Early mobilization and rehabilitation of stroke patients can improve functional recovery. However, effects of mobilization on the responses of the autonomic nervous system (ANS) and emotions in post-stroke patients are not yet clear, particularly in patients with disorders of consciousness. We evaluated the effects of standing load on the ANS and emotions of post-stroke patients (n = 8, 70.8 ± 11.8 years) with a disorder of consciousness by measuring the prefrontal cortex (PFC) activity, which plays important roles in regulation of the ANS and emotions. Employing near infrared spectroscopy (NI

BACKGROUND: Dynamic external fixation is a useful treatment option for unstable fracture-dislocations of the proximal interphalangeal (PIP) joint, because it simultaneously reduces axial pressure load on the joint surface, maintains congruent joint reduction, and permits early joint motion. However, most current devices are large, hindering finger movement, and unstable. To address these problems, we developed a dynamic external finger fixator, named the Micro Ortho Fixator®. The purpose of this study was to review the results of using the new external finger fixator to treat unstable fracture

BACKGROUND: Recent studies have demonstrated the safety and feasibility of early mobilization of patients in the hospital setting. Promoting early mobility improves patients' ability to perform daily activities and attend to basic needs. It also preserves patients' dignity and independence. We implemented a culture of mobility program to promote awareness of the importance of early mobility among health care providers, patients, and family caregivers on an inpatient orthopedic unit.
PURPOSE: The goal of the program was to empower clinical RNs, physical therapists (PTs), and occupational thera

Periprosthetic fractures after total knee arthroplasty (TKA) can present reconstructive challenges. Not only is the procedure technically complex, but patients with these fractures may have multiple comorbidities, making them prone to postoperative complications. Early mobilization is particularly beneficial in patients with multiple comorbidities. Certain patient factors and fracture types may make revision TKA the ideal management option. Periprosthetic fractures around the knee implant occur most frequently in the distal femur, followed by the tibia and the patella. Risk factors typically a

PURPOSE: The aim of this study was to biomechanically assess patellar tendon repair techniques with additional cable wire or polydioxanone suture (PDS) cord augmentation in comparison with a suture-anchor repair technique.
METHODS: Patellar tendon repair was performed in 60 specimens using a porcine bone model. Yield load, maximum load, stiffness and elongation of patellar tendon reconstructions with (1) cable wire augmentation, (2) PDS cord augmentation or (3) suture anchor repair were evaluated using a cyclic loading and load-to-failure test setup.
RESULTS: In comparison with suture anchor

BACKGROUND/AIM: To compare outcome of early mobilisation and plaster immobilisation in patients with a simple elbow dislocation. We hypothesised that early mobilisation would result in earlier functional recovery.
METHODS: From August 2009 to September 2012, 100 adult patients with a simple elbow dislocation were enrolled in this multicentre randomised controlled trial. Patients were randomised to early mobilisation (n=48) or 3 weeks plaster immobilisation (n=52). Primary outcome measure was the Quick Disabilities of the Arm, Shoulder, and Hand (Quick-DASH) score. Secondary outcomes were the

Pneumonia and urinary tract infections develop in roughly 1/3 of the survivors of cerebral stroke. Post-stroke pneumonia results from aspiration and uroinfection from neurogenic disturbances of urination and bladder catheterization. Post-stroke immune deficiency increases the risk of infectious and inflammatory complications. These conditions are treated in accordance with the general principles of management of these diseases. Prophylaxis of pneumonia is reduced to the principle of "nothing orally" before the swallowing capacity is tested, early mobilization, and oral cavity hygiene. Studies

Neuromuscular complications of critical illness are common, and can be severe and persistent, with substantial impairment in physical function and long-term quality of life. While the etiology of ICU-acquired weakness (ICUAW) is multifactorial, both direct (ie, critical illness neuromyopathy) and indirect (ie, immobility/disuse atrophy) complications of critical illness contribute to it. ICUAW is often difficult to diagnose clinically during the acute phase of critical illness, due to the frequent use of deep sedation, encephalopathy, and delirium, which impair physical examination for patient

