Cogan syndrome: an analysis of reported neurological manifestations.

BACKGROUND: Patients with Cogan syndrome typically present with nonsyphilitic interstitial keratitis and acute onset of sensorineural hearing loss. Neurological manifestations have been reported, but various frequencies and mechanisms have been proposed. REVIEW SUMMARY: We critically reviewed the English literature of Cogan syndrome to determine the nature, frequency, and most likely mechanisms of its neurological manifestations. CONCLUSIONS: On the basis of our review, we believe that Cogan syndrome can be associated with neurological manifestations. Our conclusion is based on reported tiss…

Cogan syndrome: confocal microscopy assessment of corneal damage.

PURPOSE: To perform in vivo assessment of corneal alterations in patients with Cogan syndrome (CS) and to correlate these findings with prior histopathologic descriptions. METHODS: Four consecutive patients (8 eyes) presenting with typical CS underwent confocal microscopy examination. At the moment of evaluation, ocular inflammation was quiescent in all the patients. The images were studied singularly, and then compared to those obtained from 5 healthy controls. Statistical analysis was performed with Student t test. RESULTS: All the cases showed multiple brightly reflective deposits in the …

[Cogan I syndrome: clinical aspects, therapy and prognosis].

BACKGROUND: Cogan I syndrome is a rare, inflammatory, systemic disease that is typically characterized by severe audiovestibular dysfunction and various inflammatory eye changes. PATIENTS AND METHODS: Between 1978 and 1996 ten patients with Cogan I syndrome were treated at the eye hospital, ENT clinic and medical clinic of the University of Tübingen. Retrospectively, the clinical picture, course of the disease and outcome were investigated by reviewing the charts of these patients. RESULTS: Patient ages ranged from 18 to 78 years (mean 37 years) at the time of disease manifestation. Eight pa

[Cogan syndrome as a rare cause of deafness].

Cogan syndrome is a rare systemic disease characterized by the occurrence of audiovestibular and ocular symptoms. We present the case of a 62-year-old female patient who received a cochlear implant after unilateral deafness. Despite successful implantation, the disease progressed with chronic headache, recurrent otitis, fluctuating intracochlear impedances, and slowly progressing hearing loss in the contralateral ear. Through interdisciplinary collaboration, the diagnosis of Cogan syndrome was established, and biological therapy was initiated.

[Cogan syndrome with audiovestibular disfunction: 2 cases report].

We describe a case of Cogan syndrome in patients with clinical manifestations, related examinations and diagnosis, treatment, and review of relevant literature. Both patients had bilateral moderate to severe sensorineural hearing loss. The collateral function test showed that there were bilateral semicircular canal paresis, and the head impulse test had a bilateral gain value between 0.09 and 0.55. It showed obvious compensatory saccade wave, the head impulse suppression test bilateral no anti-compensation saccade wave.Cogan syndrome is a rare syndrome with unspecific symptoms and early diagnosis…

Endovascular treatment of multiple aneurysms complicating Cogan syndrome.

To report the use of endografts to manage multiple aneurysms due to Cogan syndrome (CS). A 38-year-old woman with descending thoracic aorta and right common carotid artery aneurysms due to CS was treated with endovascular grafts. After 4 years, angio computed tomography scan demonstrated complete exclusion of the aneurysms with no signs of endoleak, whereas echo color Doppler showed patency of the carotid graft, no signs of restenosis, no progression of the disease in the landing zones, and complete aneurysm exclusion. Endovascular repair seems to have favorable long-term outcomes and should b…

[Cogan syndrome: sudden, bilateral high-grade hearing loss].

We report 2 cases of Cogan's syndrome. One female patient had a typical interstitial keratitis and bilateral hearing impairment, the other had dramatically progressive deafness, combined with failure of the left vestibule and severe conjunctivitis resistant to treatment and otitis externa. The typical Cogan's syndrome with interstitial keratitis developed only a few weeks later. The disease is caused by an auto-immune response. The hearing impairment is mainly due to endolymphatic hydrops. Corticosteroids are the treatment of choice.

Atypical Cogan's syndrome: A case report.

