Percutaneous/minimally invasive techniques in treatment of femoral shaft fractures with an intramedullary nail.

Intramedullary nailing of the femur is often the treatment of choice in isolated fractures of the femoral shaft. Some surgeons are reticent to use intramedullary nailing for proximal and distal femoral fractures. However, indications for femoral nailing can be appropriately extended to highly comminuted fractures as well as distal and proximal fractures when the correct technique and reduction maneuvers are utilized. Implementation of these techniques will also respect the critical soft tissue envelope that is so important in fracture healing. Deforming forces of the muscles of the hip, knee a

Secular change of the incidence of four fracture types associated with senile osteoporosis in Sado, Japan: the results of a 3-year survey.

We conducted a three-year survey of fracture incidences associated with senile osteoporosis--compression fractures of the spine, femoral neck fractures, distal radius fractures, and fractures of the proximal end of the humerus--to examine secular change. The survey was conducted between 2004 and 2006 on patients in Sado City. We calculated the incidence of each fracture based on the population of Sado City (per 100,000 person-years). Only clinical or incident fractures were diagnosed as new fractures for compression fractures of the spine. Incidence of compression fracture of the spine was the

Biomechanical comparison of inverted triangle and L-shaped screw configurations with medial buttress and anteromedial support plate in Pauwels type III femoral neck fractures.

INTRODUCTION: Surgical stabilization of Pauwels Type III femoral neck fractures remains a significant challenge due to high vertical shear forces. While the medial buttress plate is a recognized solution, it requires extensive deep dissection. This study aims to compare the biomechanical performance of various screw configurations combined with either a medial buttress or anteromedial support plate. MATERIALS AND METHODS: Twenty-five third-generation synthetic femurs were used to create a standardized 70-degree (Pauwels III) fracture model. Specimens were divided into five groups (n = 5): (A)

Femoral neck fracture as an atypical presentation of child abuse.

Femoral neck fractures account for 46% of the proximal femur fractures in children; however, the overall incidence of proximal femur fractures in children is <1%. Pathologic causes for proximal femur fractures include local or global causes of bone weakness, such as metabolic bone conditions or neoplastic causes. Nonpathologic causes of femur fractures are caused almost exclusively by high-energy trauma and are readily identified. Nonaccidental injury leading to femoral neck fracture is unusual and can be challenging to diagnose when the medical history is not forthcoming. Femoral neck fractur

Hip morphology and bone mineral density are associated with hip fracture type in elderly Japanese patients.

BACKGROUND: Femoral neck fractures and trochanteric fractures have distinct biomechanical mechanisms, but the anatomical determinants of fracture type remain incompletely understood. We compared proximal femoral morphology, acetabular morphology, and bone mineral density between fracture types in elderly Japanese patients and evaluated factors associated with fracture type. METHODS: This retrospective study included 194 surgically treated patients (77 trochanteric fractures and 117 femoral neck fractures) between April 2022 and March 2024. Standardized radiographs and computed tomography reco

Outcome Analysis of Dual Implant Osteosynthesis for Ipsilateral Proximal and Shaft Femur Fractures: Do We Need Cephalomedullary Nails?

Introduction Most surgeons prefer a single implant for segmental proximal and diaphyseal femur fractures, although results are controversial and still no consensus for proper management is present. This prospective study analyses the functional and radiological outcome of managing 17 patients with ipsilateral shaft and proximal femur fractures by dual implant osteosynthesis at our center. Methods Over a two-year period, we managed 17 patients with a mean age of 35 years, with cancellous cannulated screws or dynamic hip screws for intracapsular femur fractures and improvised proximal femoral na

Vascular injuries due to cerclage passer: Relevant anatomy and note of caution.

PURPOSE: The cerclage passer instrument was introduced for aiding percutaneous reduction in spiral femur fractures, thereby overcoming the risk of devascularization of the fracture ends commonly associated with traditional open cerclage wiring techniques. This may, however, predispose to iatrogenic vascular injuries owing to the proximity of the vessels and several other factors. We report four cases with femoral artery (superficial and deep) injuries caused by the cerclage passer in proximal femur and distal femur shaft fractures and incidence of these injuries in our institution. METHODS: R

Characteristics and Treatment Strategies for Basicervical and Transcervical Shear Fractures of the Femoral Neck.

This study aimed to define basicervical and transcervical shear fractures using area classification and to determine the optimal osteosynthesis implants for them. The clinical outcomes of 1042 proximal femur fractures were investigated. A model of the proximal femur of a healthy adult was created from computed tomography images, and basicervical and transcervical shear fractures were established in the model. Osteosynthesis models were created using a short femoral nail with a single lag screw or two lag screws and a long femoral nail with a single lag screw or two lag screws. The minimum prin

Key Decision-Making in Post-Failed Internal Fixation of Intertrochanteric Fractures Hip Arthroplasty: A Multicenter Retrospective Study on Fracture Healing's Impact on Femoral Component Selection.

