Maternal uniparental disomy for chromosome 14.

We report the first case of maternal uniparental disomy of chromosome 14 in humans. The male proband inherited a balanced 13;14 Robertsonian translocation from his mother. Molecular studies showed that neither chromosome 14 was of paternal origin. The proband is of above average intelligence, but he has hydrocephalus, a bifid uvula, premature puberty, short stature, and small testes. It is not known if the clinical findings are related or coincidental to the uniparental disomy.

[Choanal atresia: review of embryology and pathogenesis].

In the paper a study on the embryology and pathogeny of choanal atresia is presented, with particular reference to its association with other congenital malformations. The experience of the AA., gained through 10 of those cases seen at the ENT-department of the Servasa Hospital, Alicante, is the low incidence of coexisting malformations. They emphasize one case of familiar presentation (mother-daughter), other with eye coloboma and bifid uvula coexistence and a last one with a sensorineural deafness of moderate degree.

A 25-year review of cases with submucous cleft palate.

OBJECTIVES: To evaluate the effect of diagnostic and operative time on velopharyngeal closure in submucosal cleft palate (SMCP). METHODS: S … h a mean diagnostic age of 51.1 months (range: 0-132 months). The clinical presentations were bifid uvula in 4 patients and speech dysfunction in 11. The cases suspected from bifid uvula were diagnosed significantly earlier than those with speech dysfunction (19.5 versus 56.8 months, p<0.01). Velopharyngeal closure was improved in 6 cases, and the avera …

Role of PET/CT in Oropharyngeal Cancers.

Oropharyngeal squamous cell carcinoma (OPSCC) comprises cancers of the tonsils, tongue base, soft palate, and uvula. The staging of oropharyngeal cancers varies depending upon the presence or absence of human papillomavirus (HPV)-directed pathogenesis. The incidence of HPV-associated oropharyngeal cancer (HPV + OPSCC) is expected to continue to rise over the coming decades. PET/CT is a useful modality for the diagnosis, staging, and follow up of patients with oropharyngeal cancers undergoing treatment and surveillance.

Sudden and unexpected death due to unilateral tonsillitis.

Tonsillitis is a relatively well treatable disease. Complications of untreated tonsillitis can even today result in life-threatening complications. The aim of this study is to present a case of a 47-year-old man without previous history of health problems, who suffocated after a brief episode of breathing difficulties. At the forensic autopsy, left-side necrotising tonsillitis with adjacent soft tissue, uvula, and epiglottis swelling and inflammation and significant airway narrowing were found.

Heterotopic neuroglial tissue causing upper airway obstruction in a newborn.

Heterotopic neuroglial tissue, composed of differentiated neuroectodermal tissue, represents developmental heterotopia of neuroglial tissue rather than true neoplasm. Herein, we present a patient with nasopharyngeal heterotopic neuroglial tissue who presented with respiratory distress and feeding difficulty in early days after birth. Magnetic resonance imaging showed a cystic lesion measuring about 3 x 1.5 cm in the nasopharynx near the uvula. The lesion was resected and confirmed histologically as a heterotopic neuroglial tissue.

Obstructive Sleep Apnea: A Case Report.

Obstructive sleep apnea (OSA) is characterized by repeated episodes of upper airway blockage and collapse during sleep accompanied by awakenings with or without oxygen desaturations. During obstructive sleep apnea events, the oropharynx in the back of the throat compresses, causing arousal, oxygen desaturation, or both, leading to fragmented sleep. The hyperplastic uvula is a common clinical finding in patients with obstructive sleep apnea. The various diagnostic and treatment modalities of obstructive sleep apnea are discussed in this article.

Wallenberg's Syndrome: An Unusual Case of Dysphagia.

A 56-year-old man presented with sudden-onset oropharyngeal dysphagia and vomiting of central etiology. Neurological evaluation showed uvula deviation to the left, paresis of the mid-right portion of the soft palate, lateralization of gaze to the right side, and dysphonia. Magnetic resonance imaging (MRI) showed an infarction in the left lateral medullary region, therefore the diagnosis of Wallenberg's syndrome was established. The neurological issues along with the dysphagia gradually improved and the patient was discharged.

Evaluation of acute trismus by MRI: a case report. Bilateral peritonsillar cellulitis.

We describe a rare case of a bilateral peritonsillar cellulitis (PTC). The clinical presentation of fever, trismus and odynophagia was consistent with PTC, more evident on the right side; but the presence of bilateral tonsillar swelling and midline uvula confounded the diagnosis. In spite of the throat examination was performed with a great difficulty due to trismus, the T2 weighted fat saturated STIR magnetic resonance imaging (MRI) guided us to make the diagnosis and to start the intravenous antibiotic treatment immediately.

