Structure-Guided Molecular Engineering of a Vascular Endothelial Growth Factor Antagonist to Treat Retinal Diseases.

BACKGROUND: Dystonia is one of the most common forms of movement disorder, caused by the co-contraction of antagonistic muscles, leading to abnormal postures and considerable disability. Non-motor symptoms, notably psychiatric disorders, are well recognised comorbid features of the disorder. However, there is no standardised model for the management of these symptoms in dystonia, with them frequently going undiagnosed and untreated. An internet-based cognitive behavioural therapy programme may provide a future model of care that also maximises available resources. METHODS: This study represen

Stem Cell-Based RPE Therapy for Retinal Diseases: Engineering 3D Tissues Amenable for Regenerative Medicine.

Recent clinical trials based on human pluripotent stem cell-derived retinal pigment epithelium cells (hPSC-RPE cells) were clearly a success regarding safety outcomes. However the delivery strategy of a cell suspension, while being a smart implementation of a cell therapy, might not be sufficient to achieve the best results. More complex reconstructed tissue formulations are required, both to improve functionality and to target pathological conditions with altered Bruch's membrane like age-related macular degeneration (AMD). Herein, we describe the various options regarding the stem cell sourc

The role of placental growth factor (PlGF) and its receptor system in retinal vascular diseases.

Placental growth factor (PlGF) is a member of the vascular endothelial growth factor (VEGF) family. Upon binding to VEGF- and neuropilin-rec … ulating preclinical evidence that indicates that this growth factor could have an important role in retinal diseases. High levels of PlGF have been found in aqueous humor, vitreous and/or retina of patients exhibiting retinopathies, especially those with diabetic retinopathy (DR) and neovascular age-related macular degeneration (nvAMD). Expression of this growth f …

Size, surface charge, and shape determine therapeutic effects of nanoparticles on brain and retinal diseases.

UNLABELLED: Nanoparticles can be valuable therapeutic options to overcome physical barriers to reach central nervous system. Systemically administered nanoparticles can pass through blood-neural barriers; whereas, locally injected nanoparticles directly reach neuronal and perineuronal cells. In this review, we highlight the importance of size, surface charge, and shape of nanoparticles in determining therapeutic effects on brain and retinal diseases. These features affect overall processes of delivery of nanoparticles: in vivo stability in blood and other body fluids, clearance via mononuclear…

Vitreous fluid levels of beta-amyloid((1-42)) and tau in patients with retinal diseases.

PURPOSE: A decrease in beta-amyloid(1-42) (Abeta42) and an increase in tau in the cerebrospinal fluid are reported to be characteristic phenomena in Alzheimer's disease patients. To test the idea that Abeta42 and tau contribute to the development of retinal diseases, we measured Abeta42 and tau concentrations in the vitreous fluid from patients with macular hole (n = 13), diabetic retinopathy (n = 15), or glaucoma concurrent with other ocular diseases (n = 8). METHODS: Vitreous samples were collected from patients who underwent vitrectomy, and sensitive and specific enzyme-linked immunosorben…

Color contrast perimetry. The spatial distribution of color defects in optic nerve and retinal diseases.

Color contrast perimetry was used to evaluate central visual field defects in a group of 28 patients with visual loss resulting from optic nerve or retinal diseases. Kinetic perimetry was performed using colored test objects of constant luminance, equated to a white surround of 10 ft lamberts. Colored test objects were varied in size and in extent of color saturation. Test object color saturation was varied from a white that matched the color and luminance of the adapting background toward either the blue or the red color maxima of a video tangent screen. All central visual field defects that …

Atypical Haemorrhagic Presentation of Neuro-Retinitis and Serous Retinal Detachment Secondary to Cat-Scratch Disease.

Cat-scratch disease (CSD) is a self-limited disease caused by Bartonella henselae, a fastidious gram-negative intracellular bacillus bacterium. Neuroretinitis, a form of optic neuropathy characterised clinically by optic disc swelling and a macular star, is an uncommon manifestation of CSD occurring in approximately 1-2% of cases. We report a case of a 14-year-old female who presented to the emergency department with a chief complaint of acute painless vision loss described as a large black spot in the centre of her right eye vision 2 weeks after being scratched by cats. Fundus examination rev

Adult lead exposure increases blood-retinal permeability: A risk factor for retinal vascular disease.

Low-to-moderate level developmental and adult lead exposure produces retinal dysfunction and/or degeneration in humans and experimental animals. Although high level in vivo or in vitro lead disrupts blood-brain-barrier tight junctions and increases its permeability, the blood-retinal-barrier (BRB) has not been examined. There were four overall goals. First, generate environmentally relevant dose-response models of short-term lead exposure in adult rats. Second, assess retinal histology and functional integrity of the BRB. Third, investigate the transmembrane proteins occludin and claudin-5 as

No Evidence for Retinal Damage Evolving from Reduced Retinal Blood Flow in Carotid Artery Disease.

INTRODUCTION: Carotid artery disease (CAD) comprising high-grade internal carotid artery stenosis (CAS) or carotid artery occlusion (CAO) may lead to ipsilateral impaired cerebral blood flow and reduced retinal blood supply. OBJECTIVE: To examine the influence of chronic CAD on retinal blood flow, retinal morphology, and visual function. METHODS: Patients with unilateral CAS ≥ 50% (ECST criteria) or CAO were grouped according to the grade of the stenosis and to the flow direction of the ophthalmic artery (OA). Retinal perfusion was measured by transorbital duplex ultrasound, assessing centra

Decreased retinal thickness in patients with Alzheimer's disease is correlated with disease severity.

BACKGROUND AND PURPOSE: The loss of retinal ganglion cells observed in Alzheimer's disease (AD) may be attributable to a neurodegeneration of the neuro-retinal structure. Amnestic mild cognitive impairment (aMCI) has been considered a prodromal stage of AD. We evaluated retinal thicknesses in patients with aMCI and AD compared to healthy controls using spectral-domain optical coherence tomography (OCT) to investigate whether changes in retinal thickness are correlated with the clinical severity of dementia. METHODS: Patients with aMCI (n = 14), mild to moderate AD (n = 7), severe AD (n = 9),

[The eye as a mirror of the heart-retinal findings in heart failure and cardiovascular diseases].

BACKGROUND: Due to its unique anatomical and functional characteristics, the retina represents a promising organ for the evaluation of systemic vascular diseases. Modern imaging techniques, particularly optical coherence tomography angiography (OCT-A), enable noninvasive and quantitative assessment of retinal microcirculation. The aim of this review is to present current findings on the significance of retinal biomarkers in cardiovascular diseases, with a focus on heart failure. RESULTS: Numerous studies show associations between retinal vascular changes and arterial hypertension, coronary he

Hierarchy-Aware and Knowledge-Guided Learning for Multi-Label Classification of Retinal Diseases From Fundus Images.

Retinal diseases (RD) are major causes of global vision impairment. Automated diagnosis using fundus images has significant clinical value, particularly in multi-label classification of RD. Recently, hierarchy-aware methods have shown potential in improving classification performance by leveraging hierarchical relationships among disease categories. However, implicit hierarchy-aware methods often fail to capture complex semantic relationships required to make multi-level predictions, while explicit hierarchy-aware methods fail to maintain hierarchy level consistency. Additionally, existing app…
