiology (less Functional Residual Capacity, higher oxygen consumption), increased risk of laryngospasm and lack of intravenous access increase the risk of a severe airway complication in the event of airway occlusion. Nitrous oxide does not facilitate tranquil mask placement on an unwilling child and does not meaningfully speed mask induction. Exposure to…

Minimizing the Risk of Perioperative Bleeding in a Child with Hemophilia A during Dental Rehabilitation under General Anesthesia: A Case Report.

Hemophilia, among other bleeding disorders, raises concerns for dental service providers who routinely use sharp hand and rotary instruments, address highly vascular soft tissue and provide dental extractions. In pediatric dentistry, dealing with fearful or irritable children increases the possibility of trauma and subsequent bleeding risks in hemophilic pediatric dental patients. In the current report, we discuss how anesthetic, pediatric and dental management may contribute to the delivery of safe and complete dental treatment for such children. This report describes the safe performance of

Early-life midazolam exposure persistently changes chromatin accessibility to impair adult hippocampal neurogenesis and cognition.

Linkage between early-life exposure to anesthesia and subsequent learning disabilities is of great concern to children and their families. Here we show that early-life exposure to midazolam (MDZ), a widely used drug in pediatric anesthesia, persistently alters chromatin accessibility and the expression of quiescence-associated genes in neural stem cells (NSCs) in the mouse hippocampus. The alterations led to a sustained restriction of NSC proliferation toward adulthood, resulting in a reduction of neurogenesis that was associated with the impairment of hippocampal-dependent memory functions. M…

[Diagnostic electroretinography in the young child].

As soon as visual impairment is suspected in a newborn or an infant by an ophthalmologist or pediatrician, it becomes important to document retinal function. Electroretinography (ERG) in general anesthesia as an outpatient procedure allows to assess the functional state of the retina. In addition, we make use of the anesthesia for ophthalmoscopopy, photography, slit lamp examination and occasionally tonometry. Within the first 10 min of dark adaptation we can electroretinographically distinguish between rod- and cone-mediated b-waves. Anesthesia is performed with Ketamine, occasionally supplem

Risk factors for adverse events in children with colds emerging from anesthesia: a logistic regression.

BACKGROUND: Recent upper respiratory infection (URI) in children increases respiratory adverse events following anesthesia for elective surgery. The increased risk continues weeks after resolution of acute URI symptoms. Few systematic analyses have explored specific risk factors. This logistic regression explores the relationship between preoperative URI symptoms and adverse events during emergence from anesthesia. METHODS: Data were combined from control groups of several prospective observational and interventional studies in elective pediatric anesthesia in a tertiary care pediatric hospit…

Evaluation of perfusion index as a predictor of successful caudal block in pediatric patients: A prospective randomized study.

BACKGROUND AND AIMS: Caudal block is among the most widely administered regional anesthesia in pediatric patients. The clinical signs and objective assessments are not fast and reliable enough to provide a good feedback. Perfusion index (PI) is considered as a sensitive marker to assess the efficacy of caudal block. We aim to assess PI as an indicator for success of caudal block in pediatric patients. MATERIAL AND METHODS: Sixty pediatric patients scheduled for elective surgery of lower abdomen and below were included. Patients were randomly allocated into two groups (n = 30): Group 1 receive

[Spinal anesthesia for a inguinal hernia repair in a small child with laryngeal stenosis].

We experienced spinal anesthesia for inguinal hernia repair in combination with general anesthesia in a 4-year-old child with functional laryngeal stenosis and tendency of laryngeal edema. His airway was managed without endotracheal tube or laryngeal mask airway because these devices could worsen the upper airway stenosis. Spinal anesthesia offered reliable and potent analgesia leading to safe anesthetic management under spontaneous breathing. Although spinal anesthesia in combination with general anesthesia is not common in pediatric patients, it is effective and safe to apply for a case requ

Efficacy of two anatomically preformed supraglottic devices, the BlockBuster Laryngeal mask airway and the Ambu AuraGain, for ventilation in anesthetised paralyzed pediatric patients: a randomized trial.

BACKGROUND: The BlockBuster Laryngeal Mask Airway (LMA) provides a higher oropharyngeal seal pressure (OSP) than Ambu Aura Gain (AAG) in children at an intracuff pressure of 60cmH2O. Lower intracuff pressures prevent laryngopharyngeal morbidity; therefore, we compared their OSP at 40cmH2O. METHODS: Seventy children scheduled to undergo elective surgery under general anesthesia were randomly assigned to the AAG (Group A) or BlockBuster LMA (Group B) groups. After device placement, the cuff was inflated to 40cmH2O pressure. Ease of insertion, OSP, number of attempts, insertion time, glottic ali
