This clinical report describes the design and fabrication of an implant-retained and soft-tissue-supported obturator prosthesis and a magnetic retained midfacial prosthesis for a patient with a midfacial defect following bilateral total maxillectomy, removal of the left zygoma, and excision of the entire upper lip. The patient underwent multiple head and neck surgeries for 4 separate primary oral cancers, 1 recurrence, and 1 pulmonary metastasis. The definitive obturator prosthesis was retained by 1 zygomatic implant with soft-tissue support and retention. The midfacial prosthesis was attached

With previous prosthetic restorations of bilateral maxillary resections, tissue erosion and bleeding on the cephalic parts of the cavity have been caused by movement and pressure from the prosthesis. Retention of a prosthesis is a major determinant in its successful use. Physical and psychologic stresses are common when one is unable to use a prosthesis comfortably. The use of a musculocutaneous flap and an obturator prosthesis had provided near-immediate reconstruction of a massive midfacial defect. This method of reconstruction resulted in soft tissue replacement for appearance and functiona

Presentation of the results obtained using the intraprostatic prosthesis UroLume in 78 patients wit BPH obstruction, 69 of which presented high surgical risk (ASA IV). Mean age was 79.8 years (r: 62-93). All patients carried urethral catheters, except 4 (5.1%) who had a provisional metal coil that required replacement. Prosthesis were implanted successfully in 72 cases (93.3%). The most significant exclusion criterion was an excessive length of prostate urethra. Mean follow-up was 15.3 months (r: 3-38). Mean maximum flow at 1 year after implant was 12.7 mL/sec; mean symptoms score (I-PSS score

BACKGROUND: Prosthetic joint infections is estimated to occur in 1-2% of primary total joint arthroplasty. Debridement, antibiotics, irrigation and retention of prosthesis (DAIR) is the traditional treatment for acute prosthetic joint infections.
OBJECTIVE: To determine risk factors of treatment failure in subjects managed with debridement, antibiotics, irrigation and retention of prosthesis for acute prosthetic joint infections.
METHODS: Our prospective, double blind and randomized investigation included 70 subjects, of both sexes, aged 63-72 years, who were managed with debridement, antibi

BACKGROUND: Personalizing prosthesis control is often structured as human-in-the-loop optimization. However, gait performance is influenced by both human control and intelligent prosthesis control. Hence, we need to consider both human and prosthesis control, and their cooperation, to achieve desired gait patterns. In this study, we developed a novel paradigm that engages human gait control via user-fed visual feedback (FB) of stance time to cooperate with automatic prosthesis control tuning. Three initial questions were studied: (1) does user control of gait timing (via visual FB) help the pr

Lower lip defects after oncologic resection can severely affect oral competence, speech, esthetics, and psychosocial well-being. Surgical reconstruction alone may not fully restore dynamic perioral function, requiring adjunctive prosthetic rehabilitation. A 58-year-old male with right lower alveolar squamous cell carcinoma underwent mid-segmental mandibulectomy with immediate free fibula flap reconstruction and primary implant placement, followed by radiotherapy. One year later, he presented with partial lower lip deficiency, drooling of saliva, speech difficulty, and esthetic compromise. A tw

Despite the important role of facial prosthetic treatment in the rehabilitation of head and neck cancer patients, delay in its implementation can be unavoidable, preventing patients from receiving a prompt facial prosthesis and resuming a normal social life. Here, we introduce an innovative method for the fabrication of an interim facial prosthesis. Using a 3D modeling system, we simplified the fabrication method and used a titanium reconstruction plate for facial prosthesis retention. The patient received the facial prosthesis immediately after surgery and resumed a normal social life earlier…

A 49-year-old male patient with residual cleft palate and missing pre-maxilla presented with an ill-fitting and unaesthetic maxillary denture. The posterior teeth were periodontally sound but crowded and had defective restorations and secondary caries. We restored the bilateral maxillary canines and first premolars with conical telescopic crowns having magnetic keepers. A metal-reinforced acrylic overdenture with magnetic attachments corresponding to the keepers of the telescopic crowns was fabricated. The telescopic crowns provided a single path of insertion, retention and stability to the pr

