Surgical Outcomes for C2 Tear Drop Fractures: Clinical Relevance to Hangman's Fracture and C2-3 Discoligamentous Injury.

OBJECTIVE: To analyze characteristics of surgically managed tear drop (TD) fractures of the C2 axis associated with other injuries such as hangman's fracture and C2-3 discoligamentous injury as well as treatment outcomes. METHODS: A total of 14 patients (eight men and six women) with TD fractures of the C2 , who were surgically treated at four national trauma centers of tertiary university hospitals from January 2000 to December 2017, were included in this retrospective study. The mean age of the patients was 45.5 years (ranging from 19 to 74 years). The characteristics, surgical treatment me

Epidemiology of Orthopaedic Trauma Admissions From the Emergency Department of a Major Trauma Centre in Rural North India.

Orthopaedic trauma remains a major public health problem, particularly in rural India, where preventive strategies and trauma-care resources are often limited. This single‑centre descriptive observational study aimed to describe the epidemiological profile of orthopaedic trauma patients admitted to a major trauma centre in rural North India, with emphasis on age, sex, mechanism of injury, and pattern of skeletal involvement. This observational study included 2,340 consecutive patients of all ages admitted under orthopaedics with acute traumatic injuries after initial presentation to the emerge

Personalized 3D-printed forearm braces as an alternative for a traditional plaster cast or splint; A systematic review.

Forearm fractures such as distal radius fractures are traditionally treated with a plaster or synthetic cast. Patients commonly report inconvenience of the cast, skin problems, and occasionally radial sensory nerve numbness. A known issue with casting is that the rate of secondary dislocation is high. As an alternative to casts, personalized 3D-printed braces are increasingly used. This review provides an inventory of current developments and experience with 3D-printed forearm braces. Main focus was on the design requirements, materials used, technical requirements, and preclinical and clinica

Occipital condyle fracture caused by minor head trauma.

BACKGROUND: Occipital condyle fractures (OCF) are commonly identified in patients suffering from severe craniocerebral trauma. Here, we present a 57-year-old male whose computed tomography (CT)-documented atlanto-occipital dislocation (AOD), due to just minor trauma was successfully managed with bracing alone. CASE DESCRIPTION: A 57-year-old male presented with the right upper neck pain following a motor vehicle accident. The screening cervical CT scan revealed a fracture of the right occipital condyle, while the subsequent dynamic X-rays showed no instability or AOD. The patient was treated

BLOW-IN FRACTURE OF ORBITAL ROOF. CASE REPORT.

Orbital fractures may be accompanied with severe damage of the eye bulb and other intraorbital tissues. Early clinical findings can be very mild, therefore it is vital to actively seek not only for any damage done to the soft tissues of the orbit, but also for extraorbital complications such as liquorrhea or meningitis. We report a relatively rare case of blow-in fracture of orbital roof in eleven years old boy. Patient was admitted to the emergency care after falling off a bicycle without impaired consciousness. During admission ophthalmologist evaluated the condition as severe contusion of t

Extremely Acute-Onset Cerebral Fat Embolism.

Cerebral fat embolism (CFE) causes the neurological involvement observed in fat embolism syndrome, which is a post-traumatic complication seen mostly after long bone fractures and usually presents 24-72 h after the injury. An early 80s female who sustained an isolated traumatic fracture of the left distal femur without dislocation was alert on admission but fell into a coma 55 min after the injury. Brain computed tomography showed no abnormalities. Brain magnetic resonance imaging was performed approximately 5 h after the accident, and diffusion-weighted images revealed hyperintense, dot-like

Tilt of the radius from forearm rotational axis reliably predicts rotational improvement after corrective osteotomy for malunited forearm fractures.

Forearm rotation occurs around an axis connecting the center of the radial head and the fovea of the distal ulna. The purpose of the present study was to demonstrate the usefulness of the difference between forearm and proximal radial axis in the treatment of malunited forearm fractures. We reviewed the results of eight corrective osteotomies for malunited fractures of the forearm without dislocations of the wrist or elbow. Subjects were 6 men and 2 women (mean age, 15 years; range, 10-21 years). Corrective osteotomy was performed at the fracture site. Preoperatively and at final follow-up, th

Mid-term outcome of reverse shoulder prostheses in complex proximal humeral fractures.

The functional outcome of hemiarthroplasty in displaced proximal humeral 3- and 4-part fractures or fracture dislocations in elderly patients is frequently unpredictable and depends on the position of the prosthesis and tuberosity fixation. Reverse shoulder arthroplasty represents an alternative in elderly patients. The purpose of this study was to report the results of a retrospective series of 30 reverse shoulder prostheses in trauma indications. We also compared the results of a less medialized reverse shoulder prosthesis (Arrow) with those reported for the traditional (Delta III) reverse p…

Traumatic spinopelvic dissociation or U-shaped sacral fracture: a review of the literature.

