Comparison of digital subtraction dacryocystography and dacryoendoscopy in patients with epiphora.

OBJECTIVE: To assess the diagnostic information provided by digital subtraction dacryocystography (DCG) and dacryoendoscopy in patients with epiphora. METHODS: Sixty-eight lacrimal systems of 34 patients with epiphora were prospectively studied. Patients were initially examined with syringing, followed by digital subtraction DCG and dacryoendoscopy to confirm the diagnosis. Obstructions in lacrimal pathways were evaluated by degree, location, and cause. The weighted kappa coefficient was used to compare the agreement between the tests. RESULTS: Of the 68 lacrimal pathways, partial or complet…

The prognostic value of preoperative dacryocystography in endoscopic intranasal dacryocystorhinostomy.

Endoscopic intranasal dacryocystorhinostomy has been used as a primary treatment of lacrimal obstruction and for revision of conventional dacryocystorhinostomy. This study correlates dacryocystographic anatomy with the success of the endoscopic surgical technique. Eighteen patients with epiphora and nasolacrimal obstruction underwent operations. Preoperative dacryocystography identified 11 patients with either normal or enlarged lacrimal sacs, and seven patients with cicatrized lacrimal sacs. Patients with lacrimal sac stones and tumors were excluded. Endoscopic intranasal dacryocystorhinostom…

Gadobutrol: an alternative contrast agent for digital subtraction dacryocystography.

We report the application of gadobutrol as a contrast medium for digital subtraction dacryocystography (DS-DCG) in patients with known allergy to iodinated contrast agent. Gadobutrol has the double gadolinium concentration (1.0 mmol/ml) of other gadolinium-based contrast agents. Quality of the DS-DCG images obtained with gadobutrol was comparable to DS-DCG images obtained with iodinated contrast medium. Radiodensity measurements using a micro-CT scanner confirmed a high radiodensity of gadobutrol which was comparable to the radiodensity of iopentol with a iodine concentration of 250 mg/ml and …

[Significance of preoperative digital subtraction dacryocystography for nasolacrimal stent].

Digital subtraction dacryocystography were performed prior to placement of a nasolacrimal stent. Among the 107 eyes, the sites of obstruction were in the common canaliculus in 9 eyes, above the neck of the lacrimal sac in 12 eyes and under the neck of the lacrimal sac in 86 eyes. The placement of a nasolacrimal stent may become the treatment of choice for epiphora due to obstruction of the nasolacrimal duct at the saccular or subsaccular level. After operation, these nasolacrimal ducts after stenting were patent and epiphora was resolved completely or partially. The results suggest that the pr…

Combined computed tomography and dacryocystography for complex lacrimal problems.

Two imaging modalities, computed tomography (CT) and dacryocystography (DCG), were combined to demonstrate the relationships between the lacrimal system and the surrounding soft-tissue structures. In selected cases, such as those involving severe facial trauma, midfacial tumours, significant sinus disease, or previous lacrimal, nasal or sinus surgery, this imaging technique may be useful in better evaluating the anatomy of the lacrimal system and planning a surgical approach.

CT-dacryocystography for nasolacrimal duct obstruction following paranasal sinus surgery.

Nasolacrimal duct obstruction and consequent epiphora is a complication that may develop in some patients following paranasal sinus surgery. We describe the technique of CT-dacryocystography which entails contrast injection of the lacrimal system and simultaneous computed tomography (CT) scanning of the facial structures. This technique provides concurrent information regarding obstruction of the nasolacrimal duct and the presence of disease recurrence or persistence in the adjacent paranasal sinuses. The role of CT-dacryocystography prior to endoscopic dacryocystorhinostomy is discussed brief…

Dacryocystography: comparison of water-soluble and oil-based contrast agents.

Dacryocystography has been widely used in the assessment of the nasolacrimal duct system, particularly in patients with epiphora. Our study was undertaken to evaluate image quality and level of patient discomfort during examinations with water-soluble contrast agents (iohexol [Omnipaque 240], iopamidol [Isovue 200 and 300], and 52.7% diatrizoate meglumine and 26.9% iodipamide meglumine [Sinografin]) compared with the iodized oil-based contrast agent Lipiodol. Fifty-five dacryocystograms were obtained from 41 consecutive patients. The procedure was performed first with a water-soluble contrast …

MR dacryocystography - Optimising a dynamic imaging protocol for patients with epiphora.

INTRODUCTION: Radiological investigations for epiphora often use techniques involving ionising radiation and may require invasive canalicular cannulation. Magnetic resonance dacryocystography (MRDCG) has been explored but is currently not widely utilised in clinical imaging departments. The aim of this technical report is to describe the optimisation of a non-invasive MRDCG technique using a continuous gadolinium drip to assess the anatomy and real-time functionality of the nasolacrimal drainage system (NLDS). METHODS: A protocol was developed on a 1.5 Tesla (1.5 T) magnetic resonance imaging…

[Site of a polyurethane stent in dacryocystography. Is postoperative success hereby predictable?].

BACKGROUND: Despite new methods for treating complete stenosis of the nasolacrimal duct dacryocystorhinostomy remains the standard operation. Implanting a nasolacrimal polyurethane stent through the nasolacrimal duct to preserve the natural lacrimal pathway seems to offer alternative procedure with similar success rate and without skin incision and general anesthesia. We examined whether an anatomically correct position increases its success rates. METHODS: This prospective study included 40 patients (mean age 57 years). The polyurethane stent was implanted during dacryocystography, and its p…
