The dysvascular amputee: multidisciplinary management.

The authors describe 8 years of experience in the care of amputees with dysvascular diseases using a multidisciplinary team approach. They compare this method of amputee rehabilitation with one using similar personnel but no team approach. In the hospital using the team approach, there were many more below-knee amputees, a higher number of prosthetic fittings in a shorter period and more fittings in older patients than in the control hospital.

The Krukenberg procedure in the juvenile amputee.

The Krukenberg procedure has been performed unilaterally on juvenile amputee patients with congenital bilateral absence of the hands and observed by long-term follow-up examinations. The opposite extremity was routinely fitted with a standard prosthetic prescription as an assistive hand. In all cases, the Krukenberg stump became the leading hand. In some of the cases, the prosthesis on the opposite hand has been discarded in favor of the sensibility available without. The prehension ability of the Krukenberg stump has provided the patient with all of his functional needs. The results have been

Translation and validation of the Italian amputee mobility predictor: Assessing functional mobility in lower limb amputees.

OBJECTIVES: This study aimed to translate, cross-culturally adapt, and conduct a preliminary validation of the Italian version of the Amputee Mobility Predictor (AMP-IT) to assess its psychometric properties. METHODS: In this cross-sectional study, the AMP was translated into Italian in accordance with established guidelines. Twenty-eight individuals with lower limb amputation were recruited from various rehabilitation centres. The psychometric evaluation included assessing internal consistency (Cronbach's alpha), test-retest reliability (intraclass correlation coefficients, ICCs), and concur

[Hemodynamic study of muscle perfusion in amputees].

To provide a reference for determining the hemodynamic environment of blood vessels and the nutrient supply of residual limbs, as well as for rehabilitation treatments aimed at reducing muscle atrophy after amputation, this study employed contrast-enhanced computed tomography technology to obtain angiographic images of the vascular system in residual limbs. A vascular model was established through three-dimensional reconstruction and mesh generation, followed by hemodynamic analysis using the finite element method. Based on the blood flow velocities in the femoral artery-0.09 m/s at rest, 0.23

Transcutaneous osseointegration for amputees with burn trauma.

OBJECTIVE: Transcutaneous osseointegration for amputees (TOFA) surgically implants a prosthetic anchor into the residual limb's bone, enabling direct skeletal connection to a prosthetic limb and eliminating the socket. TOFA has demonstrated significant mobility and quality of life benefits for most amputees, but concerns regarding its safety for patients with burned skin have limited its use. This is the first report of the use of TOFA for burned amputees. METHODS: Retrospective chart review was performed of five patients (eight limbs) with a history of burn trauma and subsequent osseointegra…

Sensory feedback for limb prostheses in amputees.

Commercial prosthetic devices currently do not provide natural sensory information on the interaction with objects or movements. The subsequent disadvantages include unphysiological walking with a prosthetic leg and difficulty in controlling the force exerted with a prosthetic hand, thus creating health issues. Restoring natural sensory feedback from the prosthesis to amputees is an unmet clinical need. An optimal device should be able to elicit natural sensations of touch or proprioception, by delivering the complex signals to the nervous system that would be produced by skin, muscles and joi…

Gait compensatory mechanisms in unilateral transfemoral amputees.

Individuals with unilateral transfemoral amputation depend on compensatory muscle and joint function to generate motion of the lower limbs, which can produce gait asymmetry; however, the functional role of the intact and residual limb muscles of transfemoral amputees in generating progression, support, and mediolateral balance of the body during walking is not well understood. The aim of this study was to quantify the contributions of the intact and the residual limb's contralateral muscles to body center of mass (COM) acceleration during walking in transfemoral amputees. Three-dimensional sub…

Cutaneous sensitivity in unilateral trans-tibial amputees.

AIM: To examine tactile sensitivity in the leg and foot sole of below-knee amputees (diabetic n = 3, traumatic n = 1), and healthy control subjects (n = 4), and examine the association between sensation and balance. METHOD: Vibration perception threshold (VPT; 3, 40, 250Hz) and monofilaments (MF) were used to examine vibration and light touch sensitivity on the intact limb, residual limb, and homologous locations on controls. A functional reach test was performed to assess functional balance. RESULTS: Tactile sensitivity was lower for diabetic amputee subjects compared to age matched control…

Outcomes of lower limb amputees at Cotonou.

BACKGROUND: Limb amputations are responsible for disability. We studied the outcomes of lower limb amputees in our daily practice. METHODS: This prospective analytical study over 7 years (January 2009-December 2015) included 70 amputees of lower limb. They were mainly male (73%), aged on average of 42.4 ± 18.8 years. The mean time of follow- up was 3.2 ± 1.9 years. We assessed disability on balance, walking, disability in daily life for patients with prosthesis, and the socio-economic impact of the amputation. Statistical analysis was performed with Chi2 and Mann-Whitney tests; a p-value ≤ 0.…

Skin problems in amputees: a descriptive study.

BACKGROUND: Skin problems are common in amputee patients. These problems may restrict the normal use of a prosthetic limb. We aimed to determine the range, incidence, causes and patterns of dermatological problems seen in a population of amputees. MATERIALS AND METHODS: One hundred and forty two amputees, were enrolled to the study. Age, sex, age at the time of amputation, level of amputation, reason for amputation, and types of prosthesis were noted. Dermatological problems were recorded. Stumps were swabbed for bacteriological and mycological examination, and patch tests were performed in s…

Better practical evaluation for lower limb amputees.

PURPOSE: To avoid wastage of prostheses and unfair selection methods for prosthetic prescription, we aimed to develop a practical assessment tool for amputees with doubtful potential, using refurbished second hand modular prostheses. METHOD: Thirty-seven primary amputees (33 transfemoral, 4 transtibial; 22 males, 15 females; median age 71 years, range 19-91) were considered to have doubtful potential for prosthetic use, due to single or multiple impairments. Amputees were assessed using custom-made socket and refurbished second-hand modular prostheses. Liability issues were fully covered. RE…

Knee joint proprioception in below-knee amputees.

This study determined knee joint proprioception in below-knee amputees using two methods of measurement of knee joint proprioception. The threshold for detection of passive motion and the ability to reproduce specific limb positions were recorded in nine below-knee amputees. A significant difference was observed between the prosthetic and sound limbs in the amputees in the threshold of detection test. The large angles (5 degrees to 25 degrees) involved in position reproduction tests may have provided cues through the cuff suspension. Different strategies are used by below-knee amputees to disc…

Suction socket suspension for below-knee amputees.

In this study the current use of suction suspension for below-knee prostheses is examined by means of two questionnaire surveys. The experience of 56 below-knee (B-K) amputees wearing suction socket prostheses is evaluated comparing suction prostheses with previously worn limbs. A high degree of satisfaction was found, with amputees on the whole reporting improved skin condition, diminished pain, and increased activity levels compared to previous prosthetic history. The experience and opinions of 466 certified prosthetist members of the American Orthotist Prosthetist Association are examined i…
