Characteristics associated with the intention to complete advance directives and end-of-life preferences in Brazilians with heart failure.

OBJECTIVE: To identify characteristics associated with an intention to complete advance directives (ADs) and end-of-life treatment preferences for outpatients with heart failure (HF). METHODS: A cross-sectional, analytical study. Sociodemographic and clinical data were collected from 108 patients with HF in an outpatient clinic in São Paulo, SP, Brazil. Quality of life (QoL) was assessed using the Minnesota Living with Heart Failure Questionnaire; knowledge about HF and the intention to complete ADs were assessed using a script. The relationships among variables were assessed through the chi-

An update on advance directives in the medical record: findings from 1186 consecutive patients with unresectable exocrine pancreas cancer.

BACKGROUND: The Terri Schiavo case and other recent events underscore the importance of advance directives. Yet, in the past, only a small subgroup has utilized them. This study from a large tertiary medical center was undertaken to assess current rates of advance directives among patients with incurable pancreas cancer. METHODS/RESULTS: The medical records of 1,186 consecutive patients with unresectable pancreas cancer were reviewed over a 4-year span. Only 174 patients (15%) had an advance directive in the medical record. Older age and having cancer therapy at our institution were associate

Advanced cancer patients' knowledge and opinions regarding the French law on advance directives: a multicenter cross-sectional study.

PURPOSE: Advance directives (AD) are a key tool within advance care planning (ACP), intended to support patient autonomy and guide end-of-life care. Despite their legal recognition in France, their use in the general population remains limited. Moreover, data on AD use among patients with advanced cancer, a population particularly concerned by end-of-life decisions, remain scarce. This study aimed to assess the knowledge and opinions regarding AD among patients with advanced cancer in France who were receiving specialized palliative care. METHODS: Between 2016 and 2020, we conducted a multice

Goals of care and advance directives: A 1-year retrospective analysis of a tertiary centre in Saudi Arabia.

BACKGROUND: Despite its limited therapeutic value, high costs and the fact that it can compromise a patient's quality of life, aggressive care continues to be provided at the end of life. It has been associated with a significant symptom burden for patients, a greater use of healthcare resources, delay in accessing palliative care programmes and personal/family and healthcare costs. AIM: To investigate sociodemographic, clinical and community healthcare service factors associated with aggressive end-of-life care across malignant and non-malignant conditions. METHOD: A 1-year retrospective an

The role of specialized outpatient palliative care in emergency advance directives: fewer hospitalizations, greater alignment with patient wishes.

CONTEXT: Specialized outpatient palliative care (SOPC) aims at relieving symptoms and providing psychosocial aid in the outpatient setting. SOPC also supports patients in setting up advance directives so that their will is respected in emergency situations. OBJECTIVES: The aim of this study is to analyze the impact of SOPC on the completion of advance care directives with the focus on medical emergency identification (ID) cards (= living will in a credit card format on yellow paper) and their impact on hospital admissions. METHODS: All adult patients who were admitted to the SOPC service at a

Modifiable Factors Associated with the Completion of Advance Treatment Directives in Hematologic Malignancy: A Patient-Caregiver Dyadic Analysis.

Objective: The completion rates of advance treatment preferences in patients with hematologic malignancies are low. To improve these rates, the modifiable factors associated with completion need to be determined. This study aimed to examine the associations of patient attitudes toward, and knowledge about, advance directives (ADs) with the patient-caregiver dyadic completion of advance treatment directive surveys. Methods: Using a nonexperimental correlational design, 44 patient-caregiver dyads completed the questionnaires, including a Korean-Advance Directive model. Cohen's kappa coefficient

[End-of-life practices and advance directives: a bioethical analysis based on discussions in the Brazilian Federal Legislative Branch].

The Brazilian legislative debate on end-of-life practices involves ethical-axiological tensions related to euthanasia, assisted suicide, palliative care, and advance directives, in which divergent conceptions of these practices and of the value of life confront one another, influencing the normative formulation of the dying process. This study analyzes how such tensions are constructed and justified in the discourses of the Federal Legislative Branch, examining the normative meanings and the hierarchy of values attributed to the palliative care framework and to advance directives, as well as t

Psychiatric advance directives and consent to electroconvulsive therapy (ECT) in Australia: A legislative review and suggestions for the future.

Psychiatric Advance Directives (PADs) have been adopted in many jurisdictions around the world and in most Australian states and territories. They are seen as a less restrictive and patient-centered approach to the provision of mental health care. Electroconvulsive therapy (ECT) is a restricted treatment in most jurisdictions in Australia and across the world. This paper explores the history, regulation and use of ECT and PADs and the intersections between them. It provides an overview of the legislative framework in each Australian state and territory and explores some of the issues which hav

Flaws in advance directives that request withdrawing assisted feeding in late-stage dementia may cause premature or prolonged dying.

