The influence of elbow position on the range of motion of the wrist following transfer of the brachioradialis to the extensor carpi radialis brevis tendon.

BACKGROUND: In patients who have an injury of the cervical spinal cord, the brachioradialis tendon may be transferred to the extensor carpi radialis brevis tendon to restore voluntary wrist extension. We hypothesized that the active range of motion of the wrist depends on the position of the elbow after this transfer because the brachioradialis changes length substantially during elbow flexion, which implies the maximum force that the muscle can produce varies with elbow position. The objectives of this study were to determine whether the position of the elbow influences the range of motion of…

Influence of an artificial cervical joint compared with fusion on adjacent-level motion in the treatment of degenerative cervical disc disease.

OBJECT: The authors report the preservation of motion at surgically treated and adjacent spinal segments after placing an artificial cervical joint (ACJ) and they describe the influence of interbody fusion on changes in angulation occurring in the sagittal plane at adjacent levels in the treatment of cervical spondylosis. METHODS: The authors conducted a prospective nonrandomized study of patients in whom an ACJ was placed or autologous bone graft interbody fusion was performed. Angular measurements at levels adjacent to that surgically treated were calculated using plain flexion-extension ra…

Dynamic glenohumeral stability provided by the rotator cuff muscles in the mid-range and end-range of motion. A study in cadavera.

BACKGROUND: Both static and dynamic factors are responsible for glenohumeral joint stability. We hypothesized that dynamic factors could potentially operate throughout the entire range of glenohumeral motion, although capsuloligamentous restraints (a static factor) have been thought to be primarily responsible for stability in the end-range of motion. The purpose of this study was to quantitatively compare the dynamic glenohumeral joint stability in the end-range of motion (the position of anterior instability) with that in the mid-range by investigating the force components generated by the r…

The selectivity and timing of motion processing in human temporo-parieto-occipital and occipital cortex: a transcranial magnetic stimulation study.

An extrastriate visual area near the human temporo parieto occipital junction (TPO) may selectively mediate motion processing, while contributing little to the perception of color or form. This TPO area may be the human analogue of the monkey middle temporal (MT or V5) and medial superior temporal (MST) extrastriate visual areas. The selectivity of the effect of transcranial magnetic stimulation (TMS) on motion processing was unknown, as was the timecourse of occipital to TPO motion processing. In the first experiment, unilateral TMS was delivered over TPO 50-250 ms after the onset of a random…

Early motion of the ankle after operative treatment of a rupture of the Achilles tendon. A prospective, randomized clinical and radiographic study.

BACKGROUND: Different regimens of early motion of the ankle after operative treatment of a ruptured Achilles tendon have been suggested since the late 1980s. However, as far as we know, no controlled studies comparing these regimens with conventional immobilization in a cast have been reported. METHODS: In a prospective study, seventy-one patients who had an acute rupture of the Achilles tendon were randomized to either conventional postoperative management with a cast for eight weeks or early restricted motion of the ankle in a below-the-knee brace for six weeks. The brace was modified with …

Functional blockade of the parabrachial area by tetrodotoxin disrupts the acquisition of conditioned taste aversion induced by motion-sickness in rats.

The role of the parabrachial area in conditioned taste aversion (CTA) induced by motion-sickness was studied in male Wistar rats. In the first experiment, one-trial conditioned taste aversion, to a 0.5% decaffeinated coffee solution, was induced by 30 min of vertical rotatory motion (80 rev./min) in intact rats. In the second experiment, reversible blockade of the neural activity in various brainstem sites was induced by bilateral intracerebral injections of tetrodotoxin (TTX) (10 ng/microl) after conditioning. Blockade of the parabrachial area, but neither A8 nor lateral vestibular nucleus, d…

The 3-D motion of the centre of gravity of the human body during level walking. II. Lower limb amputees.

OBJECTIVE: To analyse the motion of the centre of gravity (CG) of the body during gait in unilateral lower limb amputees with good kinematic patterns. DESIGN: Three transtibial (below-knee, BK) and four transfemoral (above-knee, AK) amputees were required to perform successive walks over a 2.4 m long force plate, at freely chosen cadence and speed. BACKGROUND: In previous studies it has been shown that in unilateral lower limb amputee gait, the motion of the CG can be more asymmetric than might be suspected from kinematic analysis. METHODS: The mechanical energy changes of the CG due to its mo…

Early transvaginal ultrasound following an accurately dated pregnancy: the importance of finding a yolk sac or fetal heart motion.

