Disinfection of outpatient ophthalmic devices: a critique of "semicritical" designation.

Ophthalmic devices used in outpatient clinics are undergoing scrutiny by regulatory agencies and hospital infection control groups. Applying a nearly century-old classification for disinfection and sterilization of reusable devices and instruments, they are faulting ophthalmologists for not using high-level disinfection (HLD) listed in newer manufacturer instructions for use. By this classification, A-scan and B-scan ultrasound probes, fundus and laser contact lenses, and gonioscopy lenses are grouped with other "semicritical" devices, such as gastrointestinal endoscopes, laryngoscope blades,

Needleless dispensing pin with 1-way valve does not decrease bacterial contamination of communal-use intravenous fluid bags in simulated clinical scenario.

OBJECTIVE: To determine whether using a needleless dispensing pin (test port) with a 1-way valve decreases bacterial contamination of communal-use IV fluid bags in a simulated clinical scenario. METHODS: 30 paired sets of IV fluid bags (0.9% NaCl, 1,000 mL) were hung in a common hospital location (anesthesia induction). Each set contained a control bag that was sampled via its native puncture port and a test bag sampled via the 1-way valved port. Fluid samples (3 mL) were removed 3 times daily for 10 consecutive days via respective bag ports. On days 0, 2, 4, 7, and 10, duplicate port surface

Whole genome sequencing reveals transmission dynamics and persistence of Listeria monocytogenes along the poultry slaughtering and processing chain.

Listeria monocytogenes (Lm) is a gram-positive foodborne pathogen responsible for severe invasive human and livestock listeriosis. The dissemination of Lm throughout the poultry production chain significantly increases the risk of human listeriosis. In this study, we investigated the epidemiology and genetic characteristics of Lm in a poultry slaughterhouse using whole-genome sequencing (WGS). Lm contamination was detected in 8.32 % (77/925) of slaughterhouse carcasses, 9.86 % (102/1035) of environmental samples, and 13.28 % (32/241) of retail chicken, showing a gradual increase along the prod

Risk of Gluten Cross-Contamination Due to Food Handling Practices: A Mini-Review.

Celiac disease (CD) is an autoimmune disease triggered by the ingestion of gluten in genetically predisposed individuals, affecting 1.4% of the world population. CD induces an inflammatory reaction that compromises small intestine villi, leading to nutrient malabsorption, and gastro and extraintestinal manifestations. Although other treatment approaches are being studied, adherence to a gluten-free diet (GFD) is the only effective intervention to date. Despite this, about 50% of patients experience persistent inflammation, often associated with unintentional gluten ingestion through contaminat

Faulty washers and soiled micropipettors may generate false positive serological results.

BACKGROUND: Serological markers such as HBsAg and anti-HIV may be present in serum at very high concentrations and this may give rise to erroneous diagnoses due to cross-contamination. OBJECTIVES: To investigate poor equipment maintenance and use, including contamination with human serum, as a potential source of erroneous assay results. STUDY DESIGN: The potential of microtitre plate washers and micropipettors to transfer material between microplate wells and between specimens was examined. For the study of micropipettors we recruited 19 UK diagnostic laboratories. RESULTS: Four out of sev

Infection risk associated with a closed luer access device.

The potential for microbial contamination associated with a recently developed needleless closed luer access device (CLAD) (Q-Syte; Becton Dickinson, Sandy, UT, USA) was evaluated in vitro. Compression seals of 50 multiply activated Q-Syte devices were inoculated with Staphylococcus epidermidis NCTC 9865 in 25% (v/v) human blood and then disinfected with 70% (v/v) isopropyl alcohol followed by flushing with 0.9% (w/v) sterile saline. Forty-eight of 50 (96%) saline flushes passed through devices that had been activated up to a maximum of 70 times remained sterile. A further 25 Q-Syte CLADs that

Sterility of Miniature C-arm Fluoroscopy in Hand and Upper Extremity Surgery.

