Mesenteric Oxygenation Changes Associated With Necrotizing Enterocolitis and Pneumoperitoneum After Multiple Blood Transfusions: A Case Report.

BACKGROUND: The multifactorial pathology and broad clinical presentation of necrotizing enterocolitis (NEC) development in premature infants make prediction of disease onset extremely challenging. Over the past decade, packed red blood cell (PRBC) transfusions have been temporally linked to the development of NEC in severely anemic preterm infants, although this issue is highly controversial. PURPOSE: In this case study, we describe events of an extremely low birth-weight infant who developed NEC complicated by pneumoperitoneum after receiving multiple PRBC transfusions. Specifically, we desc…

Does pneumoperitoneum adversely affect growth, development and liver function in biliary atresia patients after laparoscopic portoenterostomy?

PURPOSE: We assessed the effect of high partial pressure of arterial carbon dioxide (PaCO2) due to pneumoperitoneum (PP) on growth (height/weight) and development (gross/fine motor function, receptive/expressive communication, and social interaction), by comparing outcome after portoenterostomy (PE) for biliary atresia (BA) using laparoscopic PE (LPE: n = 13) and open PE (OPE: n = 13) cases performed between 2005 and 2014. METHODS: Our PE is based on Kasai's original PE. All data were collated prospectively. RESULTS: Differences in duration of follow-up (LPE: 38.8 months; OPE: 38.1 months), …

Does carbon dioxide pneumoperitoneum altering pressure levels lead to ultrastructural damage of fallopian tube and ovary?

AIM: To assess carbon dioxide pneumoperitoneum and its different pressure levels related to cellular injury on ovarian surface epithelium, endothelium, and fallopian tube ciliated epithelium in laparoscopic rat model. MATERIALS AND METHODS: Twenty-four Wistar-Albino female rats were randomized into three groups. Laparotomy was applied for Group 1 (control). Groups 2 and 3 had laparoscopy with pneumoperitoneum pressures at 10 mmHg and 15 mmHg, respectively. After 150 minutes (last 30 minutes was after desufflation for Group 2 and 3) in all groups, bilateral ovariectomy and salpingectomy were p…

Carbon dioxide pneumoperitoneum, intraperitoneal pressure, and peritoneal tissue hypoxia: a mouse study with controlled respiratory support.

BACKGROUND: Animal experiments have suggested that the laparoscopic peritoneal environment is hypoxic. This study aimed to investigate whether peritoneal tissue is hypoxic on a cellular level during a carbon dioxide (CO(2)) pneumoperitoneum at different intraperitoneal pressures (IPPs) and to determine the short-term effects of surgical injury on the hypoxia status of peritoneal tissue in the injured peritoneum and the distant noninjured peritoneum at cellular and molecular levels. METHODS: Experiment 1: Mice were divided into five groups according to the following treatments: anesthesia alon…

Iatrogenic lung overinflation resulting in pneumothorax and pneumoperitoneum during oxygen therapy after general anesthesia: A case report.

RATIONALE: Oxygen therapy is a critical component of postoperative care, particularly after general anesthesia. Although generally safe, its improper administration can lead to serious complications. This report details an incident of lung overinflation during postoperative oxygen therapy that resulted in pneumothorax and pneumoperitoneum. PATIENT CONCERNS: A 63-year-old female patient with tracheostomy underwent pressure sore reconstruction under general anesthesia. Postoperatively, the patient was transferred to the recovery room, and oxygen therapy was initiated. During oxygen administrati…

Symptomatic Pneumoperitoneum After Percutaneous Radiological Gastrostomy in Patient With Duchenne Disease Dependent on Non-Invasive Mechanical Ventilation.

Pneumoperitoneum secondary to gastrostomy is a rare, usually benign complication, often due to air insufflation. It can occur after both endoscopic and radiological techniques, though less frequently with the latter. Percutaneous radiological gastrostomy is preferred in neuromuscular patients where deep sedation poses respiratory risks. We report a case of a Duchenne muscular dystrophy patient on non-invasive ventilation who developed symptomatic pneumoperitoneum, likely exacerbated by aerophagia. We propose minimally managing this with a semi-permanent percutaneous drain, allowing continued v…

Novel association between true surgical pneumoperitoneum secondary to bowel perforations in a newborn and congenital diaphragmatic hernia.

A late preterm male baby with antenatally diagnosed left-sided congenital diaphragmatic hernia (CDH) was delivered electively at 36 weeks of gestation. He remained stable until day 2 of life, when he developed severe persistent pulmonary hypertension secondary to sepsis, including persistent Candida sepsis, delaying CDH repair. On day 13, he developed sudden abdominal distension, and radiography confirmed pneumoperitoneum without pneumothorax. Primary peritoneal drainage released a gush of air, following which exploratory surgery was performed. Intraoperative findings revealed two separate sma…

Can aspiration provide symptom relief in pneumoperitoneum for peritoneal dialysis patients? A comprehensive case presentation and analysis.

INTRODUCTION AND IMPORTANCE: Pneumoperitoneum, the presence of free air in the peritoneal cavity, is typically a concerning finding, often associated with gastrointestinal perforation. However, in peritoneal dialysis (PD) patients, pneumoperitoneum can occur without such severe underlying pathology, frequently due to air entering during dialysis exchanges. CASE REPORT: This case report discusses a 64-year-old male with end-stage renal disease (ESRD) on PD who presented with abdominal pain and distention. The patient reported an incident during a PD session where his dialysis bag became unusua…

Effects of pneumoperitoneum on hemodynamics evaluated by continuous noninvasive arterial pressure monitoring. A single-center observational study.

BACKGROUND AND OBJECTIVES: The induction of pneumoperitoneum (PP) during laparoscopy may cause hemodynamic alterations, especially in patients with unknown cardiovascular diseases. While invasive arterial monitoring could be considered excessive, continuous noninvasive arterial pressure (CNAP) monitoring may allow careful evaluation of hemodynamic variations during laparoscopy. MATERIALS AND METHODS: The objective of this single center observational study was to evaluate hemodynamic changes after insufflation and after deflation of PP with CNAP monitoring. Patients included where adults under…
