Application of percutaneous nephrolithotomy combined with da Vinci Xi robot in complicated upper urinary tract repair surgery: A case report.

Duplex kidney and ureter is a congenital malformation. Few patients present with hydronephrosis caused by obstruction of the ureteropelvic junction of the duplex kidney, but lower kidney calculi caused by a duplex kidney abnormality is rare. This study reports a case of a duplex kidney and ureter complicated by multiple calculi in the duplex lower kidney. Percutaneous nephrolithotomy combined with a da Vinci robot-assisted laparoscopic upper urinary tract reconstruction was performed. The lower ureter was resected, and the lower kidney was preserved. One year after the surgery, a follow-up exa

Ultrasonography-guided percutaneous nephrolithotomy with Chinese one-shot tract dilation technique based on stimulated diuresis: A report of 67 cases.

The safety and effectiveness of a novel Chinese one-shot dilation technique based on stimulated diuresis for percutaneous nephrolithotomy (PCNL) were investigated. After the feasibility of the Chinese one-shot dilation based on stimulated diuresis was verified by an animal study, this technique was applied in the clinical practice. A total of 67 patients in our department underwent the modified PCNL from July 2014 to June 2015. After the renal infundibulum was distended by stimulated diuresis, the kidney was punctured under the ultrasonographic guidance via the fornix of the target calyx. The

Instillation of skin, nephrostomy tract, and renal puncture site with ropivacaine decreases pain and improves ventilatory function after percutaneous nephrolithotomy.

BACKGROUND AND PURPOSE: Pain after percutaneous nephrolithotomy (PCNL) is well investigated, but no optimal management strategy has yet been defined. Ventilatory changes after uncomplicated PCNL remain obscure. We investigated whether pain can be managed with a combination of a parenteral non-narcotic drug and instillation of a local anesthetic into the operative field. We also measured ventilatory changes early after PCNL to determine whether this analgesic modality improves ventilatory status. PATIENTS AND METHODS: In a randomized blinded study, 34 well-matched patients underwent PCNL with

i-PCNL: a novel ureteral-catheter application for continuous unidirectional high-flow irrigation during Percutaneous Nephrolithotomy. Technique description and preliminary outcomes.

INTRODUCTION: Percutaneous nephrolithotomy (PCNL) remains the technique of choice for the treatment of complex renal stones. Despite technical advances, limitations remain in terms of visibility, fragment migration, and effective evacuation of the material. Optimizing irrigation is the key to improve the efficiency and safety of the procedure. MATERIALS AND METHODS: A prospective observational study was done at one referral center, including 14 patients undergoing supine PCNL. A continuous retrograde irrigation technique directed at the upper renal pole was evaluated by adapting commercially

Development and validation of an interpretable machine learning model for predicting systemic inflammatory response syndrome after percutaneous nephrolithotomy: A multicenter study.

BACKGROUND: Systemic inflammatory response syndrome (SIRS) is a common and potentially reversible early infectious complication after percutaneous nephrolithotomy (PCNL). Accurate perioperative risk stratification may enable timely intervention; however, existing prediction models show limited generalisability and poor clinical interpretability. METHODS: We conducted a multicenter retrospective cohort study including patients who underwent PCNL at three hospitals in China between Jan 1, 2015, and Dec 30, 2024. Patients from one center were randomly divided into training and internal validatio

Association between intraoperative low renal pelvic pressure time and postoperative infectious complications after percutaneous nephrolithotomy: a single-centre retrospective cohort study.

BACKGROUND: Percutaneous nephrolithotomy (PCNL) is frequently complicated by infectious events, some of which progress to sepsis. Conventional pressure metrics are usually based on instantaneous values, such as peak or mean pressure, and therefore fail to capture the temporal dimension of pressure exposure. This study proposes “time at low pressure” as a novel exposure metric to evaluate its association with infectious complications after PCNL. METHODS: We conducted a single-centre retrospective cohort study including adult patients who underwent PCNL with continuous intraoperative renal pelvi

Ultrasound-guided erector spinae plane block versus anterior quadratus lumborum block for postoperative analgesia after percutaneous nephrolithotomy: A randomized controlled study.

