Are We Getting Better at Achieving Optimal Lumbar Segmental Sagittal Alignment in Complex Adult Spine Deformity Surgery?

STUDY DESIGN: Retrospective multicenter study. OBJECTIVE: To investigate how advances in spine realignment have impacted lumbar segmental alignment. SUMMARY OF BACKGROUND DATA: The understanding of spine alignment and adult spinal deformity (ASD) management continues to advance. It remains unknown how these advances have influenced lumbar segmental alignment changes in the setting of surgical correction. METHODS: Patients undergoing primary thoracolumbar fusion for ASD were stratified based on enrolment in two distinct multicenter registries; forming an 'Early' (2008-2017) and a 'Late cohor

Which components of the global alignment proportionality score have the greatest impact on outcomes in adult spinal deformity corrective surgery?

PURPOSE: To investigate the impact of the Global Alignment and Proportion (GAP) score components on patient outcomes in Adult Spine Deformity (ASD) surgery. METHODS: Patients included underwent assessment via the GAP score and its individual components: pelvic version (GAP PV), lumbar lordosis (GAP LL), lumbar distribution index (GAP LDI) and spinopelvic component (GAP SP). Multivariable analyses assessed the association between alignment in these components and clinical outcomes in ASD patients. RESULTS: 762 ASD patients met inclusion criteria. Alignment in GAP SP independently predicted me

In inflammatory myopathies, dropped head/bent spine syndrome is associated with scleromyositis: an international case-control study.

BACKGROUND: Some myopathies can lead to dropped head or bent spine syndrome (DH/BS). The significance of this symptom has not been studied in inflammatory myopathies (IM). OBJECTIVES: To assess the significance of DH/BS in patients with IM. METHODS: Practitioners from five IM networks were invited to report patients with IM suffering from DH/BS (without other known cause than IM). IM patients without DH/BS, randomly selected in each participating centre, were included as controls at a ratio of 2 to 1. RESULTS: 49 DH/BS-IM patients (DH: 57.1%, BS: 42.9%) were compared with 98 control-IM pati

Anterior Column Realignment: Analysis of Neurological Risk and Radiographic Outcomes.

BACKGROUND: Anterior column realignment (ACR) is a less invasive alternative to 3-column osteotomy for the correction of sagittal imbalance. We hypothesized that ACR would correct sagittal imbalance with an acceptable neurological risk. OBJECTIVE: To assess long-term neurological and radiographic outcomes after ACR. METHODS: Patients ≥18 yr who underwent ACR from 2005 to 2013 were eligible. Standing scoliosis radiographs were studied at preoperation, postoperation (≤6 wk), and at minimum 2 yr of follow-up. Clinical/radiographic data were collected through a retrospective chart review, with t

An Unusual Cause of Camptocormia.

BACKGROUND: Camptocormia is defined as forward flexion of the spine that manifests during walking and standing and disappears in recumbent position. The various etiologies include idiopathic Parkinson's disease, multiple system atrophy, myopathies, degenerative joint disease, and drugs. CASE REPORT: A 67-year-old diabetic female presented with bradykinesia and camptocormia that started 1 year prior to presentation. Evaluation revealed levosulpiride, a dopamine receptor blocker commonly used for dyspepsia, to be the culprit. DISCUSSION: It is well known that dopamine receptor blockers cause p

Pallidal deep brain stimulation relieves camptocormia in primary dystonia.

Camptocormia, characterised by a forward flexion of the thoracolumbar spine may occur in various movement disorders, mainly in Parkinson's disease or in primary dystonia. In severe cases, patients with camptocormia are unable to walk. While treatment options are limited, deep brain stimulation (DBS) with bilateral stimulation of the subthalamic nucleus or globus pallidus internus (GPi) has been proposed as a therapeutic option in refractory cases of Parkinson's disease. Here we present two patients with severe camptocormia as an isolated form of dystonia and as part of generalised dystonia, re

[Gait disturbances related to dysfunction of the cerebral cortex and basal ganglia].

This review aimed to characterize the gait disturbances in Parkinson disease (PD) and highlight how a rehabilitation program would affect the care of patients with PD. The typical PD gait is a type of hypokinetic gait characterized by reduced stride length and velocity; shortening of the swing phase; and increase in the stance phase, double-limb support duration, and cadence rate. In the advanced phase of PD, start hesitation, shuffling and festinating gait, propulsion, and freezing of gait (FOG) become remarkable. Notably, in PD, attention may influence gait control, and sensory cueing may im

Curvature of cervical vertebra in 8020 achievers observed by lateral cephalogram.

The hypothesis of this research was that elderly people with many remaining teeth and good occlusion (8020 achievers) would be able to maintain proper head and body posture, despite aging. The purpose of this study, as a first stage, was to clarify the aging phenomenon of cervical curvature in 8020 achievers in comparison with that in young adults. Subjects consisted of twenty-eight 8020 achievers, with a mean age of 82.96+/-3.3 years and 26.5+/-4.0 teeth. For comparison, forty adults in their 20's with a mean age of 22.9+/-0.7 years and 28.2+/-0.6 teeth were also enrolled. The cervical verteb

Airway management in a patient of ankylosing spondylitis with traumatic cervical spine injury.

Traumatic cervical lesions compressing the spinal cord pose a significant risk of exacerbating the existing neurological condition during tracheal intubation and subsequent positioning. Preexisting ankylosing spondylitis with spinal column involvement renders the spinal column more rigid and introduces difficulty in airway management of the patient with traumatic cervical spinal cord. To improve ease and success, and reduce cervical spine movement, awake fibreoptic intubation (FOI) is considered the gold standard technique for airway management in such cases. Attaining appropriate position for

Is Developmental Coordination Disorder a Risk Factor for Spinal Malalignment?

Developmental Coordination Disorder (DCD) is one of the most common neurodevelopmental disorders, with a prevalence of approximately 5%-6% among school-aged children (5-11 years). The relationship between spinal deformities and DCD has been explored in various studies, indicating a notable correlation. Thus, the aim of this study was to investigate the association between DCD and spinal malalignment. A literature review was conducted in electronic databases PubMed, Scopus, Science Direct, Google Scholar, and PEDro databases (January-March 2026), for peer-reviewed studies published within the l

Interrater Reliability of Partially Automated Segmentation of Spinal Radiographs in Adult Spinal Deformity Patients.

BACKGROUND: Spinopelvic radiographic measurements are fundamental in the evaluation and treatment of adult spinal deformity (ASD). Measuring spinopelvic parameters has become quicker and more reliable with the development of automated software, such as the Knowledge Environment for Orthopedic Personalized Surgery (KEOPS), but reliability in clinical practice may be limited by variable image quality. The aim of this study is to determine the interrater reliability of partially automated measurement of spinopelvic parameters for patients with ASD using the KEOPS software. METHODS: Representativ

The influence of frailty on outcomes of adult spinal deformity surgery: A systematic review and meta-analysis.

Purpose Adult spinal deformities (ASDs) pose a growing healthcare challenge due to their high prevalence and significant impact on quality of life. Although conservative management is often preferred initially, surgery becomes essential for patients with severe or unresponsive symptoms. However, surgical intervention in elderly populations introduces unique risks due to age-associated comorbidities and the complexity of spinal deformity correction. Recent evidence suggests that frailty could provide more accurate predictions of post-operative outcomes than chronological age alone, yet the exac
