A radioimmunoassay for primate prolactin has been developed, with [(131)I] monkey prolactin, and antibodies to monkey or human prolactin. It is a report about the diagnostic and the therapy of 10 women with a hyperprolactinemia caused by microadenomas of the hypophysis. The diagnose was made by reason of clinical symptomatology and hormonal and neuroradiological investigations. In six cases the tumor was removed by transsphenoidal route. Human prolactin (hPRL) is the most recent anterior pituitary hormone in human endocrinology, whose structure has been elucidated in 1977. Eight patients with amenorrhea, galactorrhea, and hyperprolactinemia underwent excision of pituitary adenomas by means of a microsurgical procedure that used a transsphenoidal approach. Prolactin values returned to normal in five of the patients and were significantly lower in three. Five patients resumed their menses. Two became pregnant. All eight patients had used oral contraceptives. The association of oral contraceptives and prolactin-producing pituitary adenomas is discussed.

Prolactin-secreting pituitary microadenomas have been surgically selectively removed in 20 patients presenting an amenorrhea-galactorrhea syndrome, hyperprolactinemia and radiological modifications of the sella turcica. In these patients, the sella was of normal size but hypocyloidal tomography evidenced subtle changes characterizing pituitary microadenomas. The surgical intervention has been based on the twin criteria of hyperprolactinemia and sellar findings.
