Psychological mediators of the associations between adverse and benevolent childhood experiences and posttraumatic stress disorder (PTSD) and complex PTSD: A systematic review and meta-analysis.

Adverse and benevolent childhood experiences (ACEs and BCEs, respectively) are uniquely associated with posttraumatic stress disorder (PTSD) and complex PTSD (CPTSD); however, there is no systematic review on the mechanisms of these associations. This systematic review and meta-analysis aimed to synthesize studies considering psychological mediators of the associations between ACEs/BCEs and PTSD/CPTSD. Databases (PsychINFO, PubMed, PTSDpubs, PsycARTICLES, and Web of Science) were searched from inception to August 2025 for peer reviewed studies of adult populations examining psychological media

Reciprocal associations between PTSD symptoms and functioning over time among veteran men and women.

Psychological distress and functional impairment are key diagnostic criteria for posttraumatic stress disorder (PTSD). However, most studies focus on PTSD symptom distress with minimal attention on functioning. The aim of this study was to investigate the temporal dynamics of PTSD symptom severity and global functional impairment, as well as specific areas of functioning (e.g., family, work, parenting, self-care). A gender-balanced sample of 1,461 returning veteran men and women completed measures of PTSD symptom severity and functional impairment at five timepoints between 2010 and 2019. Late

Characterizing injury-related perceived threat trajectories after acute, isolated extremity injuries: A prospective cohort study.

'Perceived threat', or the anticipation of harm/loss associated with injury may influence persistent pain. Changes in perceived threat over time, and its association with pain interference and distress over time are unknown. This study recruited 138 patients (mean (SD) age 41.1(14.8), 65% female) with isolated extremity injuries within three weeks of injury and re-assessed them at 6 weeks, 3 and 6 months. Group-based trajectory modelling was used to identify trajectories of perceived threat, pain interference, anxiety, depression and post-traumatic stress symptoms, and to examine their interre

Circuit-informed modulation of traumatic memory in PTSD: integrating extinction, suppression, and reconsolidation.

Post-traumatic stress disorder (PTSD) persists when stress biases memory systems-broadening threat engrams, weakening context-appropriate inhibition, and rendering extinction fragile. Integrating preclinical and human evidence, we argue that these liabilities are tractable within a circuit-informed framework that aligns behavioral procedures with the biology of fear memory. Extinction can be stabilized by preferentially engaging hippocampal-prefrontal-amygdala pathways and their thalamic coordinators; targeted training and sleep-based reactivation strengthen top-down control over intrusions an

Death Anxiety and Trauma in Forcibly Displaced Children in the Gaza Strip: A Critical Review of Emerging Research, 2024-2025.

PURPOSE OF REVIEW: This review synthesizes recent empirical and theoretical studies from 2024 to 2025 that examine the psychological effects of war, displacement, and siege on Palestinian children in the Gaza Strip. It focuses on the interconnected issues of death anxiety, post-traumatic stress disorder (PTSD), grief, and resilience, aiming to frame mental health outcomes within structural and political contexts rather than within individual pathology. RECENT FINDINGS: Studies indicate that between 70 and 90% of children in the Gaza Strip meet criteria for PTSD or exhibit significant symptoms

Annual Research Review: Interventions for young children exposed to trauma.

The landscape of trauma-focused interventions for young children has evolved significantly, though substantial gaps remain. Early childhood trauma exposure occurs during sensitive periods of brain development with potential lifelong consequences. However, these periods also present unique opportunities for intervention to redirect trajectories toward positive outcomes. Rapid neurodevelopmental changes across early childhood necessitate interventions specifically designed for evolving capacities rather than simply "scaled down" versions of adult treatments. A review focused exclusively on evide

Trajectories and Determinants of Posttraumatic Stress in Disaster Survivors: A Group-Based Trajectory Modeling Study.

BACKGROUND: Disasters can result in long-term posttraumatic stress (PTS), the trajectory of which may vary widely among individuals. Although typical patterns of PTS have been suggested in prior studies, scholarly consensus on pattern classification and related factors remains limited. PURPOSE: This study was developed to identify the general patterns of PTS in disaster survivors over time using trajectory modeling and to examine the characteristics of each trajectory group. METHODS: A secondary analysis was conducted on original data from the Long-term Survey on the Change of Life of Disast

From polyvictimization to heterogeneous sequelae: Four-wave longitudinal investigation of children's complex PTSD symptom trajectories.