BACKGROUND: The terminal illness of late-stage (advanced) Alzheimer's and related dementias is progressively cruel, burdensome, and can last years if caregivers assist oral feeding and hydrating. Options to avoid prolonged dying are limited since advanced dementia patients cannot qualify for Medical Aid in Dying. Physicians and judges can insist on clear and convincing evidence that the patient wants to die-which many advance directives cannot provide. Proxies/agents' substituted judgment may not be concordant with patients' requests. While advance directives can be patients' last resort to at

Effect of Psychiatric Advance Directives Facilitated by Peer Workers on Compulsory Admission Among People With Mental Illness: A Randomized Clinical Trial.

IMPORTANCE: Reducing the use of coercion in mental health care is crucial from a human rights and public health perspective. Psychiatric advance directives (PADs) are promising tools that may reduce compulsory admissions. Assessments of PADs have included facilitation by health care agents but not facilitation by peer workers. OBJECTIVE: To determine the efficacy of PADs facilitated by peer workers (PW-PAD) in people with mental disorders. DESIGN, SETTING, AND PARTICIPANTS: A multicenter randomized clinical trial was conducted in 7 French mental health facilities. Adults with a DSM-5 diagnos

Advance directives and do-not-resuscitate orders in patients with cancer with metastatic spinal cord compression: advanced care planning implications.

ObjectivesCommunication about end-of-life decisions is crucial. Although patients with metastatic spinal cord compression (MSCC) have a median survival time of 3 to 6 months, few data are available concerning the presence of advance directives and do-not-resuscitate (DNR) orders in this population. The objective of this study was to determine presence of advance directives and DNR order among patients with MSCC.MethodsWe retrospectively reviewed data concerning advance directives for 88 consecutive patients with cancer who had MSCC and required rehabilitation consultation at The University of

The Prevalence of Advance Directives and Its Moderators Among Community People, Acutely and Chronically Ill Patients: A Systematic Review and Meta-Analysis.

Advance Directives (ADs) are essential in end-of-life care, enabling individuals to retain autonomy when they can no longer make decisions. However, data on ADs prevalence and influencing factors in different populations (acutely ill, chronically ill, and general) remain limited globally. This review aims to assess the prevalence of ADs in acute and chronic care settings and the general population and identify factors influencing their completion. A systematic review was conducted using PubMed, Scopus and Cochrane Library databases for English-language studies on ADs prevalence up to December

Junior medical doctors' decision making when using advance care directives to guide treatment for people with dementia: a cross-sectional vignette study.

BACKGROUND: Junior medical doctors have a key role in discussions and decisions about treatment and end-of-life care for people with dementia in hospital. Little is known about junior doctors' decision-making processes when treating people with dementia who have advance care directives (ACDs), or the factors that influence their decisions. To describe among junior doctors in relation to two hypothetical vignettes involving patients with dementia: (1) their legal compliance and decision-making process related to treatment decisions; (2) the factors influencing their clinical decision-making; an

The advance directive--a review.

The advance directive is a document that enables a competent individual to specify the form of health care he would like to have, in the event that he is unable to make such decisions in the future. This review paper traces the development of the advance directive from 1967, when it was first proposed by Luis Kutner. The Karen Ann Quinlan case and the Nancy Cruzan case are cited as examples of the case for the advance directive. The argument is that advance directives assist doctors, patients, family members and other carers with the increasingly complex health care decision making. Reservatio

Development and Psychometric Properties of PEACE-Q: A Questionnaire on Attitudes Towards Physician-Assisted Dying, Euthanasia, Advance Directives and Care at the End-of-Life.

Public attitudes toward euthanasia, physician-assisted dying (PAD), advance directives (ADs), and end-of-life (EOL) decision-making in the ICU remain underexplored, while existing tools address these issues fragmentarily. This study aimed to develop and psychometrically evaluate a novel, self-report instrument named Physician-assisted dying, Euthanasia, Advance directives and Care at the End-of-life Questionnaire (PEACE-Q) to cohesively assess relevant public attitudes. PEACE-Q was developed through literature review, expert evaluation and pilot testing, with initial validation conducted in Gr

End-of-life decision-making and changing preferences in patients with a left ventricular assist device for destination therapy: insights from advance directives in Japan.

End-of-life decision-making and the evolution of patient preferences over time remain insufficiently explored in destination therapy with left ventricular assist device (DT-LVAD) in Japan. This retrospective observational study analyzed standardized advance directives from DT-LVAD patients in Japan. The advance directives comprised (1) preferred end-of-life medical care, (2) designated surrogate decision-makers, and (3) personal life wishes. This study comprises two components: a cross-sectional analysis of all patients at the time of LVAD implantation, and a longitudinal analysis evaluating c
