The hidden mortality of pediatric firearm violence.

INTRODUCTION: Firearms and motor vehicle collisions (MVC) are leading causes of mortality in children. We hypothesized that firearm injuries would have a higher mortality than MVCs in children and a higher level of resource utilization METHODS: Trauma patients <18 years old at a Level 1 pediatric trauma center sustaining gunshot wounds (GSW) or MVCs 2009-2019 were included. The primary outcome was mortality. The secondary outcome was immediate surgery. The California Department of Public Health's Overall Injury Surveillance tool was queried for patients <18 with GSW or MVC 2006-2015 to compare…

Spinal Injury Associated With Firearm Use.

Objective Injuries associated with firearms are a significant health burden. However, there is no comprehensive study of firearm spinal injuries over a large population. It was the purpose of this study to analyze the demographics of spinal firearm injuries across the entire United States for all ages using a national database. Methods A retrospective review of prospectively collected data using the Inter-University Consortium for Political and Social Research Firearm Injury Surveillance Study 1993-2015 (ICPSR 37276) was performed. The demographic variables of patients with spinal injuries due…

An ergonomic comparison of firearm safety mechanisms.

As with most machines, the integral safety mechanism on firearms is vital to injury or fatality free operation. Presently, there is little or no standardisation in the design of these mechanisms. In this investigation, five existing designs found on both military and commercial rifles were evaluated ergonomically to determine the most effective characteristics for incorporation into future designs. The designs were evaluated experimentally on ease of use, visual effectiveness and operational impact. Three groups, representing a total of 30 subjects with widely varying experience, were selected…

BACKGROUND: firearm wounds of the chest are now common at our institution. The management algorithm for firearm wounds has not been evaluate … ciated abdominal injuries occurred in 8% of stab and 34% of firearm injuries. Pneumothoraces due to firearms were uncommon and rarely required drainage. More pneumothoraces were treated nonoperatively among firearm injuries in contrast to stabbing injuries where the opposite applied. Involving Concerned Significant Others in Firearm Suicide Prevention: Development of the Family FireArms Secure Storage Training Intervention.

BACKGROUND: Firearms account for 71% of veteran deaths by suicide and unsecure firearm storage practices are directly associated with increased suicide risk. Most firearm suicide prevention interventions deployed by the US Department of Veterans Affairs (VA) are directed at high-risk populations; however, a significant proportion of suicide attempts and deaths by suicide occur in individuals with undetected risk. Veterans may be open to discussions with a trusted, concerned significant other (CSO) such as a family member, friend, or spouse, including discussions about secure firearm storage. H…

The paradox of experience: firearm access, beliefs about risk, and storage behaviors among those exposed to firearm suicide.

OBJECTIVE: The presence of a firearm in the home increases suicide risk, and risk is greater when firearms are stored unsecured. Thousands of people are exposed to firearm suicides each year. Little is known about whether having access to a firearm is associated with knowing someone personally who died by firearm suicide; and how knowing someone personally who died by firearm suicide impacts beliefs of firearm suicide risk and firearm storage practices. METHODS: Data (N = 7810) were obtained through Ipsos Knowledge Panels using probability-based sampling and weighted to reflect the U.S. RESU…

Differential Effects of US State Firearm Policies on Black and Non-Black Firearm Homicide and Suicide, 1981‒2022.

Objectives. To estimate the effects of 7 state firearm policies on yearly rates of firearm mortality (homicide and suicide) among racial demographic groups. Methods. We studied yearly firearm homicide and suicide counts by race for all US states from 1981 to 2022 via de-biased, autoregressive, Bayesian negative binomial models. Results. Permissive policies were associated with 1.97 more homicides (86% posterior probability of increased mortality; 80% credible interval [CrI] = -0.31, 4.38) and 0.32 more suicides (93% probability of increase; 80% CrI = 0.04, 0.58) per 100 000 Black population. Support for age related firearm policies among Black American firearm owners: examining the role of children in the home.

