Body composition assessment for risk stratification of hepatic steatosis in patients with type 2 diabetes and MASLD undergoing osteoporosis screening.

BACKGROUND AND AIMS: Metabolic dysfunction-associated steatotic liver disease (MASLD) is the most common cause of chronic liver disease worldwide. Type 2 diabetes (T2D) and obesity are major risk factors for its development and progression. Body mass index (BMI), although widely used in clinical practice, fails to adequately reflect individual differences in body composition and fat distribution. This study evaluated the association of adiposity and reduced muscle mass - measured by dual-energy X-ray absorptiometry (DXA) - with hepatic steatosis severity in patients with MASLD and T2D. METHOD

Primary hyperparathyroidism presenting as atraumatic bilateral hip fractures: a case report.

BACKGROUND: Primary hyperparathyroidism (PHPT) affects up to 1.5% of older adults, causes bone loss at cortical sites and increases fracture risk. However, atraumatic hip fractures due to PHPT are rare. We report the case of an older female diagnosed with PHPT after presenting with atraumatic bilateral neck of femur fractures. CASE PRESENTATION: An 82-year-old white woman was referred to the emergency department with progressive left hip pain over two months and was unable to bear weight on presentation. There was no history of trauma, falls, prior fracture or diagnosis of osteoporosis. X-ray

Sex Differences in BP From an Automated Oscillometric Compared With Manual Device.

BACKGROUND: Automated blood pressure (BP) devices may be less accurate in females than males, but this requires further investigation. This study aimed to determine sex differences in automated BP, measured with a single brand and model of device, compared with manual BP, with a focus on cuff sizes and associations with measures of adiposity. METHODS: Automated (Omron HEM-907XL) and manual BP were taken sequentially in a random order among a subsample of participants attending the US National Health and Nutrition Examination Survey, 2017 to 2018. Anthropometry and dual-energy x-ray absorptiom

Impact of Potassium-Binders on the Interpretation of Bone Mineral Density Following Kidney Transplantation.

BACKGROUND: Following kidney transplantation (KTx), episodes of hyperkalemia are common and effectively treated with sodium zirconium cyclosilicate (SZC, Lokelma) or patiromer (PAT, Veltassa). The presence of SZC/PAT in the gastrointestinal tract may, if radio dense, influence the interpretation of lumbar spine (LS) dual-energy x-ray absorptiometry (DXA)-scans. We investigated the effect with/without PAT/SZC in KTx on LS DXA readings. METHODS: A prospective, single-center study of KTx recipients in need of potassium-lowering therapy was performed. All participants performed two LS DXA-scans,

Prevalence and impact of preoperative osteoporosis on healthcare utilization and patient-reported outcomes in primary total knee arthroplasty.

PURPOSE: Osteoporosis is a well-recognized risk factor for complications after total knee arthroplasty (TKA). However, the effect of pre-TKA osteoporosis on healthcare utilization and patient-reported outcomes is poorly understood. Here, we characterize the association between pre-TKA osteoporosis and (1) healthcare utilization and patient-reported pain and function outcome measures; and (2) dual X-ray absorptiometry (DEXA) scan T-scores and the aforementioned outcomes. METHODS: A prospective cohort of primary elective TKA patients between July 2015 and January 2020 was obtained (n = 6318), o

Association between bone mineral metabolism and vascular calcification in end-stage renal disease.

BACKGROUND: Development of vascular calcification is accelerated in patients with end-stage renal disease. In addition to traditional risk factors of cardiovascular disease (CVD) abnormal bone and mineral metabolism together with many other factors contribute to the excess cardiovascular burden in patients on dialysis. Aortic calcification score and coronary calcification score are predictive of CVD and mortality. The aim of this study was to evaluate the possible relationship between arterial calcification and bone metabolism. METHODS: Thirty two patients on dialysis were included. All patie

A prospective case-control pilot study to evaluate bone microarchitecture in children and teenagers on long-term parenteral nutrition using HR-pQCT.

Long-term parenteral nutrition (PN) may induce bone complications. Tridimensional bone imaging techniques such as high-resolution peripheral quantitative computed tomography (HR-pQCT) allow the assessment of both compartmental volumetric densities and microarchitecture. Our aim was to evaluate these parameters in children and teenagers receiving long-term PN. This cross-sectional, case-control study included children older than 9 years undergoing PN for at least 2 years. They were age-, gender- and puberty-matched with healthy controls (1:2). Evaluation included biological assessment of bone m

Interleukin 17A: a Janus-faced regulator of osteoporosis.

Interleukin (IL)-17A is a well-described mediator of bone resorption in inflammatory diseases, and postmenopausal osteoporosis is associated with increased serum levels of IL-17A. Ovariectomy (OVX) can be used as a model to study bone loss induced by estrogen deficiency and the role of IL-17A in osteoporosis development has previously been investigated using various methods to inhibit IL-17A signaling in this model. However, the studies show opposing results. While some publications reported IL-17A as a mediator of OVX-induced osteoporosis, others found a bone-protective role for IL-17 recepto

Peri-prosthetic bone mineral density after simultaneous bilateral total knee arthroplasty under oral bisphosphonate therapy - A comparison between mobile- and fixed-bearing prostheses.

BACKGROUND: Peri-prosthetic bone mineral density (BMD) decreases after total knee arthroplasty (TKA). We aimed to specifically compare peri-prosthetic BMD changes between mobile- and fixed-bearing prostheses in patients undergoing oral bisphosphonate therapy, hypothesizing that mobile-bearing components would have a favorable effect on postoperative peri-prosthetic BMD. METHODS: This prospective cohort study investigated 30 patients who underwent simultaneous bilateral TKA for primary knee osteoarthritis between December 2007 and September 2012. All patients underwent mobile-bearing TKA in on

High baseline fat mass, but not lean tissue mass, is associated with high intensity low back pain and disability in community-based adults.

OBJECTIVES: Low back pain is the largest contributor to disability worldwide. The role of body composition as a risk factor for back pain remains unclear. Our aim was to examine the relationship between fat mass and fat distribution on back pain intensity and disability using validated tools over 3 years. METHODS: Participants (aged 25-60 years) were assessed at baseline using dual-energy X-ray absorptiometry (DXA) to measure body composition. All participants completed the Chronic Pain Grade Scale at baseline and 3-year follow-up. Of the 150 participants, 123 (82%) completed the follow-up.

Fat redistribution and accumulation of visceral adipose tissue predicts type 2 diabetes risk in middle-aged black South African women: a 13-year longitudinal study.

BACKGROUND: Cross-sectional studies in South Africa (SA) have shown that black SA women, despite being more insulin resistant, have less visceral adipose tissue (VAT) and more subcutaneous adipose tissue (SAT) than white women. This study aimed to investigate whether baseline and/or change in body fat and its distribution predict type 2 diabetes (T2D) risk in middle-aged black SA women, 13 years later. METHODS: We studied 142 black SA women who are the caregivers of the Birth-to-Twenty plus cohort, and who had normal glucose tolerance (NGT) at baseline. At baseline and follow-up, fasting bloo
