[Abdominal pain syndrome as the chief symptom of an anxiety and depressive disorder. Successful treatment with inpatient psychotherapy].

OBJECTIVE: Though frequently misinterpreted, gastrointestinal complaints are among the most common psychosomatic syndromes. CASE REPORT: The following report describes the case-history of a 60-year old woman, hospitalized for recurrent abdominal pain and diarrhoea. During the course of parallel internal and psychosomatic examinations an organic cause could be excluded and the psychogenesis of the complaints identified. RESULTS: Once the diagnosis of a somatoform disorder had been established the symptoms were successfully treated within the framework of in-patient psychotherapy. CONCLUSIONS

Effects of psychiatric comorbidity on treatment outcome in patients undergoing diamorphine or methadone maintenance treatment.

BACKGROUND: Comorbid psychiatric disorders among opioid-dependent patients are associated with several negative outcome factors. However, outcomes of maintenance treatment have not been sufficiently established, and no evidence is available with respect to heroin-assisted treatment (HAT). METHODS: For patients in the German heroin trial outcome measures were analyzed for HAT versus methadone maintenance treatment (MMT) both for patients with and without a comorbid diagnosis according to CIDI. RESULTS: 47.2% of the sample had at least one comorbid psychiatric diagnosis, mainly neurotic, stres

Association between temporomandibular disorders and somatization: a narrative review.

Temporomandibular disorders (TMD) are common orofacial pain conditions with multifactorial etiologies. Somatization refers to the manifestation of psychological distress as physical symptoms in the absence of clear medical causes. A growing body of clinical research has recently shown a strong association between TMD and somatization. A substantial proportion of TMD patients exhibit moderate to high levels of somatic symptoms, leading to greater pain intensity, longer disease duration, and heightened psychological distress. The TMD-somatization relationship has been underpinned by complex path

Dissociative Visual Loss in Children and Adolescents.

Psychogenic vision disorders in children and adolescents are a common disorder primarily encountered by ophthalmologists at the onset because, as with other disorders of dissociation, the presentation suggests a neurologic or other somatic condition. Loss of visual acuity, blurred vision and visual field restriction-often described as tunnel vision-appears to be typical. The onset may be sudden, frequently related to a wide range of stressful life events (school failure, family conflicts, accidents). While the majority of these children quickly recover from their symptoms, a substantial percen

Pseudopheochromocytoma.

Pseudopheochromocytoma manifests as severe, symptomatic paroxysmal hypertension without significant elevation in catecholamine and metanephrine levels and lack of evidence of tumor in the adrenal gland. The clinical manifestations are similar but not identical to those in excess circulating catecholamines. The underlying symptomatic mechanism includes augmented cardiovascular responsiveness to catecholamines alongside heightened sympathetic nervous stimulation. The psychological characteristics are probably attributed to the component of repressed emotions related to a past traumatic episode o

Generalized Anxiety Disorder and Pain.

In this article, the co-occurrence of anxiety disorders (in particular generalized anxiety disorder) and pain conditions is described, characteristics of chronic pain are explained, and data on the prevalence of co-comorbidity of both conditions are reviewed. Further, hypotheses on the possible psychosocial and neurobiological backgrounds of the high rate of co-occurrence are discussed. This review will also focus on the role of 'unexplained' pain syndromes (e.g. somatic symptom disorder and fibromyalgia) and anxiety. Finally, we address possible treatment strategies for patients with both con

["Zurich Vertigo Meeting"--phobic postural vertigo].

Phobic postural vertigo has been described as a syndrome that is distinguishable from agoraphobia, acrophobia, and "space phobia". Closely related to locomotion, it is characterized by a combination of nonrotational vertigo with subjective postural and gait instability mainly in patients with an obsessive-compulsive personality. The monosymptomatic disturbance of balance manifests with superimposed attacks that occur with and without recognizable provoking factors in the same patient and are experienced with and without accompanying excess anxiety, misleading both patient and physician to a fa

Imagined halitosis: a social phobia symptom?

