Transvesical prostatectomy in elderly patients.

PURPOSE: We assessed the results of transvesical prostatectomy in patients older than 80 years. MATERIALS AND METHODS: We studied 98 patients 80 to 90 years old who underwent transvesical prostatectomy between 1986 and 1993, including those with a large prostate (preoperative estimated weight more than 80 gm.), numerous or large cystolithiasis and large bladder diverticulum, which are indications for open prostatectomy. Clinical data were obtained by chart review. RESULTS: The indications for surgery were urinary retention in 53 patients (54%), severe obstructive urinary symptoms in 18 (18.4…

Persistent Ompahalomesenteric Duct in a Newborn With 16p11.2 Deletion: Report of New Findings.

Omphalomesenteric duct (OMD) is an embryologic communication between the extraembryonic yolk sac and the primitive midgut. The OMD malformations are relatively rare diseases associated with developmental defects related to the closure of the omphalomesenteric duct. These malformations can present in various forms, including cysts, fistulas, or sinuses, and may be either asymptomatic or symptomatic. Meckel's diverticulum is the most common outcome of failed OMD resorption. We report a case of a newborn with a symptomatic OMD malformation associated with a 16p11.2 microdeletion. This is the firs…

Novel Utilization of a Penrose Drain as a Spacer During Temporary Artificial Urinary Sphincter Explantation and Rectourethral Fistula Repair.

A 65-year-old male patient with a history of external beam radiation therapy for prostate cancer and multiple urological surgeries developed a rectourethral fistula after treatment for urethral diverticulum with stones. In managing this complex case, a Penrose drain was utilized as a spacer during artificial urinary sphincter cuff removal to preserve the urethral space for future sphincter re-implantation. This report highlights the novel application of a Penrose drain as a spacer in urological surgery and its benefits in minimizing tissue contraction and preserving urethral health.

Hybrid management of type B aortic dissection in a patient with right-sided aortic arch and aberrant left subclavian artery.

This report describes a patient with a right-sided aortic arch, aberrant left subclavian artery and Kommerell diverticulum, who presented with aneurysmal degeneration of the aortic root to the descending aorta, in addition to an acute type B2-10 aortic dissection. He underwent hybrid treatment with a valve-sparing aortic root replacement, transverse arch replacement with reattachment of the right subclavian artery, bilateral common carotid arteries, and thoracic endovascular aneurysm repair with left subclavian artery embolization and a left common carotid to subclavian artery bypass.

Concurrent double aortic arch and circumflex aorta repair in a symptomatic child: a case report.

Double aortic arch with circumflex aorta is a rare type of vascular ring. Symptoms result from external compression of the trachea and esophagus. The best surgical approach for patients with double arch and circumflex aorta is debated, and options include the highly complex aortic uncrossing procedure. Herein we describe the surgical treatment of a patient with concurrent double aortic arch and circumflex aorta by division of the non-dominant arch and ligamentum arteriosum, plication and posterior tacking of the diverticulum of Kommerell. This left thoracotomy approach provided complete sympto…

Epicardial Ablation of Persistent Junctional Reciprocating Tachycardia in an Infant With Tachycardia-Induced Cardiomyopathy.

A 3-month-old infant who developed persistent junctional reciprocating tachycardia (PJRT)-induced cardiomyopathy that was successfully treated with radiofrequency ablation. To our knowledge this is the youngest reported patient with a successful epicardial lesion placed in a diverticulum off the coronary sinus and also the first report of a PJRT connection located at an epicardial site distinct from the mitral and tricuspid valve annulus. We use this case to highlight how low-power lesions in the coronary sinus in the youngest of patients can achieve results safely. (Level of Difficulty: Advan…

Endoscopic Approaches to Cricopharyngeal Myotomy and Pyloromyotomy.

Endoscopic cricopharyngeal myotomy has been demonstrated to be a safe and efficacious procedure with favorable outcomes for the treatment of cricopharyngeal dysfunction with or without Zenker diverticulum. It is a less invasive approach with decreased morbidity compared with the open approach and minimal reported complications. Peroral endoscopic pyloromyotomy is a novel technique for the treatment of gastroparesis. It has shown promising results in terms of its safety, complication profile, and symptom improvement in a minimally invasive approach that is appealing to many patients. As further…

PATENT VITELLINE DUCT WITH GANGRENOUS SMALL BOWEL PROLAPSE: CASE REPORT AND REVIEW OF LITERATURE.

Vitelline duct anomalies (VDA) are rare complications of persistent omphalomesentric duct or vitelline duct connecting the developing embryo with the yolk sac. VDA can be asymptomatic (detected incidentally) or symptomatic, most common of which is Meckel's diverticulum. A patent vitelline duct is the. most common symptomatic presentation in African children and we present here a four day old neonate with patent vitelline duct with ileal prolapse. The neonate was operated with the patent vitelline duct and gangrenous ileum resected and end to end ileal anastomosis done.

Transverse colon diverticulitis with calcified fecalith.

Left colonic diverticula are common in Western populations, whereas right colonic diverticulosis primarily occurs in Oriental populations. Diverticulitis of the transverse colon is very rare, with very few cases reported in the literature. Herein, we report a case of transverse colon diverticulitis caused by a calcified stone in a 69-year-old female. This was a solitary diverticulum. The signs and symptoms of the disease are similar to acute pancreatitis. To the best of our knowledge, this is the first report describing the MRI findings of a patient with trans-verse colon diverticulitis caused…

Femoral hernia containing strangulated ileum along with the uterus and both ovaries: a first case report.