Complex post-traumatic stress disorder (CPTSD) is clinically associated with chronic and repeated traumatic exposures during childhood, but related research examining trajectories of CPTSD alongside their risk and protective factors remains critically scarce. This study aims to investigate CPTSD trajectories and identify predictors among de facto unattended children in China. 298 de facto unattended children were assessed at 4 intervals in 2 years on CPTSD symptoms and external (threats and deprivation experiences) and internal associated factors (feelings of loneliness, security, and hope). The role of medical evidence certifying PTSD for legal outcomes in Swedish asylum cases 2016-2020.

Medical evidence is introduced when health conditions are invoked as legally relevant circumstances in asylum cases. Although research shows that medical evidence can affect asylum outcomes, we lack systematic knowledge of how it is assigned legal relevance, especially for mental health conditions. The study draws on 175,394 asylum decisions issued by the Swedish Migration Agency in 2016, 2018, and 2020, selecting cases (N = 1021) referring to medical certificates concerning post-traumatic stress disorder (PTSD). Using a coding scheme, three researchers coded decision texts. The codes were use

From healing to leading: a case study of a community-led mental health program for forcibly displaced people.

There are over 120 million forcibly displaced people in the world, with numbers steadily increasing. The majority of refugees and asylum seekers are displaced in urban settings with fragmented access to health services, and those living in countries without refugee protections face particularly dire circumstances. High rates of trauma exposure and related mental disorders persist alongside structural barriers to care, underscoring an urgent public health equity challenge. This community case study describes the Cope program, a refugee community-led mental health and psychosocial support (MHPSS

Efficacy of 3,4-methylenedioxymethamphetamine (MDMA)-assisted therapy for posttraumatic stress disorder: A systematic review and meta-analysis of clinical and functional outcomes.

Posttraumatic stress disorder (PTSD) is a chronic and disabling condition and identifying beneficial therapies is timely and important. We aimed to estimate the efficacy of 3,4-methylenedioxymethamphetamine-assisted therapy (MDMA-AT) compared with control on clinical and functional outcomes in PTSD. A PRISMA-compliant search (PROSPEROCRD42022353261) up to August 14, 2025, covered nine databases and manual searches to identify randomised controlled trials (RCTs). Methodological quality was assessed using the Cochrane Risk of Bias tool (RoB2), and the certainty of the evidence for each outcome w

The role of genetic predictors in PTSD development.

OBJECTIVE: Aim: To analyse the role of polymorphic sites in genes encoding key regulators of the hypothalamic-pituitary-adrenal axis and the neurotrophic system, which play an important role in determining genetic susceptibility to the development of post-traumatic stress disorder. PATIENTS AND METHODS: Materials and Methods: To prepare this review, a systematic search was conducted in the Scopus, PubMed and ResearchGate databases using keywords relating to the genetic aspects of PTSD development. The analysis included publications from the last ten years that examined molecular-genetic mecha

Incremental Main and Moderating Effects of Mindfulness on the Association Between Trauma and Emotional Distress.

OBJECTIVE: Experiencing trauma is associated with several forms of emotional distress. Although emotional distress has long been expressed in terms of categorical diagnoses, recent research has emphasized the utility of dimensional models of distress. An example of this is the tripartite model, which posits three dimensions of emotional distress: general distress (GD), anxious arousal (AA), and anhedonic depression (AD). One factor associated with problems captured in the tripartite model is mindfulness, which has five facets (observe, describe, acting with awareness, non-judging, and non-reac

Understanding Posttraumatic Growth in Conflict-Affected Children and Adolescents: A Systematic Scoping Review.

PURPOSE: The United Nations High Commissioner for Refugees reports that armed conflicts burden children, comprising nearly half of the population in most war and conflict regions. Despite experiencing trauma from exposure to armed conflict, some children experience positive psychological change as well. The study aims to systematically review existing literature to explore the factors that promote posttraumatic growth in children and adolescents exposed to armed conflict.
