Initiation of male sperm-transfer behavior in Caenorhabditis elegans requires input from the ventral nerve cord.

BACKGROUND: The Caenorhabditis elegans male exhibits a stereotypic behavioral pattern when attempting to mate. This behavior has been divided into the following steps: response, backing, turning, vulva location, spicule insertion, and sperm transfer. We and others have begun in-depth analyses of all these steps in order to understand how complex behaviors are generated. Here we extend our understanding of the sperm-transfer step of male mating behavior. RESULTS: Based on observation of wild-type males and on genetic analysis, we have divided the sperm-transfer step of mating behavior into fou

Posterior approach for double nerve transfer for restoration of shoulder function in upper brachial plexus palsy.

Restoration of shoulder function is one of the most critical goals of treatment of brachial plexus injuries. Primary repair or nerve grafting of avulsion injuries of the upper brachial plexus in adults often leads to poor recovery. Nerve transfers have provided an alternative treatment with great potential for improved return of function. Many different nerves have been utilized as donor nerves for transfer to the suprascapular nerve and axillary nerve for return of shoulder function with variable results. As our knowledge of shoulder neuromuscular anatomy and physiology improves and our exper…

Reporter expression persists 1 year after adeno-associated virus-mediated gene transfer to the optic nerve.

OBJECTIVE: To determine the foci and duration of protein expression following virus-mediated gene transfer to the optic nerve. METHODS: A cytomegalovirus (CMV) promoter was linked to a lacZ-SV40 polyA reporter gene or a humanized green fluorescent protein (hgfp) reporter gene, then inserted into a bacterial plasmid containing adeno-associated virus (AAV) terminal repeat sequences. The CMV-lacZ or the CMV-hgfp construct were injected into the vitreous cavity of strain-13 guinea pigs. Controls consisted of eyes injected with AAV without the promoter and reporter elements or eyes that received n

Low-output lymphatic leakage after contralateral seventh cervical nerve cross transfer: a clinical and anatomical observation.

BACKGROUND: Contralateral seventh cervical nerve (CC7) cross transfer restores upper limb function in hemiplegia after brain injury. As a cervical procedure involving dissection near major lymphatic structures, CC7 transfer carries a potential risk of lymphatic leakage, a complication well described in other neck surgeries but not systematically studied in this context. METHODS: This study aims to evaluate lymphatic leakage after CC7 transfer through a combined clinical analysis and cadaveric anatomical study. We retrospectively reviewed 507 patients who underwent CC7 transfer from May 2023 to

Comparative Evaluation of Adeno-Associated Virus and Lentivirus Mediated Gene Transfer in Adult Rat Optic Nerve.

BACKGROUND: The optic nerve serves as a vital conduit for visual signaling, and its degeneration in optic neuropathy results in irreversible vision loss. It is also a widely used model for studying central nervous system (CNS) injury and repair. Although adeno-associated virus (AAV) and lentivirus are extensively applied in CNS research, their transduction efficiency and cell-type specificity within the optic nerve remain poorly characterized. This study aimed to identify the most effective viral vector, serotype, and promoter for direct gene delivery to the adult rat optic nerve. METHODS: Sp

Dental pulp stem cells alleviate Schwann cell pyroptosis via mitochondrial transfer to enhance facial nerve regeneration.

Dental pulp stem cells (DPSCs) have demonstrated remarkable potential in enhancing peripheral nerve regeneration, though the precise mechanisms remain largely unknown. This study investigates how DPSCs alleviate Schwann cell pyroptosis and restore mitochondrial homeostasis through intercellular mitochondrial transfer. In a crab-eating macaque model, we first observed that DPSC-loaded nerve conduits significantly promoted long-term nerve regeneration, facilitating tissue proliferation and myelin recovery. We further established a rat facial nerve injury (FNI) model and found that DPSC treatment

Established and novel quantitative electromyographic techniques with potential for planning nerve transfer surgery: a pilot study.

Donor nerve health likely underlies much variability in nerve transfer outcomes. Standard electromyography (EMG) and clinical examination suffer from subjectivity and a lack of standardization when assessing nerve health. Quantitative electromyography promises to assess nerve health more accurately. The objective of this retrospective pilot study was to evaluate rapid and feasible quantitative electromyography methods in determining donor nerve health for planning nerve transfer surgery and its correlation with functional outcomes. For this study, the branch of the radial nerve supplying the s…

Does age matter? The role of patient age in brain plasticity analyzed after a nerve transfer.

OBJECTIVE: Nerve transfers, which are increasingly applied not only for traumatic peripheral nerve injuries but also in oncologic reconstructions, spinal cord injuries, and a wide range of neurological disorders, rely heavily on the capacity of the central nervous system to reorganize and reassign motor control pathways. Among the multiple factors influencing postoperative recovery, age-dependent brain plasticity appears to play a critical role in determining the degree to which patients can regain functional control after reinnervation. This study aims to characterize this correlation in dept…

Acute Flaccid Myelitis: Mid-Term Clinical Course of Knee Extension Paralysis and Outcomes of Nerve Transfer.

BACKGROUND: Acute flaccid myelitis (AFM) is a rare debilitating poliomyelitis-like illness characterized by the sudden onset of flaccid palsy in the extremities. The purpose of this study was to report the mid-term clinical course of knee extension in AFM and the effect of contralateral obturator nerve-to-femoral nerve transfer (CONFNT) for restoration of knee extension in AFM. METHODS: Twenty-six patients with lower extremity palsy due to AFM were referred to our clinic for possible surgical reconstruction. Their median age was 4.0 years, and the first evaluation of the palsy was done at a m…

Volar versus dorsal approach for supinator to posterior interosseous nerve transfer: An anatomical study in cadavers.

INTRODUCTION: Supinator to posterior interosseous nerve (SPIN) transfer allows reconstruction of finger/thumb extension and thumb abduction for low radial nerve palsy, incomplete C6 tetraplegia, and brachial plexus injury affecting C7-T1. No study has compared dorsal versus volar approach to perform SPIN transfer. This comparison is studied in the present work, assessing supinator motor branch length and ability to achieve nerve transfer from either approach. METHODS: Ten fresh frozen cadavers were randomly allocated to receive either a dorsal or volar approach to PIN and supinator radial and…

Partial tibial nerve transfer for foot drop from deep peroneal palsy: Lessons from three pediatric cases.

Peroneal nerve palsy with resultant foot drop has significant impacts on gait and quality of life. Traditional management includes ankle-foot-orthosis, tendon transfer, and arthrodesis-each with certain disadvantages. While nerve transfers for peroneal nerve injury have been reported in adults, with variable results, they have not been described in the pediatric population. We report the use of partial tibial nerve transfer for foot drop from deep peroneal nerve palsy in three pediatric patients. The first sustained a partial common peroneal nerve laceration and underwent transfer of a single …

Novel method for restoration of anorectal function following spinal cord injury via nerve transfer in rats.

OBJECTIVES: Nerve transfer has been developed to restore partial function after serious nerve injuries, for example, restoring bladder control after spinal cord injury (SCI). Our aim here was to establish a preclinical proof-of-concept model using nerve transfer for restoring anorectal function after SCI. SETTING: We used laminectomy to model SCI, and bilateral spinal ventral and dorsal nerve root anastomosis to re-establish connectivity to the anorectal musculature. METHODS: Multidisciplinary methods were used to assess the anatomical and functional integrity of the alternative spinal-to-an…