Previous studies have demonstrated that sterile equipment is frequently contaminated intraoperatively, yet the incidence of miniature c-arm (MCA) contamination in hand and upper extremity surgery is unclear. To examine this incidence, a prospective study of MCA sterility in hand and upper extremity cases was performed in a hospital main operating room (MOR) ( n = 13) or an ambulatory surgery center operating room (AOR) ( n = 16) at a single tertiary care center. Case length, MCA usage parameters, and sterility of the MCA through the case were examined. We found that MOR surgical times trended

Occurrence of ESBL-E. coli in feces of dairy calves and nipples of teat buckets for calves before and after automatic cleaning in two dairy farms.

Extended-spectrum β-lactamase (ESBL)-producing Escherichia (E.) coli are highly prevalent in dairy calves. In calf housing, the feeding equipment can function as a reservoir, thereby constituting a risk factor for the dissemination of ESBL-E. coli. The aim of this present study was to determine if ESBL-E. coli can be detected on the inner surface of teat buckets for calves and to evaluate if an automatic wash system specifically designed for teat buckets is suitable to eliminate existing ESBL-E. coli from the surfaces. Fecal samples from calves (aged three to 14 days) and teat swabs (before an

A Discussion of Relevant and Meaningful Extraction Solvents for Extractables Testing of Implanted Medical Devices.

During their clinical use, medical devices contact, directly or indirectly, tissue of the person (patient) whose medical condition is being treated or monitored by the device. During contact, substances present in or on the medical device can be leached from the device. Medical devices are chemically characterized to establish the patient's actual exposure to leachables. Establishing actual leachables from implanted methodical devices is problematic as so doing would require sampling of the in vivo environment in which the device is implanted. Thus, leachables from implanted medical devices ar

The Relation Between the Load, Duration, and Steam Penetration Capacity of a Surface Steam Sterilization Process: A Case Study.

In practice it is claimed that load characteristics influence a surface steam sterilization process. To the best of our knowledge, no information on this topic has been found in the literature. The purpose of this study was to find if a load influences the duration and related characteristics of a surface steam sterilization process. In a case study, every load monitoring using an objective, quantifying steam penetration test was performed during 30 days. This resulted in 98 production processes with load monitoring. The acquired data were analyzed. A relation between the weight of a load and

Improper positioning of the elevator lever of duodenoscopes may lead to sequestered bacteria that survive disinfection by automated endoscope reprocessors.

BACKGROUND: Some outbreaks associated with contaminated duodenoscopes have been attributed to biofilm formation. The objective of this study was to determine whether bacteria within an organic matrix could survive if the elevator lever was improperly positioned in the automated endoscope reprocessor (AER) after 1 round of reprocessing. METHODS: Duodenoscope lever cavities with an open or sealed elevator wire channel were inoculated with 6-7 Log10 of both Escherichia coli and Enterococcus faecalis in ATS2015 (Healthmark Industries, Fraser, MI) and dried for 2 hours. The duodenoscopes with the

Cleaning Methods and Microbial Safety of Child Feeding Equipment: A Systematic Review.

Microbial contamination of child feeding equipment (CFE) includes feeding bottles, teats, and breast pumps. This contamination is a recognized pathway for enteric pathogen exposure during early childhood, particularly during the complementary feeding period (6-24 months). Inadequate or inconsistent cleaning of CFE has been associated with increased diarrheal risk. Many cleaning approaches are practiced globally. However, comparative evidence on their effectiveness remains fragmented. This systematic review (CRD420251058230) aimed to evaluate the effectiveness and efficacy of commonly used CFE

Time-dependent persistence of blood as a contaminant in medical device processing.

In order to ensure safe processing of medical equipment, it is mandatory to study all factors that may affect the results of cleaning. Previously, it has been shown that superficial blood residues are most difficult to clean when they have coagulated but not dried. The present study aimed to verify this finding in the joints of surgical forceps, using real instruments under realistic conditions. The persistence of blood in the joints of surgical forceps showed significant time-dependent differences within the first hours after contamination. This can have a noticeable impact on validation prac