It is a considerable challenge for a prosthodontist to rehabilitate and sustain the prosthesis in edentulous patients with bilateral maxillectomy. Compelling evidence is lacking with respect to the treatment outcome when the maxillary defects are closed surgically in comparison to their prosthetic rehabilitation. Four edentulous male patients, with bilateral maxillectomy defects were referred for prosthodontic intervention to meet their nutritional and speech issues. In the absence of intra oral supporting and retentive structures, it was decided to retain the obturator utilizing extraoral aid

An implant-retained maxillofacial overdenture with a pharyngeal speech aid prosthesis was fabricated for a patient with a nonsurgically treated cleft palate who was unable to achieve velopharyngeal closure. Computer-aided design and computer-aided manufacturing were used to fabricate a metal-reinforced prosthesis using the Ivotion Denture System and subtractive manufacturing with geographic guides. Magnetic attachments were incorporated to improve the retention and stability of the prosthesis. Masticatory function, deglutition, and esthetics were found to be improved at the 6-month follow-up.

We describe a case of Salmonella enteritidis infection of a prosthetic knee joint that was cured with ceftriaxone therapy for 6 weeks and replacement of the tibial component of the prosthesis. Eleven other cases of salmonella prosthetic joint infection have been reported in the English-language literature. Five infections occurred within 20 days of prosthesis placement, and seven occurred several months to years later; ten of 12 infections involved hip prostheses. Nine of 12 patients who had prosthesis removal were cured of the infection. Two of the three patients with retention of the prosthe

Abutment is an important component serving as a connecting link between the implant and the implant-supported prosthesis. In accordance with mechanical, biological, and other conventional norms, it can compensate the deviation between the implant and the prosthesis, which plays a significant role in maintaining the health of peri-implant tissue and the longevity, stability, and effectivity of prosthesis. However, choosing an anterior implant abutment correctly and conveniently is challenging, given the great variety of anterior abutment in clinical practice. Through virtual implant design, the

The objective was to describe the use of a maxillary obturator supported by dental implants and ball abutments in an adult edentulous unilateral cleft patient. Adult edentulous cleft patients who have had no corrective surgeries or failed attempts at cleft repair in childhood suffer from oronasal communication resulting in hypernasal speech, dysphagia, and compromised masticatory ability. Tissue-supported obturator prostheses in such patients provide for compromised function due to instability and lack of retention. The treatment using fibromucosal-supported obturators is well established in t

Regardless of cause, the treatment of an edentulous patient with microstomia is difficult and often ingenious. Prosthodontic treatment modalities previously described are reviewed. A new type of prosthesis, the collapsible mandibular swing-lock complete denture, is introduced. The prosthesis incorporates a cast cobalt-chromium framework with a lingual hinge and a conventional labial swing-lock. This combination allows the prosthesis to be collapsible while maintaining structural durability. Advantages include ease of insertion and removal while providing maximum coverage for support, retention

The rehabilitation of facial deformities is a challenging endeavour that necessitates customising the procedure for each patient. Significant physical and psychological impacts might arise as a result of the deformity in the orofacial region. Post-COVID rhino-orbital mucormycosis has led to rise in extraoral and intraoral defects since 2020. To avoid further surgery, an economical maxillofacial prosthesis is an excellent choice as it is aesthetic, durable, long-lasting and retentive. This case report describes the prosthetic rehabilitation of the patient with post-COVID mucormycosis maxillecto

Prosthetic rehabilitation of auricular defects can be a demanding procedure due to a broad variety of clinical presentations and a wide array of treatment options. The retention of auricular prostheses is a major factor influencing the successful outcome of rehabilitative treatment following cancer surgery or trauma. Auricular prostheses have been retained by methods including implants, adhesives and spectacle frames but the choice of repair ultimately depends on patient factors, amount of soft tissue loss, and location of the auricular defect. This article describes a newer technique of using
