Central venous pressure monitoring in critical care settings.

Patients who present with acute cardiovascular compromise require haemodynamic monitoring in a critical care unit. Central venous pressure (CVP) is the most frequently used measure to guide fluid resuscitation in critically ill patients. It is most often done via a central venous catheter (CVC) positioned in the right atrium or superior or inferior vena cava as close to the right atrium as possible. The CVC is inserted via the internal jugular vein, subclavian vein or via the femoral vein, depending on the patient and their condition. Complications of CVC placement can be serious, so its risks

Optimal management of the critically ill: anaesthesia, monitoring, data capture, and point-of-care technological practices in ovine models of critical care.

Animal models of critical illness are vital in biomedical research. They provide possibilities for the investigation of pathophysiological processes that may not otherwise be possible in humans. In order to be clinically applicable, the model should simulate the critical care situation realistically, including anaesthesia, monitoring, sampling, utilising appropriate personnel skill mix, and therapeutic interventions. There are limited data documenting the constitution of ideal technologically advanced large animal critical care practices and all the processes of the animal model. In this paper

Multi-modality monitoring of cystic fibrosis lung disease: the role of chest computed tomography.

Cystic fibrosis [CF] lung disease is characterized by progressive bronchiectasis and small airways disease. To monitor CF lung disease traditionally spirometry has been the most important modality. In addition to spirometry chest radiography was used to monitor progression of structural lung abnormalities. However, the importance of chest radiography in disease management has been limited due to its poor sensitivity and specificity to detect disease progression. Over the last decade chest CT has become the gold standard for monitoring the severity and progression of bronchiectasis. Small airwa

Neurologic monitoring on cardiopulmonary bypass: what are we obligated to do?

Improving survival from congenital cardiac repairs using cardiopulmonary bypass has appropriately shifted focus to neurologic outcomes. Hypoxic-ischemic mechanisms are the major cause of neurologic injury in neonatal cardiac surgery, and modifications of techniques of cardiopulmonary bypass can affect organ oxygen delivery and the propensity to injury both during and after surgery. Through successive refinements in the techniques of cardiopulmonary bypass, the risk factors for hypoxic-ischemic injury have been reduced, but not eliminated. The application of specific monitoring to enhance detec

Brain Death Determination During VA-ECMO After Pediatric Cardiac Surgery: A Case-Based Analysis of the 2023 Unified Consensus.

BACKGROUND: Determination of brain death/death by neurologic criteria (BD/DNC) in patients supported with extracorporeal membrane oxygenation (ECMO) is particularly challenging due to extracorporeal carbon dioxide clearance, which may interfere with apnea testing. The 2023 unified consensus guideline introduced stricter physiologic requirements, including a mandatory arterial pH threshold during apnea testing. METHODS: We report a pediatric patient supported with venoarterial ECMO following cardiac arrest in whom BD/DNC was diagnosed in 2021 according to the 2011 pediatric guideline. Apnea te

Weaning from venovenous extracorporeal membrane oxygenation for acute respiratory failure: challenges and opportunities.

PURPOSE OF REVIEW: The process of weaning from venovenous extracorporeal membrane oxygenation (V-V ECMO) is a critical step in the recovery of patients with severe acute respiratory distress syndrome (ARDS), yet clinical practice is highly variable and lacks strong evidence-based guidance. This review summarizes the current understanding and emerging data on weaning from V-V ECMO while highlighting key areas for future research. RECENT FINDINGS: While several single-center studies have evaluated structured weaning protocols, no definitive multicenter trial with patient-centered outcomes has b

Developing Biomarkers for the Skin: Biomarkers for the Diagnosis and Prediction of Treatment Outcomes of Alzheimer's Disease.

Alzheimer's disease (AD) is a neurodegenerative disorder characterized by memory decline and cognitive impairment. Research on biomarkers can aid in early diagnosis, monitoring disease progression, evaluating treatment efficacy, and advancing fundamental research. We conducted a cross-sectional longitudinal study to see if there is an association between AD patients and age-matched healthy controls for their physiologic skin characteristics, such as pH, hydration, transepidermal water loss (TEWL), elasticity, microcirculation, and ApoE genotyping. The study used the Mini-Mental State Examinati

The Swan-Ganz catheter: a review.

