A modified technique and simplified laboratory procedure for ocular prosthesis fabrication.

PURPOSE: In many cases dentist tries to satisfy the patient with stock prosthesis that comes in standard sizes, shapes and colors. However, most of the patients complain of discomfort even after appropriate adjustment because of insufficient adaptation to underlying soft tissues. The rehabilitation of a patient with loss of eye as a result of congenital defect, pathology or accidental trauma is a challenge in terms of re-establishment of the esthetics of the patient. The purpose of this technical procedure is to provide a well fitting prosthesis to the patient, reducing the number of laborator

Ocular findings in the Laurence-Moon-Bardet-Biedl syndrome.

PURPOSE: To improve the description of the ocular part of the Laurence-Moon-Bardet-Biedl syndrome. METHODS: We examined 44 Scandinavian individuals who all had retinal dystrophy plus at least 2 more of the traditional cardinal signs of the syndrome: obesity, hypogenitalism, polydactyly and mental retardation. RESULTS: Full-field electroretinograms were obtained in 36 of the individuals and were abnormal in all. The dark adaptation thresholds were elevated by on average 3.5 log units. Symptoms of night blindness were observed at a mean age of 4 years and visual problems at daytime at 6-7 year

[Visual compromise of automobile drivers by frontal photo-flash in mesoptic conditions].

The time for recovery of recognition of an optotype presented at a distance of 40 m subsequent to exposure to a flash gun (Eso company, Tettnang; flash energy 200 Ws, duration 1/1000 s; distance 10 m) was studied on 97 subjects aged 20 to 83 years. A Landolt-ring (contrast 1:5, diameter 29 cm), mounted on a disk (diameter 58 cm) which could be rotated in steps of 45 degrees served as a test target. The flash gun could be dimmed down by a red filter (Schott RG 665). The tests were performed at night without additional illumination except the low beam of the car. The flash gun was foveally fixat

[Glaucoma and ocular hypertension: the importance of intraocular pressure in treatment decisions in France].

PURPOSE: Several recent clinical studies have shown the benefit of lowering intraocular pressure in both ocular hypertensive and glaucoma patients. However, in patients with open-angle glaucoma (OAG) or ocular hypertension (OHT), there is no consensus on the value of intraocular pressure (IOP) to be reached to prevent the progression of visual field defects. This IOP value, called target IOP, is defined by the ophthalmologist according to the patient and the progression of the disease, without a clear explicit justification of this decision. In France, ophthalmologists' education is mainly bas

[Ocular hypertension and optic neuropathies secondary to dysthyroid orbitopathy: Review of the literature].

Dysthyroidism, especially Graves' disease, causes potentially severe orbital disease. This is frequently accompanied by ocular hypertension stemming from multiple pathophysiological mechanisms. Adaptations in the technique of intraocular pressure measurement must occur, using portable equipment if necessary. Glaucomatous optic neuropathy secondary to dysthyroid orbitopathy is rare, and screening for signs of compressive optic neuropathy is essential in the case of visual field loss. In cases of secondary glaucomatous optic neuropathy, treatment of the intraocular pressure is based mainly on sy

Pax6 downregulation mediates abnormal lineage commitment of the ocular surface epithelium in aqueous-deficient dry eye disease.

Keratinizing squamous metaplasia (SQM) of the ocular surface is a blinding consequence of systemic autoimmune disease and there is no cure. Ocular SQM is traditionally viewed as an adaptive tissue response during chronic keratoconjunctivitis sicca (KCS) that provokes pathological keratinization of the corneal epithelium and fibrosis of the corneal stroma. Recently, we established the autoimmune regulator-knockout (Aire KO) mouse as a model of autoimmune KCS and identified an essential role for autoreactive CD4+ T cells in SQM pathogenesis. In subsequent studies, we noted the down-regulation of

Renal cell carcinoma choroidal metastasis recorded by smartphone with interface eyepiece adapter mounted on slit lamp: A case report.

RATIONALE: Ocular metastasis of renal cell carcinoma (RCC) is rare, and mainly located on the choroid. We report a choroidal metastasis from RCC, which was recorded by a smartphone with an interface eyepiece adapter mounted on a slit lamp. PATIENT CONCERNS: A 45-year-old female presented with 1-month history of painless occlusion of the vision field on the left eye, who had undergone right nephrectomy for RCC 19 months ago. DIAGNOSES: A smooth, hemispherical and brown protrusion was found behind the pupil nasally. An enhanced computed tomography scan of the orbit showed a slightly high-densi

Intraocular Pressure Elevation Compromises Retinal Ganglion Cell Light Adaptation.

PURPOSE: Functional adaptation to ambient light is a key characteristic of retinal ganglion cells (RGCs), but little is known about how adaptation is affected by factors that are harmful to RGC health. We explored adaptation-induced changes to RGC physiology when exposed to increased intraocular pressure (IOP), a major risk factor for glaucoma. METHODS: Wild-type mice of both sexes were subjected to 2 weeks of IOP elevation using the bead model. Retinas were assessed using a multielectrode array to record RGC responses to checkerboard white noise stimulation under both scotopic and photopic l

Perceptual visual filling-in of toxoplasmic retinochoroiditis scotomas.

UNLABELLED: Perceptual visual filling-in of toxoplasmic retinochoroiditis chronic scotomas was studied in 13 patients. Although there is deficit of visual inputs, patients with such chronic lesions perceive the region in the visual field uninterrupted. Targets were programmed to appear just outside the edge of the retinal lesion, and healthy retinal areas in the same eye. The results showed no significant difference in perceptual filling-in latencies (P>0.05). CONCLUSIONS: (i) neural mechanisms of filling-in at the blind spot are probably involved explaining this perceptual phenomenon; (ii) a

Podcast on Cross-speciality Perspectives on Practical Management of Atopic Dermatitis-Associated Ocular Surface Diseases.

