Ocular findings in the evaluation of abusive head trauma in children.

PURPOSE: Abusive head trauma (AHT) is a form of child physical abuse that can result in major ocular injuries. This study describes the clin … ltiple retinal hemorrhages, 4 (8%) had vitreous hemorrhages, 2 (4%) retinal detachments, and 1 (2%) retinoschisis. CONCLUSION: Fundus examination is an important part of the comprehensive examination of children with suspected AHT; the majority of patients had an abnormal fundus examination with the tendency of bilateral findings.

Retinal hemorrhage variation in inertial versus contact head injuries.

BACKGROUND: Abusive head trauma (AHT) is frequently accompanied by dense/extensive retinal hemorrhages to the periphery with or without retinoschisis (complex retinal hemorrhages, cRH). cRH are uncommon without AHT or major trauma. OBJECTIVE: The study objectives were to determine whether cRH are associated with inertial vs. contact mechanisms and are primary vs. secondary injuries. PARTICIPANTS AND SETTING: This retrospective study utilized a de-identified PediBIRN database of 701 children <3-years-old presenting to intensive care for head trauma. Children with motor vehicle related trauma …

Traumatic Head Injury and the Diagnosis of Abuse: A Cluster Analysis.

OBJECTIVES: Data guiding abusive head trauma (AHT) diagnosis rest on case-control studies that have been criticized for circularity. We wish … urs; respiratory compromise; subdural hemorrhage or fluid collection; and ophthalmologist-confirmed retinoschisis. Variables substantially (P < .001 and OR < 0.10 in any cluster algorithm) more prevalent in cluster 2 were linear parietal skull fracture and epidural hematoma. Postpartitioning analysis revealed that cluster 1 had a high prevalence of physician-di …

Wide field of view swept-source optical coherence tomography for peripheral retinal disease.

BACKGROUND/AIMS: To assess peripheral retinal lesions and the posterior pole in single widefield optical coherence tomography (OCT) volumes. METHODS: A wide field of view (FOV) swept-source OCT (WFOV SSOCT) system was developed using a commercial swept-source laser and a custom sample arm consisting of two indirect ophthalmic lenses. Twenty-seven subjects with peripheral lesions (choroidal melanomas, choroidal naevi, sclerochoroidal calcification, retinitis pigmentosa, diabetic retinopathy, retinoschisis and uveitis) were imaged with the WFOV SSOCT. Volumes were taken in primary gaze. Using t…

Variations in the Formation of Lamellar Macular Hole through Epiretinal Membrane, Macular Pseudohole, and ERM-Foveoschisis.

PURPOSE: To investigate morphological changes associated with lamellar macular hole (LMH). METHODS: We retrospectively reviewed data for all eyes with LMH, as determined by spectral-domain optical coherence tomography (OCT), from patients who had undergone vitrectomy and had at least 12 months of preoperative and 6 months of postoperative follow-up data. Eyes were classified as those with morphological changes in the epiretinal membrane (ERM), macular pseudohole (MPH), or ERM-foveoschisis (ERM-FS) that were observed before LMH formation (group A), and those in which LMH was present at the fir

Spontaneous resolution of foveal detachment in traction maculopathy in high myopia unrelated to posterior vitreous detachment.

BACKGROUND: Foveal detachment associated with foveoschisis usually takes a progressive course, and is associated with a poor visual outcome. The purpose of this study was to report the spontaneous resolution of foveal detachment in patients with myopic traction maculopathy without posterior vitreous detachment. METHODS: A retrospective study involving eight cases of high myopia with foveoschisis and foveal detachment in which the subfoveal fluid had spontaneously resolved. The clinical characteristics and optical coherence tomography (OCT) findings were described. RESULTS: All cases involved

Optical coherence tomography characteristics in eyes with optic pit maculopathy.

PURPOSE: To study the optical coherence tomography characteristics of optic pit maculopathy and propose the anatomical pathophysiology of fluid conduit from pit to macula. METHODS: The charts of 32 patients (32 eyes) with the diagnosis of optic pit maculopathy were reviewed. Clinical variables that were studied included duration of visual impairment, initial visual acuity, and associated fundus features. Optical coherence tomography characteristics including communication with pit, layer of retina involved in schisis, and presence of subretinal fluid were noted. RESULTS: The mean age of the

PURPOSE: To determine the incidence and characteristics of intrachoroidal cavitation (ICC) in the eyes with high myopia. METHODS: In this retrospective, non-interventional, comparative study, we analysed the case records of 108 eyes with and without pathological myopia (PM). PM was defined as having a refractive error (spherical equivalent) of ≥ - 6.0 D or axial length ≥ 26 mm. Evaluating the usefulness in neuro-ophthalmology of visual field examinations peripheral to 30 degrees.

The value of the information obtained from Goldmann manual kinetic perimetry beyond 30 degrees was examined. Of 229 randomly selected patients in a University eye clinic who had visual fields performed for reasons other than glaucoma or ocular hypertension only 3 patients had abnormalities confined to the PVF of one or both eyes. In none of these three patients was the PVF necessary to detect disease (Graves' disease, 2 cases; retinoschisis, 1 case). The PVF was useful in determining the localization of the disorder and/or the therapeutic management in 14 patients of whom 4 of these had retini…