BACKGROUND: Percutaneous nephrolithotomy (PCNL) is associated with significant postoperative pain. Ultrasound-guided erector spinae plane block (ESPB) and anterior quadratus lumborum block (QLB) are increasingly used for analgesia; however, direct comparative evidence in PCNL remains limited. This randomized controlled study compared their analgesic efficacy following unilateral PCNL. METHODS: Eighty-four adult patients (American Society of Anaesthesiologists I-III) were randomized to receive ESPB or anterior QLB. Three patients in each group had block failure. The primary outcome was time to

Can dynamic changes in preoperative urinary white blood cells and nitrite predict infectious complications after percutaneous nephrolithotomy in culture-positive patients?

BACKGROUND: Percutaneous nephrolithotomy (PCNL) frequently encounters postoperative infectious complications, notably fever and urosepsis. Although preoperative urine culture guides antibiotics, delayed reporting hampers timely risk stratification. Urinalysis is simple, cost-effective, and repeatable; however, most existing studies focus on single static measurements and lack systematic evaluation of temporal trends. OBJECTIVE: To evaluate the association between temporal changes in preoperative urinary white blood cells (uWBC) and nitrite (NIT) and the risk of postoperative fever and uroseps

Evaluation of non-papillary access on the 3D-printed renal collecting system model in fluoroscopic supine percutaneous nephrolithotomy: a pilot study.

OBJECTIVE: Fluoroscopic access techniques may not reliably differentiate between papillary and non-papillary renal access during percutaneous nephrolithotomy (PNL), potentially leading to misinterpretations that could increase complication risks. This pilot study aimed to evaluate whether a novel radiopaque 3D-printed collecting system model could be used to investigate the feasibility of identifying non-papillary accesses using different biplanar fluoroscopic techniques in the supine PNL setting. MATERIALS AND METHODS: This experimental study was conducted in February 2023 using a 3D-printed

Clinical Efficacy of 10 Min of Active Prewarming for Preserving Patient Body Temperature during Percutaneous Nephrolithotomy: A Prospective Randomized Controlled Trial.

Background: Percutaneous nephrolithotomy (PNL) poses a risk of hypothermia. Additionally, general anesthesia lowers the thresholds for shivering and vasoconstriction, which leads to dysfunction of central thermoregulation. Perioperative hypothermia is associated with adverse outcomes after surgery. In this study, we aimed to demonstrate that prewarming for 10 min can effectively prevent early hypothermia during PNL. Methods: A total of 68 patients scheduled for elective PNL were recruited to this study from January to June 2022, but two patients were excluded because of a change in the surgica

Outcomes in Lower Pole Kidney Stone Management Using Mini-Percutaneous Nephrolithotomy Compared With Retrograde Intra Renal Surgery: A Randomized Controlled Trial.

Background Because of the anatomical properties of the inferior calyx, lower pole stones are difficult to remove through the ureter, even if the stones are fragmented. Retrograde intra-renal surgery (RIRS) is typically employed to treat the smaller lower pole stones (1.0-2.0 cm) while percutaneous nephrolithotomy (PCNL) is primarily used to treat the larger diameter stones or when RIRS has failed to clear the stones. This study was conducted to compare mini-PCNL and RIRS for the management of lower pole kidney stones in terms of stone clearance. Material and methods This randomized control tri

Mini Versus Standard Percutaneous Nephrolithotomy for the Management of Renal Stone Disease: Systematic Review and Meta-analysis of Randomized Controlled Trials.

CONTEXT: Mini percutaneous nephrolithotomy (mPNL) involves the creation of a smaller access tract compared with standard PNL (sPNL). Smaller tract sizes could lead to decreased blood loss, pain, and need for blood transfusion. Previous studies on this topic have reported variable findings and were of poor quality. OBJECTIVE: To compare the safety and efficacy of mPNL with those of sPNL for the management of patients with renal stone disease by conducting a systematic review and meta-analysis. EVIDENCE ACQUISITION: Systematic literature search was performed to identify relevant randomized con

A Novel Technique Using a Thulium Fiber Laser for Simultaneous Percutaneous Nephrolithotomy and Transpelvic Endopyelotomy for High-Insertion Ureteropelvic Junction Obstruction.

Background: Endopyelotomy is a minimally invasive option for treatment of ureteropelvic junction (UPJ) obstruction. Although largely supplanted by laparoscopic or robot-assisted laparoscopic pyeloplasty, it retains efficacy and utility in the absence of a crossing vessel in patients not fit for laparoscopy, patients with secondary obstructions or strictures, or those with stones requiring simultaneous treatment. Antegrade endopyelotomy is most commonly performed with scissors, cold knife, or more recently, using a Holmium laser. Herein we present the first reported case of simultaneous antegra