Traumatic spinopelvic dissociation is a rare high-energy injury pattern, characterised by a transverse sacral fracture in conjunction with bilateral sacral fracture-dislocations. It has a high incidence of neurological complications. The true nature of the injury is easily missed and diagnosis is delayed because it commonly presents in patients with severe associated injuries. In the absence of diagnosis and treatment, it can lead to progressive deformity and chronic pain. Early realignment and fixation of the unstable lumbopelvic junction, with adjunctive decompression of compromised lumbosac

Overuse of concomitant foot radiographic series in patients sustaining minor ankle injuries.

Radiographic series of the foot are often obtained in conjunction with ankle X-rays when the clinical presentation is limited to trauma to the ankle. The Ottawa Ankle and Foot Rules were established in 1992 and serve as reliable guidelines to determine when an ankle or foot series is warranted in patients who have sustained minor ankle and/or foot injury. We retrospectively reviewed radiographic studies of all patients over a period of 18 months who simultaneously had ankle and foot plain radiographs performed for acute complaints limited to the ankle alone. Of the 243 patients who met our inc

[Bilateral fractures of the humerus after hypoglycemic seizure].

HISTORY: An 87 year-old woman with insulin dependent diabetes was referred to our hospital because she had sustained a generalized tonic-clonic seizure. Initial blood sugar concentration before hospital admission was 1.1 mmol/l. INVESTIGATIONS: Conventional radiography and computed tomography of both shoulders confirmed the diagnosis of bilateral proximal fractures of the humerus with posterior dislocation. The cause of the seizure was found to be iatrogenic because timing of application of the dual-acting insulin medication had been wrongly changed, and the patient had developed severe hypog

Pelvis and hip joint injuries as a reconstructive factors in car-to-pedestrian accidents.

The pelvic girdles of 371 pedestrian victims of road traffic accidents were evaluated during postmortem examinations. Additionally, 144 hip joints were opened. The pelvic injuries were found in 28% of the pedestrians hit exclusively in their upright position and 52% of the victims run over by a vehicle. The side of the body hit by a car was determined on the basis of the location of blood suffusions within the subcutaneous tissues and muscles of lower limbs as well as the character of injuries found in the knee and upper ankle joints. The findings were verified with the data from court records

[Fractures of the foot (excluding the talus and calcaneus)].

The submitted retrospective investigation analyzes a group of 108 casualties with injuries of the skeleton of the foot who were hospitalized in the years 1985-90. Excluded was the group of patients with fractures of the talus and calcaneus because they consider problems of these fractures specific. The structure of the group is influenced by the specialty of the authors' workplace--i.e. treatment of multiple injuries. The characteristic of the group, the mechanism of the injury, data concerning time, a summary of the diagnoses, primary treatment and complications are summarized in tables. Twen

[Femoral head preserving therapy in medial femoral neck fractures. A clinical study of the effects of treatment method on the outcome].

The main problem with fractures of the femoral neck is the high incidence of avascular necrosis and non-union. While the rate of pseudarthroses has been lowered by treatment with stable compression screws, no effect of this technique on the incidence of head necrosis is evident. The reasons for this are rupture of vessels, wide dislocation of fragments and diminution of blood supply owing to intracapsular hematoma. Since 1982 we have been using a defined indications and therapy schedule for these fractures, which is based on the main principle of open operative treatment in younger patients. T

[Treatment of missed Monteggia lesion in children--case report].

INTRODUCTION: A Monteggia lesion is a dislocation of the radial head associated with a fracture with the proximal third of the ulna. It is rare in children and the dislocation of the radial head is often missed at the time of injury. There are a lot of described treatment methods: open reduction of the radial head and reconstruction of the annular ligament combined with ulnar osteotomy, the same method without reconstruction of the annular ligament, gradual lengthening and angulation of the ulna by Ilizarov method without the opening of radiocapitelar joint. CASE OUTLINE: A 14-year-old boy ha

An experience with Goel-Harms C1-C2 fixation for type II odontoid fractures.

OBJECTIVE: Type II odontoid fractures need surgical stabilization for disabling neck pain and instability. Anterior odontoid screw fixation is a well-known technique. However, certain patients require posterior fixation. We present our surgical results and experiences with nine cases managed by the Goel-Harms technique. MATERIALS AND METHODS: This is a retrospective review of nine patients operated on between January 2019 and December 2021 for Type II odontoid fractures with posterior fixation technique. Their clinical profile was collected from case files. The radiological data were retrieve

Medial patellofemoral ligament reconstruction in osteogenesis imperfecta using quadriceps tendon graft: a rare case report.

Osteogenesis imperfecta (OI) is a rare connective tissue disorder associated with bone fragility and ligamentous laxity, predisposing patients to recurrent patellar instability. Surgical treatment of recurrent patellar instability is challenging because standard medial patellofemoral ligament (MPFL) reconstruction techniques involving patellar tunneling may increase the risk of patellar fracture. We present a 15-year-old male patient with OI type V and recurrent patellar dislocation treated with a minimally invasive MPFL reconstruction using a pedicled quadriceps tendon graft. This patella-spa