Massive oropharyngeal papillomatosis causing obstructive sleep apnea in a child.

Obstructive sleep apnea in children is most often secondary to severe adenotonsillar hypertrophy. A 5 1/2-year-old boy presented with loud snoring, increasing dysphagia, nocturnal choking, and apnea. Extensive papillomatosis of the uvula, soft palate, and nasopharynx was found to be causing the obstruction. Although multiple papillomas of the larynx and tracheobronchial tree are well known, the occurrence of extensive papillomas of the oral cavity is rare. The unusual clinical presentation and pathologic significance are discussed.

An infant with streptococcal uvulitis presenting with airway obstruction.

We report a case of 7-month-old infant girl with severe uvulitis with airway obstruction. Her uvula was erythematous and markedly swollen. Stridor and retraction were noted. The symptoms were resolved after treatment with ampicillin, and a throat swab isolated Group A streptococcus. It is extremely rare in children for uvulitis to cause respiratory distress from airway obstruction. However, in infants with narrow airways, physicians should consider the risk of progression to respiratory distress.

A clinical sign to predict difficult tracheal intubation: a prospective study.

It has been suggested that the size of the base of the tongue is an important factor determining the degree of difficulty of direct laryngoscopy. A relatively simple grading system which involves preoperative ability to visualize the faucial pillars, soft palate and base of uvula was designed as a means of predicting the degree of difficulty in laryngeal exposure. The system was evaluated in 210 patients. The degree of difficulty in visualizing these three structures was an accurate predictor of difficulty with direct laryngoscopy (p less than 0.001).

Connections and oculomotor projections of the superior vestibular nucleus and cell group 'y'.

Attempts were made to determine brainstem and cerebellar afferent and efferent projections of the superior vestibular nucleus (SVN) and cell … aterally from ventral and lateral parts of SVN and rostral MVN; and ipsilaterally from the nodulus, uvula and medial parts of the flocculus. Purkinje cells (PC) in medial parts of the flocculus project to central regions of SVN, while PC in the nodulus and uvula appear to project mainly to dorsal peripheral regions of SVN. SVN receives sparse projection …

The pathology of diphtheria.

Diphtheria is an acute, communicable disease caused by Corynebacterium diphtheriae. The disease is generally characterized by local growth of the bacterium in the pharynx with pseudomembrane formation or, less commonly, in the stomach or lungs; systemic dissemination of toxin then invokes lesions in distant organs. Acute disease of the upper respiratory tract usually involves one or more of the following: tonsillar zones, larynx, soft palate, uvula, and nasal cavities. A recent epidemic in Russia emphasized the role of vaccination in reducing disease in children and adults.

Primary malignant melanoma of the oral cavity.

Primary mucosal melanomas of the upper aero-digestive tract are uncommon and represent only 1.7 to 3 per cent of all primary melanomas. The most frequent localizations are the nasal fossae, the paranasal sinuses and the oral cavity where melanomas most often arise from the palate and alveolar ridge, buccal mucosa, lips, tongue and floor of mouth. The authors describe a case of malignant melanoma of the uvula, review the literature and discuss the aetiology, pathology and diagnostic and therapeutic problems.

Cerebellar corticovestibular projections from lobule IX to the descending vestibular nucleus in the cat. A retrograde wheat germ agglutinin-horseradish peroxidase study.

The distribution of Purkinje cells projecting to the descending vestibular nucleus was studied in lobule IX (uvula) of the cat by the retrograde wheat germ agglutinin-horseradish peroxidase method. In the transverse plane labeled Purkinje cells were seen diffusely within 1.0 mm from the midline and densely for 250-500 microns at around 1.0 mm lateral to the midline. Reconstruction of the distribution in the horizontal plane revealed that they were distributed in longitudinal areas extending in the apicobasal extent of lobule IX.

Radiographic studies of upper airway obstruction with cor pulmonale in a patient with pycnodysostosis.

The purpose of this article is to present radiographic aspects of obstructive anatomical alterations of the upper airway before and during sleep in a 4 1/2-year-old boy affected with pycnodysostosis. The striking length of the uvula and the inspiration-exspiration changes in its position was an important factor in the obstruction of the upper airway. For better and earlier understanding of the actual mechanism of upper airway obstruction, the radiological data should be obtained during the same circumstances that the patient is experiencing his maximum discomfort.