BACKGROUND AND PURPOSE: Multimodal techniques can aid early rehabilitation and discharge of patients following primary joint replacement. We hypothesized that this not only reduces the economic burden of joint replacement by reducing length of stay, but also helps in reduction of early complications.
PATIENTS AND METHODS: We evaluated 4,500 consecutive unselected total hip replacements and total knee replacements regarding length of hospital stay, mortality, and perioperative complications. The first 3,000 underwent a traditional protocol while the other 1,500 underwent an enhanced recovery p

OBJECTIVE: To determine the relationship of self-reported and clinical measures to the number of days to return to sport following acute lateral ankle sprains.
BACKGROUND: In order to direct rehabilitation, injury classification schemes should include self-reported and clinical measures that help prognosticate the number of days to return to sport (DAYS).
METHODS AND MEASURES: Twenty Division II college athletes (7 men, 13 women; mean age = 19.2 +/- 1.1 years) were assessed following an acute lateral ankle sprain and upon return to sp

The problems experienced by most patients with trochanteric fractures are caused by their advanced age, particularly multimorbidity and osteoporosis, and due to this early postoperative functional treatment is only possible with full weight-bearing ability. To avoid perforation of the femoral head by overstress and collapse of the weak cancellous bone, any stabilizing device should be designed with an H-beamed blade, providing a large load-bearing surface. In our experience the 130 degrees-Double-T-blade-Plate is an effective implant for the treatment of pertrochanteric fractures, offering hig

In this study the results of conservatively treated stable fractures of the thoracolumbar spine in 112 cases are reported. The follow-up of these patients was carried out by clinical and radiological examinations. It proved that functional treatment used in 94% of all cases was superior in its results and lack of pain to Böhler the method according of closed correction and immobilization by cast. The radiological examination showed no coherence between narrowing of the verebra after early mobilization and function of the spine. We divided 131 patients who had a subtrochanteric fracture into four clinical groups: elderly patients who had a fracture secondary to minor trauma, patients who had a fracture due to high-energy trauma, those who had a pathological fracture, and patients who had a fracture that previously had been treated unsuccessfully with internal fixation. In each patient operative fixation was performed using the Zickel device, and overall satisfactory results were obtained in 90 per cent. Considering the challenging nature of subtrochanteric fractures, complications, which included a rate of non-union o

Opinions differ on the desirability of postoperative relief following intertrochanteric adduction osteotomy in Legg-Calvé-Perthes' disease. We therefore undertook an analysis of our patients who were given postoperative relief with a Thomas splint up to 1974. Subsequently, patients were allowed to load the joint normally after the osteotomy had knitted. Comparison of the results in the two groups of patients indeed makes it questionable whether postoperative relief with Thomas splints is desirable. If adduction is performed in Stage I of Perthes' disease, relief appears neither beneficial nor

Management of the severely comminuted subtrochanteric fracture of the femur has been extremely difficult due to problems in stabilization, failure of fixation, and/or failure of the device, as well as associated high morbidity and mortality. Twenty-three such fractures treated with the Association for the Study of Internal Fixation (ASIF) blade plate fixation are reviewed. There were 17 primary unions (0 to 6 months), two delayed unions (6 to 12 months), and two nonunions with device failure, requiring reoperation and bone grafting before healing. There were two deaths, but only one was due to

BACKGROUND CONTEXT: Separation surgery has emerged as a key surgical strategy for metastatic spinal cord compression (MSCC), aiming to create a circumferential decompressive margin that allows safe delivery of postoperative radiotherapy. However, despite its widespread adoption, the clinical value of objectively confirming separation on early postoperative magnetic resonance imaging (MRI) remains unclear.
PURPOSE: To evaluate whether separation success confirmed by MRI at 2-3 weeks postoperatively is associated with improved neurological recovery, functional outcomes, and survival in patients
