Derivation and Validation of a Score Using Prehospital Data to Identify Adults With Trauma Requiring Early Laparotomy.

IMPORTANCE: A scoring tool to identify which adults with traumatic injury will require early laparotomy could help improve prehospital triage and system readiness. OBJECTIVE: To develop and validate a prediction model using prehospital information for early laparotomy following trauma. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study analyzed data from the 2017 version of the American College of Surgeons Trauma Quality Improvement Program database. All adult patients with traumatic injury aged 18 years or older who were admitted to a US trauma center in 2017 were included.

Monteggia fractures: analysis of patient-reported outcome measurements in correlation with ulnar fracture localization.

BACKGROUND: Monteggia fractures and Monteggia-like lesions result after severe trauma and have high complication rates. Preliminary biomechanical studies suggested a correlation between ulnar fracture localization and clinical result. OBJECTIVES: Key objective was to evaluate whether the site of the ulnar fracture can be correlated to clinical outcome after open reduction and internal stabilization. METHODS: In a retrospective, monocentric study 35 patients who underwent surgical treatment after suffering a Monteggia injury or Monteggia-like lesion were included. Fractures were classified ac

Lateral Subcutaneous Locking Compression Plate and Small Incision Reduction for Distal-third Diaphyseal Humerus Fractures.

OBJECTIVE: Iatrogenic radial nerve injury is a great challenge for orthopaedic surgeons who deal with distal-third diaphyseal humerus fractures. Conventional open reduction and internal fixation (ORIF) remains the gold standard, but complications such as nonunion and iatrogenic radial nerve injury still occur. We fixed the fractures with a lateral locking compression plate (LCP) subcutaneously after small incision reduction to protect the radial nerve. This study reports the clinical and radiographic outcomes of our modified method. METHODS: Thirty-eight patients with distal-third diaphyseal

Erb's Palsy: Visual Diagnosis in Art before Medical History?

UNLABELLED: <p>Background: Close observation is fundamental to both art and medicine, although both disciplines use it in different ways. As doctors need to practice observation of their patients every day, visual arts are increasingly used for the development of observational skills within the medical curriculum. This is particularly useful for students and residents in surgery or in specializations with a surgical component such as gynecology and obstetrics, where close observation of anatomical structures and radiological imaging is essential in providing quality care. Conversely, artists -

Missed Opportunities in Orthopaedics for Intimate Partner Violence Identification: A Retrospective Review Over 24 years.

BACKGROUND: Intimate partner violence (IPV) commonly manifests as a musculoskeletal injury, yet the majority of orthopaedic surgeons estimate IPV to be rare in "their" orthopaedic patients. This work aimed to (1) provide education on the prevalence and manifestations of IPV, (2) investigate departmental referral patterns to Domestic Abuse Intervention Programs (DAIP) at 2 high-volume quaternary-academic centers, and (3) characterize IPV cases identified by orthopaedic surgery providers. METHODS: The DAIP registry from 2000 to 2024 was queried for patients reporting IPV. Referrals were categor

Medial elbow approaches for coronoid fractures: risk to the ulnar nerve.

BACKGROUND: Coronoid fractures often require open reduction internal fixation (ORIF) to restore elbow stability. The flexor pronator split, flexor carpi ulnaris (FCU) split, and Taylor and Scham (T&S) approaches are frequently used medial approaches to access the coronoid. The ulnar nerve can be released or transposed when performing these exposures. The optimal medial surgical approach and management of the ulnar nerve has not been clearly defined. The purpose of this study was to compare postoperative ulnar nerve complications in coronoid fractures undergoing ORIF following a medial surgical

NovoSorb® Biodegradable Temporising Matrix (BTM) for reconstruction of paediatric acute ischaemic limb injuries secondary to sepsis: A literature review and case series.

