The epidemiology of pediatric traumatic brain injury presenting at a referral center in Moshi, Tanzania.

BACKGROUND: Over 95% of childhood injury deaths occur in low- and middle-income countries (LMICs). Patients with severe traumatic brain inju … Scores (GCSs, severe 3-8, moderate 9-13, and mild 14-15), morbidity at discharge as measured by the Glasgow Outcome Scale (GOS, worse functional status 1-3, better functional status 4-6), and mortality status at discharge. The analysis included descriptive statistics, bivariable analysis and multivariable logistic regression to report the predictors of mortality and mo …

Risk factors in the outcome of children with minor head injury.

A subset of children with minor head injury is known to develop serious neurological consequences, but identifying this subset has been difficult. In adults, multiple variables such as skull fracture, Glasgow Coma Scale score, unconsciousness, and amnesia are significant factors that determine whether to admit the patient to the hospital and the patient's outcome. As an objective assessment of neurological function, however, the Glasgow Coma Scale has limited usefulness in children, particularly those under 36 months of age. We report our experience with 937 children having head injuries using

Methods of functional outcome assessment in neurocritical care trials: A systematic review and meta-analysis.

BACKGROUND: Acute brain injury due to conditions such as subarachnoid haemorrhage and traumatic brain injury are associated with physical, p … functional outcome measure. RESULTS: We identified 176 studies, including 60 494 participants. The Glasgow Outcome Scale or the Glasgow Outcome Scale Extended was used in 68% (N = 120) of studies and the Modified Rankin Scale in 38% (N = 67) of studies. Outcome assessment was reported at different timepoints, using different methods of application, and often dichotomi …

Demographics, supratentorial injuries, and outcome at 6 months (Glasgow Outcome Scale) were recorded. RESULTS: Initial Glasgow Coma Scale (GCS) in 11 patients was less than 5 (group I). A Case of Hyperacute Onset of Vasospasm After Aneurysmal Subarachnoid Hemorrhage and Refractory Vasospasm Treated with Intravenous and Intraventricular Nitric Oxide: A Mini Review.

BACKGROUND: A case of hyperacute vasospasm, indicating a poor prognosis after aneurysmal subarachnoid hemorrhage (SAH), is reported, and a review is presented of the literature addressing use of nitric oxide (NO) donors in cases of refractory vasospasm and recurrent delayed cortical ischemias (DCI). CASE DESCRIPTION: A 65-year-old woman was admitted within 1 hour after aneurysmal SAH (Hunt and Hess grade III, Fisher modified by Frontera grade IV). A hyperacute vasospasm had been confirmed arteriographically, the right middle cerebral artery (MCA) aneurysm was immediately coiled and a standard

Assessment of outcome prediction in severe brain damage patients with brainstem auditory evoked response.

Patients with various neurological disease resulting in severe brain damage and having Glasgow Coma Scale (G.C.S.) score of less than ten (mean 5.6) were studied with brainstem auditory evoked response (BAER) during the first few days of onset. Among them were subarachnoid hemorrhage, head injury, intracerebral hemorrhage, pontine hemorrhage, intraventricular hemorrhage and bleeding tumor. BAERs were graded into four levels from one (normal) to four (most abnormal). Outcomes were categorized through the Glasgow Outcome Scale. Comparison of the results between BAER and G.C.S. as predictors of o…

Cognitive Performance of Brazilian Patients With Favorable Outcomes After Severe Traumatic Brain Injury: A Prospective Study.

OBJECTIVE: The aim of this study was to investigate the cognitive performance of patients with favorable outcomes, determined by the Glasgow Outcome Scale, 1 yr after hospital discharge due to severe traumatic brain injury. DESIGN: This was a prospective case-control study. From 163 consecutive adult patients with severe traumatic brain injury included in the study, 73 patients had a favorable outcome (Glasgow Outcome Scale score of 4 or 5) 1 yr after hospital discharge and were eligible for the cognitive evaluation, of which 28 completed the evaluations. The latter were compared with 44 heal…

BACKGROUND: Penetrating head injuries with impacted foreign bodies are rare, associated with a high incidence of morbidity and potentially l … n the form of antibiotics and dehydrating measures as required. The primary outcome measure was the Glasgow Outcome Scale (GOS) at the end of follow-up. RESULTS: At the end of follow-up, ten (62.5 %) patients had a GOS score of 5, two (12.5 %) patients had a score of 4, and four (25 %) patients had a score of 1. Predictive value of hyperthermia and intracranial hypertension on neurological outcomes in patients with severe traumatic brain injury.

BACKGROUND: Intracranial hypertension (ICH) and hyperthermia are common after traumatic brain injury (TBI) and associated with worse neurological outcomes. This study sets out to determine the combined power of temperature and intracranial pressure (ICP) for predicting neurologic outcomes and prolonged length of stay (LOS) following severe TBI. METHODS: High resolution (every 6 seconds) temperature and ICP data were collected in adults with severe TBI from 2008-2010. Temperatures were plotted against concurrent ICP and divided based on breakpoints (Temperature: <36, 36-38.5 or >38.5 °C, ICP:

Association of cerebral microdialysis monitoring with clinical outcomes after traumatic brain injury; A systematic review.

OBJECTIVE: Cerebral microdialysis (CMD) enables bedside monitoring of cerebral metabolism after traumatic brain injury (TBI). This systematic review evaluates associations between CMD-derived analytes and mortality and functional outcome in adults with moderate-to-severe TBI. METHODS: A systematic review was conducted of adult patients with moderate-to-severe TBI (Glasgow Coma Scale ≤12) undergoing CMD monitoring of glucose, lactate, pyruvate, lactate/pyruvate ratio (LPR), glutamate, and/or glycerol. Prespecified outcomes were mortality and functional outcome (Glasgow Outcome Scale, Glasgow O…

Noninvasive assessment of the intracranial pressure in non-traumatic intracranial hemorrhage.

The article describes the modified technique of measuring the diameters of the optic nerve sheath (ONSD) for assessment of the intracranial pressure (ICP) in patients with intracerebral or subarachnoid hemorrhage (SAH). The CT scans of 443 patients were analyzed retrospectively. The ONSDs were measured at 3mm behind the globe and at the point where the ophthalmic artery crosses the optic nerve. The ONSD/eyeball transverse diameter (ETD) ratio was calculated. The correlation analysis was performed with the Glasgow Coma Scale score, Hemispheric Stroke Scale score, Glasgow Outcome Score, and inva

Evaluation of the GCS-Pupils Score for PrOgnosis in trauMatic brAin injury- The COMA Study.

OBJECTIVES: Glasgow Coma Scale-Pupils (GCS-P) score has been found to be strongly related to in-hospital mortality in retrospective studies. We hypothesized that GCS-P would be better prognosticator than Glasgow Coma Scale (GCS) in patients with traumatic brain injury (TBI). METHODS: In this prospective, multicentric, observational study, GCS and GCS-P scores were noted in adult TBI patients at ICU admission. Demographic variables, relevant clinical history, clinical/radiological findings and ICU complications were also noted. Extended Glasgow Outcome scale was noted at hospital discharge and…
