Valve repair with autologous pericardium for organic lesions in rheumatic tricuspid valve disease.

BACKGROUND: Surgical repair of pathologic tricuspid valve disease often fails because of severe anatomic distortion of the valve apparatus, particularly in patients with rheumatic heart disease. This usually leads to tricuspid valve replacement despite the associated prosthesis-related complications. This study examines our experience of tricuspid valve repair with autologous pericardium for organic rheumatic tricuspid valve disease. METHODS: From 1996 to 2007, 31 patients underwent repairs for rheumatic tricuspid valve disease characterized by retracted leaflets and inadequate leaflet area.

Bioprosthetic vs. mechanical prostheses in patients aged 50-70 years undergoing aortic valve replacement surgery: outcomes analysis from propensity score matching.

AIMS: Bioprosthetic valves are increasingly used for surgical aortic valve replacement (SAVR) in patients ≤ 70 years though the relative benefits compared to mechanical valve replacement remain uncertain. This study aims to compare mortality and other outcomes by prosthesis type for patients aged 50-70 years who received SAVR in a well-characterized Australian cohort. METHODS AND RESULTS: Data were prospectively collected at the time of heart valve surgery and linked administrative outcome data. This analysis includes all patients aged 50-70 years who had SAVR in Western Australian public hos

Mid-term results of valve replacement with the ATS valve: a seven-year follow up.

BACKGROUND AND AIM OF THE STUDY: The ATS open pivot bileaflet valve has been the prosthesis of choice at the authors' hospital since 1997. Herein is presented a retrospective analysis of the mid-term results of valve replacement with the ATS valve prosthesis. METHODS: Between 1997 and 2005, a total of 280 patients (mean age 58.3 years) underwent valve replacement with the ATS valve; these included 133 aortic valve replacements (AVR), 115 mitral valve replacements (MVR), and 32 double (aortic plus mitral) valve replacements (DVR). Preoperatively, 162 patients (57.9%) were in NYHA functional cl

Low-field magnetic resonance imaging in the evaluation of mechanical and biological heart valve function.

Magnetic resonance imaging (MRI) has been frequently considered unsafe for patients with ferromagnetic implants: risks to be considered include induction of electric current, heating and dislocation of the prosthesis. Previous in vitro and in vivo studies have indicated the possibility of performing MRI examinations on patients with prosthetic heart valves. The aim of our study was to verify the presence of artifacts at the level of the prosthetic heart valve in vivo using a low-field MR unit (0.2 T) and to define the possibility of a functional analysis of the valve in patients with biomedica

Paraprosthetic Leakage and Infection Two Years after the Konno Procedure: A Case Report.

A middle-aged female presenting with progressive heart failure was admitted to the emergency department. She had a history of mitral and aortic valve replacement and a reoperation involving the Konno procedure. Echocardiography suggested a possible paraprosthetic leakage, which was confirmed during surgery. Additionally, severe infection, evidenced by an abscess near the great arteries, was discovered intraoperatively, despite the absence of fever or any prior signs of infection. This case underscores the importance of accurate diagnosis and thorough intraoperative exploration in identifying t

Commissural fusion as etiology of bioprosthetic mitral stenosis in a patient with rheumatic heart disease.

We report commissural fusion as a unique morphologic etiology of early bioprosthetic mitral valve failure in a woman with a history of rheumatic mitral stenosis. She had undergone mitral valve replacement with a 25-mm Edwards Magna Ease bovine pericardial bioprosthesis 3 years earlier and presented with progressive dyspnea. Transesophageal echocardiography revealed severe bioprosthetic stenosis due to commissural fusion. She underwent percutaneous valve-in-valve implantation with a 26-mm Edwards Sapien 3 prosthesis. Marked symptomatic improvement was noted postprocedurally. We speculate that c

[Myocardial infarction as a consequence of infective endocarditis in valvar bioprosthesis].

A sixteen year-old male, followed in the Unicamp Cardiology Clinic with a previous record of rheumatic heart disease and a biological mitral prosthesis, presented with infective endocarditis. In spite of a good initial response to antibiotic therapy, the patient died in two weeks. Autopsy disclosed endocarditis of the prosthetic valve and embolic occlusion of the circumflex branch of left coronary artery.

A totally endoscopic approach for aortic valve surgery.

OBJECTIVES: Our goal was to describe a new approach for totally endoscopic aortic valve replacement. METHODS: From October 2017 through December 2020, a total of 266 consecutive patients underwent totally endoscopic aortic valve replacement. Reoperations and combinations were excluded. RESULTS: A total of 266 patients with a median age of 72 (64, 79) years underwent totally endoscopic aortic valve replacement; of these, 250 (93.98%) patients were designated to undergo surgery because of aortic valve stenosis. The median follow-up index was 0.69 (0.30, 0.90). Major adverse cardiac and cerebro

Right-parasternal approach for aortic valve replacement and ascending aortic repair after ante-thoracic route gastric tube reconstruction.

Due to the limitations of surgical incisions and approaches brought on by the presence of gastric tube (GT), open heart surgery following ante-thoracic route GT reconstruction remains challenging. A-73-year-old man, who had a history of esophageal resection and ante-thoracic route GT reconstruction required aortic valve replacement (AVR) concomitant with ascending aortic repair (AAR) for aortic stenosis and dilated ascending aorta. We performed open heart surgery via a right-parasternal approach to avoid injury to the GT and nutrient arteries. This approach provided a good operative field, sim

Quadruple Valve Replacement for Carcinoid Heart Disease.

INTRODUCTION: Carcinoid heart disease most frequently involves the tricuspid or, more rarely, the pulmonary valve and presents with right heart failure as 5-HT is metabolized by the lung. Left-sided valve involvement is quite rare. We describe our experience of 3 patients presenting with heart failure secondary to carcinoid heart disease affecting all four cardiac valves. There are only four previous isolated case reports in the literature. METHODS: All three patients underwent quadruple valve replacement during a single operation. Right ventricular outflow tract reconstruction with a pericar

Trileaflet Semilunar Valve Reconstruction: Acute Porcine in Vivo Evaluation.

Objective: The surgical treatment of malformed semilunar valves in congenital heart defects is challenging in terms of providing both longevity and the potential to grow with the recipient. We investigated a new surgical technique "Trileaflet Semilunar Valve Reconstruction" in an acute porcine model, a technique with geometrical properties that could remain sufficient and allow for some growth with the child. Methods: An acute 60-kg porcine model was used. With echocardiography, baseline pulmonary valvular geometry and hemodynamics were investigated. On cardiopulmonary bypass, the pulmonary le
