Cardiac resynchronization therapy (CRT) addresses abnormal left ventricular (LV) activation that produces detrimental effects on cardiac systolic and diastolic function. CRT improves symptoms and ventricular performance, promotes reverse remodeling, and decreases mortality and hospitalization in patients with congestive heart failure (CHF). Atrial-synchronized biventricular stimulation reverses many of the temporal delays in mechanical activation associated with LV dysfunction and conduction system disease. The therapy evolved from anecdotal application through surgical implantation of LV paci

Although rare, cardiac amyloidosis is the commonest cause of infiltrative myocardiopathy. The diagnosis may be suspected clinically in patients with mainly right ventricular failure of sudden onset. The aim of this study was to assess the diagnostic value of M-Mode and 2D echocardiography in this condition. Seven cases of cardiac amyloidosis were studied. Biventricular hypertrophy, usually more severe on the left side with reduction in size of the left ventricular chamber, was observed in all cases. Parameters of systolic and diastolic function were abnormal. A significant pericardial effusion

OBJECTIVES: This study evaluated accessory pathway location, its relation to retrograde P wave polarity on the surface electrocardiogram and radiofrequency ablation efficacy and safety in a large group of patients with permanent junctional reciprocating tachycardia.
BACKGROUND: Permanent junctional reciprocating tachycardia is an uncommon form of reciprocating tachycardia, almost incessant from infancy and usually refractory to drug therapy. It is characterized by RP > PR interval and usually by negative P waves in leads II, III, aVF and V4 to V6. Retrograde conduction occurs through an acces

BACKGROUND: Isolated left ventricular apical hypoplasia is a rare and lesser-known form of cardiomyopathy characterized by specific findings on cardiac imaging. It is thought to be congenital in origin, with clinical features that can range from being asymptomatic to presenting with heart failure and arrhythmias.
CASE SUMMARY: A 43-year-old woman presented with chest pain and had previously undergone evaluation at another facility, including a 2D echocardiogram and computed tomography (CT) coronary angiography. These tests revealed a dilated left atrium and ventricle and reduced left ventricu

BACKGROUND: Peripartum cardiomyopathy (PPCM) is a severe complication of pregnancy hallmarked by nonischemic reduction in left ventricular ejection fraction to <45% late in gestation or shortly postpartum. This study evaluated outcomes among women who required mechanical circulatory support (MCS) for end-stage heart failure in the setting of PPCM.
METHODS: Thirty female patients 18-46 years of age underwent advanced heart failure surgical procedures for PPCM between 2012 and 2020. Primary heart transplant patients (n = 2) were excluded. PPCM patients with a new MCS requirement (n = 28) were c

BACKGROUND: Cardiomyopathy and arrhythmias are under significant genetic influence. Here, we studied a family with dilated cardiomyopathy and associated conduction system disease in whom prior clinical cardiac gene panel testing was unrevealing.
METHODS: Whole-genome sequencing and induced pluripotent stem cells were used to examine a family with dilated cardiomyopathy and atrial and ventricular arrhythmias. We also characterized a mouse model with heterozygous and homozygous deletion of Mybphl.
RESULTS: Whole-genome sequencing identified a premature stop codon, R255X, in the MYBPHL gene enc

INTRODUCTION: Gain-of-function mutations within Scn5a, including the DeltaKPQ 1505-1507 deletion in the inactivation domain compromising myocardial repolarization, are implicated in human long QT 3 syndrome (LQT3), associated with ventricular arrhythmogenesis and sudden death.
METHODS AND RESULTS: Patch clamp studies on isolated ventricular Scn5a+/Delta myocytes from DeltaKPQ mice produced by homologous recombination in embryonic stem (ES) cells confirmed such altered electrophysiological properties of the mutant channel. Programmed electrical stimulation (PES) with decremental pacing from th

Optimal management of dextrotransposition of the great arteries with intact ventricular septum is currently an arterial switch procedure performed in the first 2 weeks of life. However, a subgroup of patients presents for surgery beyond this time for reasons of sickness, size, or late referral. Experience with 11 such patients (mean age at first-stage procedure, 4.5 months) has revealed that the left ventricle can be prepared by a surprisingly short interval period (median, 9 days) between a first-stage preparatory operation (pulmonary artery band with or without a shunt) and a subsequent seco

INTRODUCTION: Supraventricular arrhythmias (SVA) are common in mechanically ventilated patients with septic shock and may be aggravated by right ventricular (RV) dysfunction. Optimal rhythm-control strategies in this setting remain uncertain. We explored whether propafenone differs from amiodarone in maintaining sinus rhythm in patients with RV dysfunction.
METHODS: This exploratory sub-analysis of the randomized double-blind PRASE trial included patients with septic shock who developed new-onset SVA, had preserved to moderately reduced left ventricular systolic function (ejection fraction >3