Our goals were to determine the prognostic value of a yolk sac or fetal heart motion seen during an early accurately dated transvaginal ultrasound (TVU). We reviewed 225 consecutive pregnancies for fetal heart motion data. Furthermore, 63 pregnancies following in-vitro fertilization were reviewed for yolk sac information. The TVU was performed between 5 and 6 weeks following presumed conception (heart motion data) and between 22 and 32 days following in-vitro fertilization (yolk sac data). Pregnancies were followed until an ongoing pregnancy or spontaneous abortion was documented. The presence…

Comparison of the effects of a selective muscarinic receptor antagonist and hyoscine (scopolamine) on motion sickness, skin conductance and heart rate.

AIMS: Hyoscine (scopolamine), which is effective in the prophylaxis of motion sickness, shows similar binding affinities to all of the five known muscarinic receptor sub-types. The effectiveness of hyoscine was compared with zamifenacin (UK-76654), which binds selectively to the muscarinic M3 and m5 receptors. METHODS: Eighteen subjects received hyoscine hydrobromide 0.6 mg, zamifenacin 20 mg, or placebo (double-blind cross-over design). Sessions were 1 week apart and the drug (oral) was given 90 min prior to a motion sickness test. Motion sickness was elicited by cross-coupled stimulation on…

Influence of hamstring length on the standing position and flexion range of motion of the pelvic angle, lumbar angle, and thoracic angle.

Hamstring muscle length may be associated with specific pelvic and trunk postures, but the influence of hamstring length on trunk postures is not well documented. The purposes of this study were to examine the influence of hamstring length on 1) the pelvic angle, lumbar angle, and thoracic angle in standing; and 2) the flexion ranges of motion of the pelvic angle, lumbar angle, and thoracic angle during the toe-touch test. Surface platform pointers over bony landmarks and photography were used to examine the pelvic and trunk angles and ranges of motion of 10 men with short hamstrings, 10 men w…

[Musical systolic murmur produced by oscillation of the systolic anterior motion of the mitral apparatus: relation to the genesis of Still's murmur].

The genesis of a musical systolic murmur produced by systolic anterior motion (SAM) of the mitral apparatus was investigated in four patients using phonocardiography and echocardiography. Two patients (Case 1 and 3) had hypertrophic cardiomyopathy (one, the obstructive type; the other, the nonobstructive type) and the remaining two (Case 2 and 4) had redundant chordae tendineae. 1. In every patient, regular oscillation of the SAM was observed, coinciding in time with the musical systolic murmur, which was simultaneously recorded. The fundamental frequency of the musical systolic murmur was rec…

Extent of mitral calcific deposits determined by cineangiography in mitral stenosis and their effect on valve motion, hemodynamics and clinical signs.

The relation between the degree of leaflet calcium in a stenotic mitral valve and several parameters of valve mobility, hemodynamics and clinical signs was determined in 105 patients with relatively pure mitral stenosis (MS). The amount of mitral valve calcific deposits was determined by grading cineangiograms. Compared to 71 patients with no or minimal valvular calcium, the 23 patients with heavy valve leaflet calcium were significantly older, more likely to be men and more likely to be in atrial fibrillation. These patients also had a significant reduction of valve mobility in that their M-m

Arousal and stability: the effects of five new sympathomimetic drugs suggest a new principle for the prevention of space motion sickness.

Sympathomimetic agents are frequent components in antimotion-sickness drug combinations because of their usefulness in counteracting the sedation caused by stressful motion or resulting from the administration of other antimotion-sickness drugs. The noradrenergic neurochemistry of the brain's arousal-attentional systems prompted us to evaluate the efficacy of five new sympathomimetic drugs and to further define the role of arousal in susceptibility to motion. Subjects were orally administered methamphetamine (20 mg), phenmetrazine (25 mg), phentermine (37.5 mg), methylphenidate (20 mg), or pem…

[Systolic anterior motion of the mitral valve in mitral valve prolapse: report of a case and etiopathogenetic considerations (author's transl)].

The Authors describe a case of a patient suffering from mitral prolapse whose echocardiogram shows in addition to typical findings of this syndrome an annual systolic anterior motion (SAM) of the anterior mitral leaflet, generally present in the idiopathic hypertrophic obstructive cardiomyopathy and more seldom in some others disease, however not present in this present. The Authors suggest that this finding is correlated to the mitral prolapse and express some hypothesis regarding this possibility.

Prevention of motion sickness in flight maneuvers, aided by transfer of adaptation effects acquired in the laboratory: ten consecutive referrals.

Ten flyers, grounded because of nausea and vomiting, were referred as potential candidates for adaptation to cross-coupled angular accelerations in a slow-rotation room; such adaptation has been shown to "transfer" to flight maneuvers. There was no opportunity to attempt treatment in two candidates. Among the remaining eight, five regained flight status (62.5%); follow-up periods of those five candidates ranged from 10 to 27 months. In one of the three remaining candidates, a satisfactory level of adaptation was achieved but more than 4 months elapsed before his assignment to a duty squadron.
