Bioabsorbable sutures versus screw fixation of displaced tibial eminence fractures: a biomechanical study.

BACKGROUND: In 1995, we developed a simple and safe arthroscopic technique of anterograde stable fixation of the displaced tibial eminence fractures using a cannulated screw and washer, allowing immediate mobilisation and weight bearing. Some authors described similar end results by arthroscopic fixation of this abruption with trans-osseous sutures. We proved with the biomechanical study that the fixation with strong trans-osseous sutures provides a comparable stable fixation the same as the cannulated screw and washer. We developed a test machine for cyclic loading and a machine for measuring…

Are patients willing to remove, and capable of removing, their own nonabsorbable sutures?

OBJECTIVES: Providing patients with instructions and equipment regarding self-removal of nonabsorbable sutures could represent a new efficiency in emergency department (ED) practice. The primary outcome was to compare the proportion of patients successfully removing their own sutures when provided with suture removal instructions and equipment versus the standard advice and follow-up care. Secondary outcomes included complication rates, number of physician visits, and patient comfort level. METHODS: This prospective, controlled, single-blinded, pseudorandomized trial enrolled consecutive ED p…

Sutures versus staples for wound closure in orthopaedic surgery: a randomized controlled trial.

BACKGROUND: A recently published meta-analysis comparing metallic staples to sutures in orthopaedic procedures revealed three fold increase in risk for infection in stapled wounds. The studies included in the meta-analysis are at risk of bias due to experimental design limitations. A large randomized controlled trial is proposed to direct orthopaedic surgeons in their choice of wound closure material. METHODS/DESIGN: A parallel group randomized controlled trial with institutional review board approval will be conducted. Patients will be randomized intraoperatively to have skin wounds closed w…

Is Catgut Really Obsolete? Experience with Buried Subcuticular Catgut Sutures in Operative Wounds.

A good scar after surgery is desirable. Skin closure with subcuticular sutures is known to give better scars. Absorbable sutures reduce suture track infection. Catgut is a monofilament absorbable suture, which has not been generally recommended for skin for fear of infection and dehiscence. Buried subcuticular sutures using catgut were given in 350 cases after surgery. Disorders known to impair wound healing were excluded. Infection and dehiscence was noted in only 4.3% cases. Dehiscence in the absence of wound infection was seen in only two cases (0.6%). Excellent healing with fine linear sca…

Dimensional changes between free gingival grafts fixed with ethyl cyanoacrylate and silk sutures.

This study evaluated the dimensional changes in free gingival grafts fixed with ethyl cyanoacrylate or sutures. Twenty-four subjects with gingival recession and absence of keratinized mucosa were divided into two groups: free gingival grafts fixed with ethyl-cyanoacrylate (Group 1) and fixed with sutures (Group 2). Probing depth, clinical attachment level, gingival recession, and dimensional changes of height and width were evaluated immediately post-operatively and at 15, 30, 45, and 90 days after surgery. Results demonstrated that dimensional changes related to the area of gingival graft wer…

Local tissue reactions after nerve repair with ethyl-cyanoacrylate compared with epineural sutures.

Anastomosis of a nerve with cyanoacrylate following a lesion has previously been shown to indicate morphological and functional recovery to an extent comparable to that of conventional epineural sutures. In this study we examined the local tissue reactions after transection and repair of rat sciatic nerve, and compared sutures with a synthetic ethyl-cyanoacrylate adhesive. Many ED-1-immunoreactive macrophages were found accumulating on either side of the repair site whereas neurofilament labelling was less pronounced distal to the repair site seven days after reparation with cyanoacrylate comp…

[Effect of adjustment of corneal running sutures on cylindrical refraction after perforating keratoplasty].

Authors refer about the effect of adjusting (adjustment of the suture tension) of the running corneal suture on the final cylindrical refraction in patients who underwent penetrating keratoplasty due to keratoconus. In our article we evaluate two groups of patients. The group I consists 76 patients (76 eyes) who regularly visited our department of ophthalmology. There were 52.6% of females and 47.4% of males. The average age at the time of surgery was 33.5 +/- 5.2 years (range, 18-52 years). In this group of patients, the adjusting was performed during the surgery. In the group II 35 patients

Fibroblast growth factors lead to increased Msx2 expression and fusion in calvarial sutures.

Craniosynostosis, the premature fusion of the skull bones at the sutures, represents a disruption to the coordinated growth and development of the expanding brain and calvarial vault and is the second most common birth defect that affects the craniofacial complex. Mutations in the human homeobox-containing gene, Msx2, have been shown to cause Boston type craniosynostosis, and we have shown that overexpression of Msx2 leads to craniosynostosis in mice. Activating mutations in fibroblast growth factor (FGF) receptors are thought to cause craniosynostosis in Crouzon, Apert, Jackson-Weiss, Beare-S…

Reconstruction of rat peripheral nerve gap without sutures using freeze-dried alginate gel.

Many materials have been used for artificial tubular prostheses to assist peripheral nerve gap reconstruction. However, the clinical use of these devices has been restricted because a microsurgical procedure requires specialized techniques and expensive equipment, such as operating microscope systems. Therefore the authors developed a new gluing method, without sutures, that uses freeze-dried alginate gel. A 7-mm gap in the sciatic nerve of rats was bridged with freeze-dried alginate gel. Regeneration was evaluated by electrophysiologic testing and histologic study. Eighteen weeks after surger…

[Craniofacial growth changes following premature closure of the bilateral coronal sutures in rabbits].

The study was designed to examine the amount and the direction of craniofacial growth changes following bilateral coronal sutures (BCS) closure in young rabbits. At 2 weeks of age, the BCS were fused by the application of enamel adhesive. These animals and a control group also received amalgam implants on the frontonasal, coronal, and lambdoid sutures to monitor suture growth. At 4 or 8 weeks after suture fusion, separate groups of animals underwent release of fused sutures. Immobilization of BCS resulted in growth cessation of the coronal suture. The normal sutures underwent very rapid growth…

[Results of the treatment of Hirschsprung's disease using Duhamel technique and mechanical sutures].

The original Duhamel procedure for the treatment of Hirschsprung's disease (HD) has undergone several important modifications over the years. The use of mechanical suture in recent years has added significant advantages to this surgical procedure. Sixteen patients with HD were operated on according to this surgical technique, from 1985 to 1990. We analyse clinical, radiological and surgical aspects. We concentrate on early and late postoperative complications and the results achieved after a maximum follow-up of 4.5 years.