Cogan's syndrome is a rare autoimmune inflammatory disease, characterized by interstitial keratitis and audio-vestibular signs. The syndrome was first described in 1945 by David G. Cogan. Then, it was only in 1980 when Haynes et al. proposed diagnostic criteria for patients with other symptoms and was qualified as atypical form of Cogan's syndrome. Herein, we report a case of a 28-year-old woman with atypical Cogan's syndrome. The patient was treated with corticosteroids and received a cochlear implant.

Cogan's syndrome and other ocular vasculitides.

The clinical presentation of Cogan's syndrome has been classified as typical and atypical. Like other forms of ocular vasculitis, Cogan's syndrome has been found to have autoimmune origins with antibodies against the cornea, inner ear, and endothelial antigens. Antineutrophil cytoplasmic antibody (ANCA) and rheumatoid factor (RF) have been associated with Cogan's syndrome as well as ocular-involving vasculitides not as strongly associated with the audiovestibular manifestations such as granulomatosis with polyangiitis and rheumatoid arthritis. The mainstay of therapy has been corticosteroids a

Cogan's syndrome associated with orbital inflammation.

Cogan's syndrome is a relatively rare inflammatory disorder presenting with a combination of ocular and vestibuloauditory symptoms. The first cases were described by David Cogan in 1945. Typically the ocular signs involve the anterior segment; however there are no descriptions of orbital inflammation associated with Cogan's syndrome. We describe such a case. A 66-year-old immunosuppressed male patient with known Cogan's syndrome presented to the eye department with right-sided proptosis, chemosis and ocular injection. Ocular motility was reduced. Other than Cogan's syndrome there were no other

[Cogan's syndrome--an interdisciplinary diagnostic challenge].

Cogan's syndrome is a rare autoimmune-like disorder characterised by nonsyphilitic interstitial keratitis and Meniere's syndrome-like audiovestibular function disorder. David G. Cogan initially described the syndrome in 1945. In 1980 Haynes et al proposed diagnostic criteria for patients with other ocular symptoms and suggested this to be atypical Cogan's syndrome. We report a case of a 43 year-old woman with atypical Cogan's syndrome. The patient received a cochlear implant prior to diagnosis.

Course of auditory impairment in Cogan's syndrome.

PURPOSE: Cogan's syndrome (CS), characterized by interstitial keratitis, hearing loss, and vestibular impairment, rarely occurs in children. Hearing loss is commonly bilateral and permanent in 37%-67% of patients. To date, long-term evaluation of hearing impairment in children with CS has been reported in only 3 patients. We describe the 35-month course of hearing impairment in a teenaged boy with Cogan's syndrome. MATERIALS AND METHODS: The medical record of a 15-year-old boy with Cogan's syndrome was reviewed. Data included relevant history and physical examination, diagnostic workup, and m

Cochleovestibular impairment in pediatric Cogan's syndrome.

Cogan's syndrome is a rare, chronic inflammatory disorder that typically targets the eyes and vestibuloauditory apparatus, but it may also involve other organs. Three pediatric cases of Cogan's syndrome (ages 5, 13, and 18 years) are reported with long-term follow-up and complete and regular cochleovestibular functional evaluation and ophthalmologic and neurologic examinations. One case was a typical form (characterized by an interstitial keratitis and cochleovestibular impairment), whereas the other 2 cases were atypical forms with uveitis and polyarthritis. In all 3 cases, the first clinical

Multiple cranial neuropathy in Cogan's syndrome.

A 20 year old woman presented with recurrent alternative keratitis for four months. One month before admission, she developed progressive hearing loss, visual impairment, facial diparesis and bilateral trigeminal neuropathy. Cogan's syndrome was diagnosed. Prompt treatment with corticosteroid resulted in dramatic improvement of the ocular, otological and neurological dysfunctions.

Pyoderma gangrenosum complicating Cogan's syndrome.

Cogan's syndrome is a rare clinical entity defined by the association of a nonsyphilitic interstitial keratitis and vestibuloauditory dysfunction, typically Menière's disease-like; the condition has been reported in association with a variety of cutaneous diseases. We now report a case of pyoderma gangrenosum complicating Cogan's syndrome in a 57-year-old woman, which then healed dramatically, as more interestingly did the associated uveitis with minocycline therapy.