BACKGROUND: In recent years, ownership of firearms has increased to approximately one-third of American households. Firearm access is a threat to child safety, with the most common location of fatal childhood firearm injury being at home. Exploring differences in opinions on gun violence prevention policies among Black firearm owners could inform community-supported interventions as one approach to reduce violent injuries among disproportionately affected youth. This study examined support for child-safety-related firearm policies among Black gun owners and whether support differed between tho…

Alcohol use, risk of suicide, and access to firearms among youth in Colorado.

Firearms are the leading cause of death for U.S. children under 18 years of age, driven by their use in suicide attempts. Alcohol use is a risk factor for suicide, and approximately 22% of U.S. high school students reported recent alcohol use. We used data from a population-based health survey of high school students in Colorado, weighted to be representative of public high school students, to examine the association of alcohol use and suicide risk, and access to firearms using weighted frequencies and Rao-Scott chi-square tests. One-fifth (20.3%) of the students reported drinking alcohol in t…

Advanced deep learning for automatic classification of fired bullets from standard-issue firearms.

Gun violence continues to be a significant global issue, causing countless innocent lives to be lost each year. This study explores deep learning for automated fired bullet marking classification. To address inconsistent results from examiners' subjective same-source judgment, we automate classification to boost forensic firearm examination accuracy and reduce subjectivity. We collected a dataset of 6000 fired bullets from six types of standard-issue firearms commonly used by Chinese law enforcement agencies. Panoramic images of the lateral surfaces of the bullets were captured using the BalSc…

Virtual comparison microscopy revolutionizing firearms identification: A review of methods, validity, and admissibility.

Virtual comparison microscopy (VCM) affords firearms identification beneficial techniques and tools that the more traditional method of light comparison microscopy (LCM) does not offer. This new imaging method allows for the fast generation of 3-dimensional images that can be used for the examination of surface topography with a resolution unprecedented in firearms identification. Whether through coherence scanning interferometry or photometric stereomicroscopy, VCM is hitting its stride in a new age of firearms examination. While several validation studies indicate that VCM has a lower inconc…

Operative videothoracoscopy in the surgical treatment of penetrating firearms wounds of the chest.

We prospectively analyzed our experience with operative videothoracoscopy (OVT) performed in a field military hospital in cases of penetrating firearms wounds of the thorax (PFAWT) sustained in Chechnya. From February to April 1996, we treated 206 wounded patients, of whom 37 (18.0%) had sustained chest injuries. PFAWT were present in 23 soldiers, accounting for 62.2% of all chest injuries. Twelve injuries were confined to the thorax, eight patients had associated injuries, and three soldiers had thoracoabdominal injuries. Nineteen patients had pleural drainage performed during medical evacuat…

Cardiorespiratory demands of firearms training instruction and 15m shuttle tests in British law enforcement.

OBJECTIVES: Law enforcement agencies require minimum fitness standards to safeguard their officers and training staff. Firearms instructors (FI) are expected to maintain the same standards as their operational counterparts. This study aimed to quantify the daily physiological demands placed on FI. METHODS: 19 FI (45 ± 5 years) completed occupational tasks whilst wearing heart rate (HR) monitors for a minimum 10 days. Maximal oxygen consumption (VO2max) testing was conducted on FI during a treadmill test (TT) and a multistage shuttle test (ST). Linear regression models were used to model the r…

Reducing firearm injury: A population-level approach.

Firearm injuries are a critical threat to public health with negative psychosocial impacts that ripple out from survivors and their loved ones to communities and societies. Firearm injuries are often understood in individualistic terms, however, focusing on individual-level risk factors and approaches to mitigating consequences among individual survivors. In this commentary, we propose a reframing of firearm injuries and their prevention as a population-level health challenge. We argue that firearm injuries occur within a social, economic, and political landscape that contributes inequity in r