BACKGROUND: NovoSorb® Biodegradable Temporising Matrix (BTM) is a synthetic polyurethane bilayer matrix, used in trauma and burns reconstruction. However, there is little evidence of its use for paediatric limb ischaemia. We present a review of literature and three paediatric cases of sepsis-induced ischaemic limb injuries, reconstructed using BTM. METHODS: Data were collected on demographics, management and outcomes of three patients cared for by the senior author and plastic surgery service. The operative regimen and challenges faced are discussed.Three children, aged 1-8 years, were manage

Effects of High-Velocity Elbow Manipulation on Forearm Muscle Electromyographic Recovery in Karting Drivers: A Randomized Within-Participant Sham-Controlled Trial.

Objectives: Karting imposes high neuromuscular demands on the forearm during dynamic steering, gripping and braking. This study examined whether a single high-velocity, low-amplitude (HVLA) manipulation of the elbow acutely modified surface EMG_RMS amplitude and EMG median frequency responses during standardized isometric forearm testing after simulated karting load, rather than EMG activity during dynamic driving itself. Methods: In this randomized, sham-controlled, within-subject trial, 15 drivers completed a single-session within-participant protocol in which one upper limb was randomly all

Management of displaced supracondylar fractures of the humerus using lateral versus crossed Kirschner wires: a prospective randomized controlled clinical trial.

AIMS: We sought to evaluate whether loss of reduction in lateral Kirschner (K)-wiring is noninferior to crossed K-wiring in closed reduction and percutaneous pinning of Type III supracondylar humerus fractures. METHODS: A prospective, randomized, non-inferiority trial was conducted. Emergency department patients aged three to seven years with Type III supracondylar humerus fractures were invited to participate with informed consent. Block randomization allocated patients into one of two pin configuration groups (lateral or crossed). Surgical technique and postoperative management were standar

Sensing and decoding the neural drive to paralyzed muscles during attempted movements of a person with tetraplegia using a sleeve array.

Motor neurons convey information about motor intent that can be extracted and interpreted to control assistive devices. However, most methods for measuring the firing activity of single neurons rely on implanted microelectrodes. Although intracortical brain-computer interfaces (BCIs) have been shown to be safe and effective, the requirement for surgery poses a barrier to widespread use that can be mitigated by instead using noninvasive interfaces. The objective of this study was to evaluate the feasibility of deriving motor control signals from a wearable sensor that can detect residual motor

Distribution and type of crash damage to motorcyclists' clothing: validation of the zone approach in the European Standard for motorcycle protective clothing, EN13595.

OBJECTIVE: Though the use of protective clothing reduces the risk of injury for motorcycle riders, not all protective clothing performs the same in crashes. A European Standard for motorcycle protective clothing (EN13595) was released in 2002 that specifies 4 zones in motorcycle clothing with different levels of protective qualities and 4 different test methods for assessing damage resistance. This project examined damage location and type in clothing worn by riders following a crash to establish the distribution of impact points and validate the zones described in EN13595. METHOD: Data from

Energy attenuation performance of impact protection worn by motorcyclists in real-world crashes.

OBJECTIVE: Laboratory studies have demonstrated that impact protectors (IP) used in motorcycle clothing can reduce fracture severities. While crash studies have reported IP are associated with reduced likelihood of soft tissue injury, there is little evidence of their effectiveness in reducing fracture likelihood. This discrepancy might be related to IP quality. There are mandatory requirements for IP supplied with protective clothing in Europe, but not elsewhere. This study examines the energy attenuation performance of IP used by Australian riders. METHODS: IP were harvested from clothing w

Orthobiologic Interventions Using Ultrasound Guidance.

The application of regenerative therapies for the treatment of musculoskeletal conditions has emerged over the last decade with recent acceleration. These include prolotherapy, platelet-rich plasma, and mesenchymal stem cell therapy. These strategies augment the body's innate physiology to heal pathologic processes. This article presents an overview of platelet-rich plasma and mesenchymal stem cell therapy for the treatment of musculoskeletal injuries. A brief literature review is included, as are techniques for the use of ultrasound guidance to assist with these procedures.