There is a growing awareness among dermatology providers of ocular comorbidities in patients with the chronic inflammatory skin disease atopic dermatitis (AD). For example, the prevalence of ocular surface diseases (OSD) such as conjunctivitis is higher in patients with AD than in the general population, and the use of some AD treatments may be associated with OSD. In a recent review published in the Journal of the American Academy of Dermatology, dermatologists and ophthalmologists provided an overview of the different types, etiology, pathophysiology, and practical management of OSD associat

Pathobiology and treatment of viral keratitis.

Keratitis is one of the most prevalent ocular diseases manifested by partial or total loss of vision. Amongst infectious (viz., microbes including bacteria, fungi, amebae, and viruses) and non-infectious (viz., eye trauma, chemical exposure, and ultraviolet exposure, contact lens) risk factors, viral keratitis has been demonstrated as one of the leading causes of corneal opacity. While many viruses have been shown to cause keratitis (such as rhabdoviruses, coxsackieviruses, etc.), herpesviruses are the predominant etiologic agent of viral keratitis. This chapter will summarize current knowledg

Goldenhar Syndrome - ophthalmologist's perspective.

Goldenhar syndrome (oculo-auriculo-vertebral dysplasia, OAVS) is a rare, congenital disease arising from the abnormal development of the first and second branchial arches. The incidence is between 1:3500 and 1:5600, with a male: female ratio of 3:2. The etiopathogenesis is multifactorial and dependent on genetic and environmental factors but there are still many unknown aspects. The classic features of Goldenhar syndrome include ocular anomalies - epibulbar dermoids, microphthalmia and coloboma, ENT features such as preauricular tragi, hearing loss, low implantation of the auricular pavilion,

Wakayama symposium: interface between innate and adaptive immunity in dry eye disease.

Although the mechanism of dry eye disease is not clearly understood, it is certain that inflammation and the immune response play a major role in determining the health of the ocular surface in dry eye patients. Accurate ocular surface characterization during the early stages of dry eye disease is critical for successful treatment, because there exists no single standard, objective test to diagnose the early phase of dry eye disease. The treatment target should be direct to prevent the perpetuation of chronic inflammation and immune responses. Numerous studies have categorized dry eye disease

[Congenital cataract: general review].

Cataract is a loss of lens transparency because of a protein alteration. Etiopathogenesis is poorly understood but new mutations of different developmental genes involved are found in 25% of cases. Frequency of onset, particularly when different ocular development anomalies occur, is related to the lens induction phenomena on the eye's anterior segment structure during embryologic development. Genetic transmission is often found on the dominant autosomal mode. Diagnosis is based on a complete and detailed examination of the eye, often with general anaesthesia. This condition predisposes childr

Role of visual dysfunction in postural control in children with cerebral palsy.

INTRODUCTION: Deficient postural control is one of the key problems in cerebral palsy (CP). Little, however, is known about the specific nature of postural problems of children with CP, nor of the relation between abnormal posture and dysfunction of the visual system. AIM OF THE STUDY: To provide additional information on the association of abnormalities in postural control and visual dysfunction of the anterior or posterior part of the visual system. METHODS: Data resulting from ophthalmologic, orthoptic, neurological, neuro-radiological, and ethological investigations of more than 313 neur

Macular Diplopia Caused by Epiretinal Membrane.

Macular diplopia, also known as dragged-fovea diplopia syndrome, is a rare form of binocular diplopia resulting from foveal displacement caused by macular pathology. Epiretinal membrane (ERM) is the most common underlying cause, producing retinal misregistration that disrupts central fusion while preserving peripheral correspondence. Recognition of this entity is critical to avoid misdiagnosis as strabismic or neurologic diplopia. This type of diplopia highlights the sensory nature of macular diplopia, which arises from foveal displacement and central-peripheral fusion conflict. ERM-induced tr

Adaptive optics visual simulators: a review of recent optical designs and applications [Invited].

In their pioneering work demonstrating measurement and full correction of the eye's optical aberrations, Liang, Williams and Miller, [JOSA A14, 2884 (1997)10.1364/JOSAA.14.002884] showed improvement in visual performance using adaptive optics (AO). Since then, AO visual simulators have been developed to explore the spatial limits to human vision and as platforms to test non-invasively optical corrections for presbyopia, myopia, or corneal irregularities. These applications have allowed new psychophysics bypassing the optics of the eye, ranging from studying the impact of the interactions of mo

"Torso rotation" experiments; 2: Gaze stability during voluntary head movements improves with adaptation to motion sickness.

Following a 30-minute exposure to an unusual motor strategy called "Torso Rotation" (TR), the signs and symptoms of motion sickness appear along with perceptual illusions during movement, gaze and postural instability, and a significantly reduced vestibulo-ocular response (VOR) gain. With repeated exposure to TR, motion sickness symptoms disappear and gaze instability seems reduced, but without any concomitant change in VOR gain. Is the reduction in gaze instability a perceptual illusion or a real, measurable phenomenon? Velocity gain (eye velocity/ head velocity) was evaluated during voluntar

Dynamics of collateral circulation in progressive asymptomatic carotid disease.

Inadequacy of collateral arterial flow is the major risk factor for hemispheric infarction in association with spontaneous occlusion of the ipsilateral carotid artery. This prospective study was designed to measure the adaptation of collateral cerebral circulation through the circle of Willis in patients in whom a unilateral carotid stenosis of hemodynamic consequence develops asymptomatically. The collateral cerebral potential is assessed by ocular pneumoplethysmography (OPG) during proximal common carotid artery compression, measuring the collateral ophthalmic artery pressure (COAP).
