Role of sonography in assessment of upper extremity nerve pathologies: A literature review.

BACKGROUND AND PURPOSE: High-resolution ultrasound is a useful tool in the imaging of upper extremity nerves and depiction of morphology/structural abnormalities, with the benefits of cost-effectiveness, ease of access and capability of dynamic assessment. The focus of this review article will be sonographic assessment of pathologies of commonly imaged upper extremity nerves: radial, median and ulnar nerves. METHODS: A review of the recent literature on sonography of upper extremity nerves was performed, using databases within PubMed, Google Scholar and OVID as well as a textbook resource. R

Clinical applications of corneal confocal microscopy.

Corneal confocal microscopy is a novel clinical technique for the study of corneal cellular structure. It provides images which are comparable to in-vitro histochemical techniques delineating corneal epithelium, Bowman's layer, stroma, Descemet's membrane and the corneal endothelium. Because, corneal confocal microscopy is a non invasive technique for in vivo imaging of the living cornea it has huge clinical potential to investigate numerous corneal diseases. Thus far it has been used in the detection and management of pathologic and infectious conditions, corneal dystrophies and ecstasies, mo

Management of large high-flow arteriovenous malformations of the shoulder and upper extremity with transcatheter embolotherapy.

PURPOSE: To evaluate the efficacy of transcatheter embolization of arteriovenous malformations (AVMs) of the shoulder and upper extremity. … as achieved. Two patients experienced postembolization skin necrosis, and one experienced permanent radial neuropathy. CONCLUSIONS: Large, high-flow AVMs in the shoulder and upper extremity may be relatively refractory to intravascular treatment because of the diffuse involvement of the soft tissues by the AVM and the lack of a well-defined nidus. Transcatheter em …

Brachial plexopathy as a complication of axillary artery aneurysm in a case of Takayasu arteritis.

Takayasu arteritis is an inflammatory disease of unknown aetiology affecting large vessels. Medium vessel involvement is also well documented; however, neuropathy as a presenting manifestation is rare. In this case report, a young woman in her 20s presented with an 8-month history of intermittent claudication in the right upper limb progressing to rest pain with allodynia in C5-C8 distribution and painless right axillary mass. On examination, she had absent pulses in the right radial, brachial and subclavian artery with audible bruit in the right subclavian and abdominal aorta. CT angiogram sh

Multifocal neuropathy and Horner's syndrome due to disseminated pyomyositis in an intravenous drug user.

BACKGROUND: The formation of abscesses with necrosis within large, striated muscles leads to pyomyositis, a condition relatively rarely encountered outside the tropics. Intravenous drug users and other immunocompromised individuals are predisposed toward this infection, which may occur due to local or haematogenous spread of infection to skeletal muscles previously damaged by trauma, exercise, or rhabdomyolysis. METHODS: We report a young male intravenous drug user with rhabdomyolysis due to use of a synthetic opioid, in whom disseminated pyomyositis was detected following evaluation for scia

Compression and entrapment neuropathies.

Peripheral nerve entrapments are frequent. They usually appear in anatomical tunnels such as the carpal tunnel. Nerve compressions may be due to external pressure such as the fibular nerve at the fibular head. Malignant or benign tumors may also damage the nerve. For each nerve from the upper and lower limbs, detailed clinical, electrophysiological, imaging, and therapeutic aspects are described. In the upper limbs, carpal tunnel syndrome and ulnar neuropathy at the elbow are the most frequent manifestations; the radial nerve is less frequently involved. Other nerves may occasionally be damage

Infraclavicular brachial plexus injury following axillary regional block.

Infraclavicular brachial plexopathy is a potential complication of axillary regional block. We retrospectively reviewed 13 such injuries and found the median nerve most often affected, followed by combined median and ulnar neuropathies, and then by various combinations involving the median, ulnar, radial, and musculocutaneous nerves. All were axon-loss in type and most were severe in degree electrophysiologically. The clinical and electrodiagnostic features of these injuries are strikingly similar to those sustained after axillary arteriography, which has been associated with the medial brachi

[Tendon transfers to restore hand function following peripheral nerve injury in the arm].

Three patients, a woman aged 33 years, a man aged 42 and a woman aged 66, had a loss of hand function. This was due to denervation of a peripheral nerve, in essence the radial, ulnar and median nerves respectively. In the first two cases this was caused by trauma and in the third by compression neuropathy. The problems were loss of wrist and finger extension (a dropping hand) in the first, loss of intrinsic muscle function and thumb adduction in the second and loss of opposition in the third patient. Tendon transfers were performed by diverting a tendon insertion of a still functional muscle t

Medial brachial fascial compartment syndrome: anatomic basis of neuropathy after transaxillary arteriography.

