Periodic limb movement disorder (PLMD) and restless legs syndrome (RLS) are related sleep disorders that occur with increased frequency in spinal cord disease. Effective treatment may be obtained with dopaminergic or opioid drugs, while anticonvulsants, benzodiazepines, and possibly baclofen may be helpful. This report describes a patient who developed RLS and PLMD after acute transverse myelitis associated with infectious mononucleosis, and failed to respond to intrathecal baclofen. All symptoms of RLS/PLMD resolved after treatment with pergolide.

BACKGROUND: Splenic rupture by diffuse large B-cell lymphoma (DLBCL), which usually progresses insidiously, is extremely rare.
CASE PRESENTATION: A 60-year-old man presented with paralysis in his lower left extremity. A magnetic resonance imaging suggested transverse myelitis. No lymphadenopathy or organomegaly was noted. Two months after remission, he was referred to the emergency department complaining of presyncope. He was in preshock due to splenic rupture, and underwent laparotomy after attempts of transcatheter arterial embolization. Splenomegaly, hepatomegaly, and disseminated enlarged

Understanding the characteristics of neuromyelitis optica spectrum disorder (NMOSD) with recurrent short partial transverse myelitis (SPTM), which is very rare, contributes to the differential diagnosis of multiple sclerosis (MS). We present two Chinese aquaporin-4 immunoglobulin G (AQP4-IgG)-seropositive NMOSD cases who had at least twice SPTM during 4 and 6 years of follow-up, respectively. Their SPTMs have been mild and responded well to corticosteroids just like in the case of MS. The findings highlight the need of searching for serum AQP4-IgG (cell-based assay strongly recommended) in pat

Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune demyelinating condition of the central nervous system that may coexist with systemic lupus erythematosus (SLE), complicating diagnosis and management. We describe a woman in her 40s with long-standing SLE who presented with acute urinary retention, bilateral lower limb weakness, and diplopia following a febrile illness after unsuccessful in-vitro fertilization (IVF). Initial assessment suggested urinary tract infection, but rapid neurological deterioration prompted MRI, which revealed longitudinally extensive transverse myelitis (

Neuromyelitis optica spectrum disorder associated with aquaporin-4-antibodies (AQP4-NMOSD) is an autoimmune disease of the CNS with a high risk of visual, motor and sensory disability secondary to optic neuritis (ON) and transverse myelitis (TM) attacks. The degree of recovery is difficult to predict and may be affected by the extent of ensuing neurodegeneration. While neurodegeneration in AQP4-NMOSD is reported in the visual system after ON, its occurrence in patients with TM remains largely unknown. The aim of this study was to use advanced MRI to cross-sectionally examine the sensory and mo

Transverse myelitis (TM) is a rare neurologic disorder of acute inflammation resulting in spinal cord injury. Chronic pain in TM is a significant detriment to quality of life. Spinal cord stimulation (SCS) is an emerging treatment that has shown significant efficacy in neuropathic pain. We present a 37-year-old man with a history of idiopathic thoracic TM and refractory chronic neuropathic pain who underwent an SCS trial. He reported 70% improvement during the trial and was subsequently implanted with an SCS. He continues to experience significant pain relief and functional improvement (>80%)

Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune astrocytopathy frequently complicated by neuropathic pain. Paroxysmal symptoms likely arise from ephaptic transmission within demyelinated spinal cord tracts and are underdescribed when presenting as isolated sensory phenomena, unlike the well-characterized tonic spasms. We report a 76-year-old woman with aquaporin-4 (AQP4) antibody-positive NMOSD recovering from her first disease flare, which initially manifested as longitudinally extensive transverse myelitis (LETM). Weeks later, she developed sudden, excruciating, movement-prov