INTRODUCTION: Perioperative myocardial injury evidenced by elevated cardiac biomarkers (both natriuretic peptides and troponin) is common after major non-cardiac surgery. However, it is unclear if the rise in cardiac biomarkers represents global or more localised cardiac injury. We have previously shown isolated right ventricular (RV) dysfunction in patients following lung resection surgery, with no change in left ventricular (LV) function. Given that perioperative RV dysfunction (RVD) can manifest insidiously, we hypothesise there may be a substantial burden of covert yet clinically important

It is unknown whether obesity modifies the effect of obstructive sleep apnea (OSA) and positive airway pressure (PAP) therapy on cardiac remodeling and NT-proBNP (N-terminal pro-B-type natriuretic peptide) levels. We compared NT-proBNP and cardiac magnetic resonance imaging in adults without OSA (n=56) and nonobese (n=73; body mass index <30 kg/m2) and obese (n=136; body mass index ≥30 kg/m2) adults with OSA. We also investigated these traits in nonobese (n=45) and obese (n=78) participants with OSA adherent to 4 months of PAP treatment. At baseline, left ventricular mass to end-diastolic volu

This study was undertaken to test whether multiplane transesophageal echocardiography (TEE) offers advantages in comparison with biplane TEE in the intraoperative monitoring during cardiac surgery. A diagnostic multiplane TEE was performed in 400 patients in the immediate preoperative and postoperative periods. We systematically acquired cardiac images from the gastric fundus, lower esophagus, and upper esophagus; complete views of the descending aorta were also recorded. Usefulness of the different views in providing essential additional clinical information compared with exclusive transverse

BACKGROUND: Lutembacher syndrome (LS) is a rare condition characterized by the coexistence of mitral stenosis (MS) and an atrial septal defect (ASD). Clinical manifestations vary according to the severity of MS and the size of the ASD.
CASE PRESENTATION: We report a 13-year-old female with congenital MS and a secundum ASD who presented with decompensated right heart failure after being lost to follow-up for 10 years. Progressive worsening of MS and enlargement of the ASD resulted in significant left-to-right shunting, severe right heart volume overload, pulmonary hypertension, tricuspid annul

Heart disease in SSCL may be primary or secondary. Primary involvement includes pericarditis, myocardial fibrosis, and contraction band necrosis with congestive cardiomyopathy, conduction system fibrosis, intramural coronary artery lesions and, rarely, valvular disease. Symptoms include those of left and right ventricular failure, chest pain, palpitations, syncope, and sudden death. Pathogenesis may be related to functional Raynaud's phenomenon of the heart and/or structural small vessel disease. Therapy at present is symptomatic; however, new therapeutic approaches are warranted.

BACKGROUND: IntercellularAQ1 adhesion molecules (ICAMs) are released from the endothelium in response to shear stress. Their role in persistent pulmonary hypertension (PPHN) remains un-investigated.
METHODOLOGY: Sixty neonates (30 PPHN, 30 controls) underwent serum ICAM testing and echocardiography.
RESULTS: Right and left ventricular function was depressed in PPHN cases. Tricuspid annular plane systolic excursion was reduced (Cases 8±1 vs. Controls 11±3.5). ICAM was elevated in cases (332±96 vs. 208 ±36).
CONCLUSION: ICAM is a sensitive PPHN marker. Elevated ICAM may result from endothelia

Loss-of-function mutations in the PKP2 gene are associated with arrhythmogenic right ventricular cardiomyopathy (ARVC), a rare cardiac disease associated with a poor prognosis. The search for therapeutics and a better understanding of the molecular mechanisms of the disease require the development of cellular modelling. Using CRISPR/Cas9, we generated a hiPSC line with heterozygous 7-bp deletion in exon 10 of PKP2 (p.H695VfsX5). We demonstrated that hiPSCs were fully pluripotent and showed a high rate of differentiation into cardiomyocytes (iPS-CM). We also showed that PKP2 protein was express

We report a case of atrial functional tricuspid regurgitation with an atrial standstill in a 71-year-old woman with a history of chronic atrial fibrillation (AF). The ECG showed a flat baseline with no AF waves and regular, narrow QRS complexes, whereas the previous ECG demonstrated AF. Echocardiography revealed dilation of the right atrium and the tricuspid annulus with severe regurgitation, but mitral regurgitation was mild. No atrial contraction was detected. Bilateral ventricular function was preserved. Cardiac catheterization showed no pulmonary hypertension and an a-wave in atrial pressu