Strangulated femoral hernia is a common surgical emergency. The contents of the hernial sac commonly include omentum and small bowel, but, occasionally, strangulation of other organs such as Meckel's diverticulum, stomach, ovary, or appendix may occur, resulting in unusual clinical presentations. We report of a case with strangulated femoral hernia containing the uterus and both ovaries, and this is the first report of such a case. We believe that it is important to take into consideration herniation of the uterus and ovaries with omentum or small bowel into the femoral canal to prevent variou…

[Multiple jejunal strictures caused by gastric heterotopia].

A case of gastric heterotopia was discovered incidentally on a jejunal resection specimen in a 42-year-old patient operated for Koenig's syndrome present for 10 years. This anomaly was responsible for seven chronic ulcers with strictures at multiple levels. Gastric heterotopia, especially in the jejunum, is a rare anomaly, except in intestinal duplications and Meckel's diverticulum. The various complications are a direct result of the activity of the gastric glands: hemorrhage, Helicobacter pylori enteritis, perforation, chronic ulcer and obstructive syndrome; malignant adenocarcinomatous dege…

The surgical tactics and technical improvements are discussed against the background of personal experience with some common abdominal operations, among which the appendectomy variant leaving the cecum intra-abdominally and using chain retrograde ligature in some cases, with choice of access according to the requirements of the individual case. Special attention is called to retrocecal extraperitoneal complications and Meckel's diverticulum--clinical picture and operative tactics. Emphasis is laid on the advantages of intestinal anastomoses--latero-lateral variant, termino-terminal technique i…

Female urethral diverticula.

Most urethral diverticula should be diagnosed on clinical grounds alone, but investigations may be used to confirm the diagnosis in difficult cases, delineate the extent of the disease and investigate other entities such as stress incontinence. A high index of suspicion is required when assessing women with lower urinary tract symptoms to ensure that cases of urethral diverticulum are not missed. The cases presented demonstrate that greater awareness of the signs and symptoms of this not uncommon disease may facilitate early diagnosis. Our cases also show that surgical treatment is effective a…

[Digestive hemorrhage undiagnosed by conventional endoscopy].

Digestive bleeding undiagnosed by conventional endoscopies occurs in about 5% of the patients with gastro-intestinal bleeding. Vascular ectasias are the most common cause. More than half of these tumors are benign. Meckel's diverticulum is the cause of bleeding in two thirds of men under the age of 30. Hemorrhage from the biliary tree or pancreatic duct occurs more often in patients with a suggestive history. According to the abundance and the cause of hemorrhage numerous investigations can be undertaken and the diagnostic value of new endoscopic methods (enteroscopy, endoscopic ultrasonograph…

[Pedicle torsion, hemorrhagic ovarian infarct. A rare cause of pediatric acute abdomen].

The most common cause of acute abdomen in a child is acute appendicitis followed by mesenteric lymphadenitis, invagination, strangulation-ileus as a result of volvulus and more rarely perforated Meckel's diverticulum. However even with a child, from a differential diagnosis' aspect, a gynaecological cause should be taken in account too. From time to time one comes across a polycystic-alterated, with twisted lig. ovarii, haemorrhagic and infarctioned ovary without any endocrinological or other pathological irregularities which produces these complaints and symptoms. In the following casuistic s…

Enteroclysis: double contrast examination of the small bowel.

Enteroclysis is a double contrast study of the small bowel. This procedure allows the radiologist to evaluate the small bowel for disease processes including Crohn's disease, Meckel's diverticulum, and malabsorption diseases--all disease processes sometimes missed through single contrast SBFT. Through the use of enteroclysis, the radiologist can detect mucosal fold pattern changes, ulceration of the lumen, dilation of the loops, and intussusception, which can cause an obstruction. Even though the procedure is costly and time consuming, it provides a more accurate evaluation of the small bowel.

Impressions on floor of female bladder: "the female prostate".

The "female prostate" denotes an impression on the base of the female bladder resembling the enlarged male prostate radiographically. Though the finding is widely recognized, confusion exists as to its significance. The differential diagnosis is moderately extensive, and includes retroverted uterus, anterior vaginal tumors, urethral and bladder floor tumors, benign and malignant bony pelvic lesions, urethral diverticulum, and postsurgical changes. Several cases are presented illustrating the previously mentioned causations. An impression on the floor of the female bladder should not be routine…

Bladder base impressions in women: "female prostate".

Bladder base impressions due to prostate hypertrophy are a common urographic finding in older males. A similar appearance may be occasionally seen in females and presents a more difficult diagnostic problem. Sixteen such cases of bladder base defects in females at two institutions were identified. The impressions were caused by symphysis pubis asymmetry, postoperative change, urethral diverticulum, levator ani impression, or "urethral syndrome." Vaginal fibromyoma, ectopic ureterocele, and intramural bladder neoplasm can also cause this defect, although no such cases were found in this series.

Unsuspected latent pairing of the cystic primordium.

We have added another case of duplication of the gallbladder to the 207 collected cases reviewed by Harlaftis et al. It is the first case in which separate drainage into the duodenum is clearly established. Development from a separate diverticulum of the embryonic foregut is proposed by the authors. We agree with other authors that when multiple gallbladders found during rountine intra-abdominal exploration present no symptoms attributable to the biliary tract, no treatment is required. We also agree that any multiple organ found to be pathologic should be removed.