INTRODUCTION: Acute transverse myelitis (ATM) is a rare neurological disorder often associated with infections, autoimmunity, and vaccination. During the COVID-19 pandemic, the incidence of ATM appeared to increase; however, the causal relationship remains uncertain due to limited follow-up and variability in case reporting. This study aims to evaluate the incidence of ATM in SARS-CoV-2 patients, identify associated risk factors, and explore potential links to vaccination using real-world data.
METHODS: A retrospective cohort analysis was conducted using electronic health records from the USA

Transverse myelitis is a rare spinal cord disorder caused by local inflammation. Usually, this occurs as a complication from infection or autoimmune disease; however, there have been reported idiopathic causes such as vaccinations. A 73-year-old female with a medical history significant for Hashimoto's thyroiditis presented with new-onset paresthesias in her lower extremities. Her symptom onset was about five weeks after receiving influenza and tetanus, diphtheria, and pertussis (TDaP) vaccines. Magnetic resonance imaging (MRI) of the spine revealed an increased T2 signal of the lower cervical

Post-vaccination CNS demyelinating syndromes have been reported with a variety of vaccines including the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccines. We report a case of myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) probably associated with the mRNA-1273 (by Moderna) SARS-CoV-2 mRNA vaccine, and a case of acute transverse myelitis (ATM) probably associated with the BNT162b2 (by Pfizer-BioNTech) SARS-CoV-2 mRNA vaccine. A 38-year-old man developed left blurry vision, lower extremity weakness/paresthesia, and bowel/bladder dysfunction three day

OBJECTIVE: Deep venous thrombosis (DVT) is a severe complication in longitudinally extensive transverse myelitis (LETM) patients. It may interfere with LETM treatment and delay the recovery of the spinal dysfunction. However, there is less data about the prevalence and risk factors of DVT in patients with LETM. We analyzed data retrospectively to ascertain the prevalence of DVT and the clinical risk factors for DVT.
METHODS: Clinical data on 255 LETM patients were collected from medical records. All patients were performed color Doppler ultrasound(US) to screen DVT in both lower extremities w

BACKGROUND: Pediatric acute transverse myelitis (ATM) accounts for 20-30% of children presenting with a first acquired demyelinating syndrome (ADS) and may be the first clinical presentation of a relapsing ADS such as multiple sclerosis (MS). B cells have been strongly implicated in the pathogenesis of adult MS. However, little is known about B cells in pediatric MS, and even less so in pediatric ATM. Our lab previously showed that plasmablasts (PB), the earliest B cell subtype producing antibody, are expanded in adult ATM, and that these PBs produce self-reactive antibodies that target neuron

Aims/Background Although electromyography has been extensively used in the diagnosis of neurological diseases, there is no comprehensive understanding of the electromyography manifestations of spinal dural arteriovenous fistula. Given the widespread use of electromyography in the diagnosis of neurological conditions, it is worthwhile to holistically analyse the electromyography findings of spinal dural arteriovenous fistula to differentiate it from neurological diseases that share similar clinical manifestations. The aim of this study is to evaluate whether electromyography can distinguish spi

Background: Acute transverse myelitis (ATM) is an inflammatory disorder of the spinal cord with heterogeneous etiologies, including autoimmune, infectious, paraneoplastic, and drug-induced causes. Tumor necrosis factor-alpha (TNF-α) inhibitors have been infrequently associated with inflammatory central nervous system events, including transverse myelitis. TNF-inhibitor-associated myelitis typically presents with short-segment lesions, a normal brain MRI, and partial responsiveness to corticosteroids. Longitudinally extensive transverse myelitis (LETM) and steroid-refractory cases are uncommon.

We report a case of primary central nervous system vasculitis (PCNSV) with longitudinally extensive transverse myelitis. A 47-year-old woman presented with malaise, progressive cognitive impairment, and lower limb spasticity. Diffuse hyperintense areas in the deep cerebral white matter on the diffusion-weighted image and T2-weighted images were observed during brain magnetic resonance imaging. Gadolinium-enhanced T1-weighted images showed multiple linear enhancements. A sagittal T2-weighted image displayed a longitudinal extensive lesion of transverse myelitis in the spinal column from the upp