The authors reviewed their experience with 320 transaxillary arteriograms, as well as the English-language literature on neuropathy complicating transaxillary arteriography. Three of their patients had median and ulnar motor and sensory nerve injury, and six others had only sensory involvement. The occurrence or severity of nerve injury did not correlate well with the size or presence of an observable axillary or arm hematoma. Dissection of the axillae and arms of 25 human cadavers revealed a tough medial brachial fascial compartment (MBFC) outside a thin axillary sheath. The median and ulnar

Comparison of Different Sequences of Magnetic Resonance Imaging and Ultrasonography with Nerve Conduction Studies in Peripheral Neuropathies.

BACKGROUND: Peripheral neuropathies refer to a group of disorders in which there is damage to the nerves of the peripheral nervous system. Electrophysiologic studies are the main stay for the diagnosis of peripheral neuropathies. However, direct visualization of the nerves is possible with exact localization of site of disease with high-resolution ultrasonography (USG) and 3-Tesla magnetic resonance imaging (MRI) scanner, and newer magnetic resonance (MR) sequences. METHODS: We performed a cross-sectional study including 55 patients and 64 nerves with upper limb peripheral neuropathies. All p

Comparing iatrogenic radial nerve lesions in humeral shaft fractures treated with helical or straight PHILOS plates: a 10-year retrospective cohort study of 62 cases.

BACKGROUND: Humerus shaft fractures are common and often treated surgically with a proximal humerus internal locking system like the long PHILOS® plate. Due to its close anatomical proximity to the humerus, the radial nerve is particularly susceptible to traumatic and iatrogenic damage. The iatrogenic radial nerve damage associated with internal locking systems is described in about 7% of the cases. In order to avoid this lesion, helical plates have been suggested since 1999. This technique continues to not being used as standard as there is still a clear lack of evidence. This study aimed to

Stingray Envenomation Resulting in Neuropathy and Complex Regional Pain Syndrome Treated With Peripheral Nerve Stimulation: A Case Report.

Stingray stings frequently result in severe but temporary pain; they can occasionally result in significant complications, including peripheral nerve injuries and complex regional pain syndrome (CRPS). CRPS is characterized by severe, persistent pain and functional impairment, often following a nerve injury. Conservative treatments may be insufficient, necessitating advanced management strategies. A 19-year-old patient presented with radial neuropathy and CRPS following a stingray sting. Despite conservative treatment and orthopedic surgery, the patient's condition progressed, resulting in deb…

Sural/Radial Amplitude Ratio in Healthy Adults: Influence of Anatomical and Demographic Variables.

INTRODUCTION: The sural-to-radial sensory nerve action potential amplitude ratio (SRAR) can be used to diagnose peripheral axonal neuropathy. However, different studies have revealed varying lowest values for this index. This study aimed to identify the minimum normal value of SRAR in healthy individuals and evaluate the impact of age, sex, height, weight, body mass index (BMI), forearm length, wrist circumference, ankle circumference, and leg length on the lowest normal value of SRAR to distinguish between healthy individuals and those with axonal polyneuropathy. METHODS: This study involved

The advantages of humeral anteromedial plate osteosynthesis in the middle third shaft fractures.

BACKGROUND: Humeral shaft fractures account for 1.2% of all fractures and occur in a slightly younger population. Their causes include a fall from standing or from height, motor vehicle accident, but can be also pathological. In order to clarify which of both surgeries we performed in our Department for treating humeral shaft fractures had more advantages (anterolateral or anteromedial plating through anterolateral approach) we analyzed incidence of postoperative iatrogenic radial palsies and mean operation time required to complete each surgery. METHODS: During January 1992 to December 2009

The Stiff Elbow Revisited: How Much Motion Can Surgery Really Restore?

Background and Objectives: Post-traumatic elbow stiffness is a disabling condition often resulting from a combination of capsular contracture, osseous impingement, and residual instability, rendering it exceptionally challenging to treat. The aim of this study was to evaluate the outcomes of a comprehensive surgical approach for the treatment of post-traumatic elbow stiffness. Materials and Methods: A retrospective case series of 20 consecutive patients treated between 2018 and 2025 was conducted. Etiologies included distal humeral fractures (n = 9), terrible triad injuries (n = 4), elbow disl

Regulation of Schwann Cell-Axon Interactions in Nerve Development: The Role of Slit2 in Axonal Sorting.

Radial sorting of axons is a critical process in nerve development, ensuring proper segregation of axons to form myelinated and unmyelinated Schwann cell-axon units. This process is regulated by signals mediating communication between Schwann cells and the extracellular matrix, with laminin-211 as a key component. However, the molecular signals involved in directing Schwann cell-axon interactions are less understood, highlighting the need to identify additional molecules that mediate axon recognition and segregation. Gaining a deeper understanding of these mechanisms may shed light on the path