Right heart catheterization, first reported in 1905, is now used for bedside assessment and management of the critically ill patient. A Swan-Ganz catheter and pulmonary artery pressure monitoring are the tools employed. In spite of the complications cited, the occurrence of hazard to the patient is infrequent. The catheter is particularly valuable in the hemodynamic evaluation of cardiac function. Right and left heart pump performance can be assessed by pressure measurements. The ability of the left heart to provide an adequate cardiac output can be numerically calculated, and its adequacy at

Myocardial infarction during pregnancy: a team challenge.

Myocardial infarction during pregnancy is rare but may be increasing in frequency because of the trend toward later childbearing and an overall increase in risk factors for women (smoking, high-stress careers). The physiologic changes and stresses of pregnancy and labor provide a unique and often devastating setting for coronary diseases. The mortality rate is extremely high, approaching 40% if delivery ensues within 2 weeks of infarction. A planned, induced labor with regional anesthesia and continuous invasive monitoring in a well-equipped medical center provides the safest setting for deliv

Multimodal digital assessment of early recovery after chest tube omission in early-stage lung cancer patients undergoing wedge resection: a post hoc analysis of a prospective cohort.

BACKGROUND: Given the limited surgical trauma associated with wedge resection, the clinical impact of routine chest tube drainage may be more apparent in early postoperative recovery than in safety outcomes alone. We used a multimodal digital platform integrating wearable monitoring and electronic patient-reported outcomes (ePROs) to compare recovery after wedge resection with and without chest tube placement. METHODS: This study was a post hoc secondary analysis of prospectively collected data from a registered single-center observational cohort (NCT06118229). We included 98 patients from th

Effects of Arterial Carbon Dioxide Tension on Cerebral and Somatic Regional Tissue Oxygenation and Blood Flow in Neonates After the Norwood Procedure With Deep Hypothermic Cardiopulmonary Bypass.

Neonates undergoing the Norwood procedure for hypoplastic left heart syndrome are at higher risk of impaired systemic oxygen delivery with resultant brain, kidney, and intestinal ischemic injury, shock, and death. Complex developmental, anatomic, and treatment-related influences on cerebral and renal-somatic circulations make individualized treatment strategies physiologically attractive. Monitoring cerebral and renal circulations with near infrared spectroscopy can help drive rational therapeutic interventions. The primary aim of this study was to describe the differential effects of carbon d

Clinical benefits and risks of remote patient monitoring: an overview and assessment of methodological rigour of systematic reviews for selected patient groups.

BACKGROUND: Remote patient monitoring implies continuous follow-up of health-related parameters of patients outside healthcare facilities. Patients share health-related data with their healthcare unit and obtain feedback (which may be automatically generated if data are within a predefined range). The goals of remote patient monitoring are improvements for patients and reduced healthcare costs. The aim of this paper is to provide an overview of systematic reviews regarding remote patient monitoring for selected patient groups currently considered for the introduction of remote patient monitori

Use of shunt-side esCCO monitoring for hemodynamic management during hemodialysis as a novel approach for early detection of hypotension: a report of two cases.

Early recognition of the signs of hypotension may lead to prompt intervention by medical professionals, reducing the risk of intradialytic hypotension. The study included two patients on dialysis, one with stable hemodynamics (Case 1) and one who developed hypotensive shock (Case 2), in whom noninvasive continuous hemodynamic monitoring using shunt-side estimated continuous cardiac output (esCCO) was performed. Noninvasive blood pressure (BP), heart rate, esCCO, estimated stroke volume, systemic vascular resistance, and decrease in blood volume of each patient before and after dialysis were ev

Continuous Dual Glucose-Ketone Monitoring.

Diabetic ketoacidosis (DKA) occurring during pregnancy is an obstetric emergency that can result in significant adverse outcomes for both the pregnant person and the fetus. While DKA generally presents with glucose levels >250 mg/dL, up to 30% of DKA cases involve euglycemic DKA (euDKA) wherein glucose levels <200 mg/dL have been reported. However, detection and prevention of DKA in pregnancy can be challenging for both the pregnant individual and health care providers due to variable clinical presentations of the disease and the limitations of current ketone monitoring technologies. Non-Intrusive Monitoring and Detection of Mobility Loss in Older Adults Using Binary Sensors.

(1) Background and objective: Mobility is crucial for healthy aging, and its loss significantly impacts the quality of life, healthcare costs, and mortality among older adults. Clinical mobility assessment methods, though precise, are resource-intensive and economically impractical, and most of the existing solutions for automatic detection of mobility anomalies are either obtrusive or improper for long time monitoring. This study explores the feasibility of using non-intrusive, low-cost binary sensors for continuous, remote detection of mobility anomalies in older adults, aiming to identify b
