[Biliary fistula and bleeding after cholecystectomy].

The authors analysed 303 cases of cholecystectomy performed in their hospital and found that there were 22 cases of biliary fistula (7.2%), subhepatic infection ensued in 3 (0.9%), and massive postoperative bleeding in 5 (1.7%) with two deaths. The histology of the gallbladder bed was studied under microscope in 60 bodies. It was found that the intact gallbladder bed, rich in collagenous, elastic, and reticular fibers was a strong fibromembranous lining adhering to the liver surface, with numerous small blood vessels, bile ductuli, and lymphangiomas going through it. Leaving the lining intact …

Cholecystocolonic fistula resulting from biliary stent migration.

BACKGROUND AND AIMS: Patients may require biliary stent placement for cholecystitis, but stent dislocation rarely results in colonic perforation. We describe a patient whose biliary stent dislodged between the transverse colon and gallbladder, creating a fistula. We aim to highlight the importance of early recognition and describe a minimally invasive approach to fistula closure. METHODS: We used a novel technique using simultaneous bidirectional endoscopy, performing colonoscopy and ERCP concurrently to repair a cholecystocolonic fistula caused by biliary stent migration. RESULTS: This comb

Biliary pleural fistula detected by hepatobiliary scintigraphy.

A biliary pleural fistula is a rare complication secondary to trauma, infection, malignancy, biliary disease, malignancies, or percutaneous procedures. Its presence may be suggested by the development of a right pleural effusion in a patient with such history and can be confirmed with a hepatobiliary scan or endoscopic retrograde cholangiopancreatography. Patients are treated with antibiotics and the fistula usually spontaneously closes. If after 2 weeks the fistula persists, percutaneous drainage, sphincterotomy, or biliary stent placement can be performed to promote healing of the fistula. I

Spontaneous external biliary fistula uncomplicated by gallstones.

External biliary fistulae are rare. Only 65 cases have been reported in the literature and in each instance gallstones were a complicating factor. We report in this paper the first case of spontaneous external (cholecystocutaneous) biliary fistula uncomplicated by gallstones.

Biliary stent causing colovaginal fistula: case report.

OBJECTIVES: Perforation of the bowel during placement of a biliary stent is a known complication of this procedure. We report the endoluminal loss of a biliary stent during routine stent extraction that ultimately led to a chronic colovaginal fistula. This case emphasizes the need for evaluation of fecal passage of stents in patients with a known dislodged prosthesis. CASE REPORT: A 65-year-old white female underwent biliary stent placement for an episode of choledocholithiasis. The stent was lost in the duodenum during routine extraction. The patient was managed expectantly. She denied ever

Hepatic artery-biliary fistula during infusion chemotherapy.

A 52-year-old man with rectal cancer and liver metastasis underwent tumor resection with hepatic arterial catheterization. During adjuvant chemotherapy, he developed epigastric pain, nausea, and melena. Digital subtraction angiography revealed a hepatic artery-biliary fistula and a pseudoaneurysm at the catheter tip. This rare complication of intra-arterial chemotherapy is reported in detail.