PURPOSE: Following failed internal fixation of intertrochanteric fractures (FIF-ITF), the decision to use a long-stem or standard-stem femoral implant in hip arthroplasty is still debated. This study aimed to explore how the healing status of fractures after FIF-ITF failure affects the choice of femoral stem and clinical outcomes. METHODS: Our retrospective cohort study reviewed 105 cases of patients with FIF-ITF who underwent hip arthroplasty at three tertiary Grade A hospitals between December 2012 and December 2022. We compared the clinical outcomes between patients with healed and unheale

Use of Huckstep nail in the periimplant femoral shaft fracture.

87-year-old female underwent open reduction of distal femoral fracture and internal fixation with locking compression plate and bone graft. She was operated for ipsilateral proximal femoral fractures and stabilized by intramedullary interlocked nail 5 years ago. She developed stress fracture proximal to locked plate. We inserted Huckstep nail after removal of the previous operated proximal femoral nail without removing the remaining plate and screws. At 15 month followup the fractures have united. The Huckstep nail has multiple holes available for screw fixation at any level in such difficult …

CT-based age-related trabecular redistribution in the proximal femur and its association with fracture patterns in elderly individuals with and without diabetes.

Proximal femoral fractures are the most common fall-related fractures in the elderly. In our clinical work, we observed a phenomenon of trabecular redistribution in the proximal femur of older individuals. To investigate the distribution of bone trabeculae (compression and tension trabeculae) in the proximal femur, their changes with age, and their impact on fracture risk, we retrospectively analyzed hip computed tomography (CT) images and measured CT values in six different regions of the proximal femur: the primary compression trabecula, overlapping area, primary tension trabecula, secondary…

Fracture of the lower extremity after total hip replacement.

The incidence of ipsilateral fracture of the lower extremity after total hip replacement (THR) was evaluated in 1442 THRs performed between 1968 and 1983. Twenty-two fractures had occurred, of which 14 were femoral. Twelve of the femoral fractures occurred in women and two in men. The accumulated postoperative risk of femoral fracture during an observation period of 15 years was 25.3 per 1000. Short femoral stems were associated with proximal femoral fractures and long stems with more distal femoral fractures. No specific preoperative diagnosis such as arthrosis, rheumatoid arthritis, or femor…

Review article: Magnetic resonance imaging and computed tomography in the diagnosis of occult proximal femur fractures.

Electronic databases of MEDLINE, EMBASE, CINAHL, and the Cochrane Library as well as the Google Scholar search engine were used. Studies written in the English language highlighting the use of magnetic resonance imaging (MRI) and computed tomography in diagnosing occult proximal femoral fractures despite negative or equivocal plain radiographs were included. Two reviewers independently extracted data from each article. Raw frequencies for each of the details investigated were calculated. 15 prospective and 7 retrospective studies from 1989 to 2009 were included in this systematic review. A tot…

Complete Traumatic Separation of Proximal Femoral Epiphysis in A 2 Year Old Child.

INTRODUCTION: Hip fractures are very common in adults, but are rare in children, comprising less than 1% of all pediatric fractures. Separation of the proximal femoral epiphysis can occur in a child with a traumatic hip dislocation and an open epiphysis. Regardless of the mechanism of proximal femoral epiphyseal separation, the prognosis is poor secondary to the development of osteonecrosis. CASE REPORT: Here we are reporting a case of complete separation of the proximal epiphysis of the femur in a 2 year old female child. She was treated by open reduction and internal fixation by a single k-

The role of lesser trochanter fragment in unstable pertrochanteric A2 proximal femur fractures - is refixation of the lesser trochanter worth the effort?

BACKGROUND: Instability of osteoporotic pertrochanteric fractures is defined by loss of medial/lateral cortical integrity with the posteromedial fragment including the lesser trochanter being pivotal for load distribution. Literature addressing the importance of lesser trochanter refixation is scarce. To clarify the effect of lesser trochanter refixation on primary stability in these fractures, following study was performed. METHODS: 21 femora were match-paired in 3 groups and osteotomized, creating pertrochanteric fractures (AO-31A2). Group 1 was stabilized with a proximal femoral nail, grou

[Complex trauma of the knee joint. Diagnosis--management--therapeutic principles].

The complex nature of combined fractures and soft tissue injuries of the distal femur and proximal tibia needs special attention and specific management. Distal femoral and proximal tibial fractures in young patients are usually caused by high-energy trauma. They are complicated by a high rate of systemic and local injuries to cartilage, ligaments and skin. This small but important group with severe injuries needs a detailed treatment algorithm, because despite the treating surgeon's skill, enthusiasm and wishful thinking, these injuries frequently lead to unsatisfactory results. The combinati

The effect of pre-fracture proximal femur geometry on hip fracture type in elderly patients.

This study aimed to analyze the relationship between fracture type by determining data on the geometry of the proximal femur in the pre-fracture period in patients over 65 years of age who had hip fractures as a result of low-energy trauma. A total of 127 patients who were admitted to the hospital for reasons other than hip pathology within 1 year before the occurrence of hip fracture and who had an anterior-posterior pelvic X-ray were included in the study. Measurements were made to evaluate the proximal femur geometry, neck shaft angle, central edge angle, femoral head diameter, femoral neck