Imagined halitosis is poorly documented in the psychiatric literature. It is perhaps best thought of as a symptom rather than as a specific syndrome (a collection of symptoms that co-vary together). Many of the cases with imagined halitosis described in the literature resemble the psychiatric syndrome of social phobia.

Psychological and psychiatric aspects of fibromyalgia syndrome (FMS).

Fibromyalgia patients hardly suffer from major psychiatric illnesses. Most often, persistent somatoform pain disorder (ICD-10) and dysthymia are identified by psychiatric assessment. Features of "pain proneness" can also be found regularly, which can explain the elevated levels of stress observed in FMS. Repeated traumatic experiences during childhood and as adults can be discovered in many cases, which helps to understand some of the difficulties met in psychotherapy with FMS patients. Modified psychotherapy techniques are recommended using pain-centered behavioral methods initially, and prog

[Body Dysmorphic Disorder - Balance between Beauty Mania and Illness].

Body Dysmorphic Disorder - Balance between Beauty Mania and Illness Abstract. The body dysmorphic disorder (BDD) belongs to the somatoform disorders and is characterized by an excessive concern about a slight or not apparent defect in appearance. Typical areas of concern include the face, for example the skin texture or form and symmetry of a particular feature. Besides behavioral pecularities such as excessive checking of the defect in a mirror, the preoccupation causes clinically significant distress and impairment in social functioning. Given a prevalence of up to 20 % among patients seekin…

BACKGROUND: Pruritus is a frequent symptom in the general population and in clinics. OBJECTIVE: This article gives an overview on psychological and psychosomatic aspects of chronic pruritus and tools for their diagnosis. MATERIALS AND METHODS: This is a review. RESULTS: Chronic pruritus can lead to psychological impairment and psychic diseases, e.g., adjustment disorder, depression. Psychological factors can also be important factors in the etiology and course of chronic pruritus, either in combination with somatic aspects or, in the absence of these, as somatoform pruritus or pruritus in c

Dysmorphic concern symptoms and personality disorders: a clinical investigation in patients seeking cosmetic surgery.

Body dysmorphic disorder (BDD) is a somatoform disorder characterized by an excessive concern with an imagined or slight defect in appearance. BDD has been particularly studied in cosmetic surgery settings. The object of the present study is to investigate the relationship between personality disorders and dysmorphic symptoms in a group of 66 patients seeking cosmetic surgery. Assessment instruments included the following: a semistructured interview for demographic and clinical characteristics; the Structured Clinical Interview for DSM-IV, the Hamilton Depression and Anxiety Rating Scales, and

Association between baseline dissociation levels and stress-induced state dissociation in patients with posttraumatic-stress disorder, borderline personality disorder, and major depressive disorder.

INTRODUCTION: Dissociative symptoms are highly prevalent in patients with trauma-related disorders such as borderline personality disorder (BPD) and posttraumatic-stress disorder (PTSD), and also occur in patients with depressive disorders. Acute dissociative states are theorized to be stress-related, and some individuals experience recurring patterns of dissociation. The relationship between the intensity of dissociative episodes (trait-like dissociation) and acute dissociative states, however, is incompletely understood. In the present study, we investigated how levels of baseline (trait-lik

Review of somatic symptoms in post-traumatic stress disorder.

Post-traumatic stress disorder (PTSD) is associated with both (1) 'ill-defined' or 'medically unexplained' somatic syndromes, e.g. unexplained dizziness, tinnitus and blurry vision, and syndromes that can be classified as somatoform disorders (DSM-IV-TR); and (2) a range of medical conditions, with a preponderance of cardiovascular, respiratory, musculoskeletal, neurological, and gastrointestinal disorders, diabetes, chronic pain, sleep disorders and other immune-mediated disorders in various studies. Frequently reported medical co-morbidities with PTSD across various studies include cardiovas…
