Intraoperative motor evoked potential monitoring to patients with preoperative spinal deficits: judging its feasibility and analyzing the significance of rapid signal loss.

BACKGROUND CONTEXT: Transcranial motor evoked potential (MEP) monitoring has been widely adopted in spine surgery, but so far the useful monitoring data for patients with preoperative spinal deficits (PPSDs) are limited. Originally we thought that they seemed technically more difficult and less reliable in performing the MEP monitoring to PPSDs. PURPOSE: Our objective was to study (1) the feasibility of MEP monitoring in PPSDs and the (2) the significance of rapid MEP loss. STUDY DESIGN/SETTING: A retrospective case notes study from a prospective patient register was used as the study design

Anesthetic Management of a Pediatric Patient With Mitochondrial Depletion Syndrome and Hypertrophic Cardiomyopathy Undergoing Scoliosis Correction: A Case Report.

This report describes a case of a 15-year-old pediatric patient with mitochondrial depletion syndrome, associated with hypertrophic cardiomyopathy, who underwent scoliosis correction surgery under neurophysiological monitoring. As propofol is mitochondrion-toxic, the anesthetic approach was modified to involve dexmedetomidine infusion combined with boluses of midazolam and ketamine. The patient was hemodynamically stable during surgery with effective neuromonitoring readings, and the surgery was concluded uneventfully. This case highlights the safety and efficacy of this anesthetic regimen in …

Continuous somatosensory evoked potentials monitoring is highly sensitive to intraoperative occlusion of iliac artery during anterior lumbar interbody fusion: case report.

We report a case of thrombotic occlusion of the left common iliac artery during an L5-S1 anterior interbody fusion exposed via a retroperitoneal approach. The loss of distal blood flow was detected by loss of cortical and peripheral somatosensory evoked potentials on the left lower extremity. Restoration of the blood flow resulted in gradual return of evoked potentials of the involved extremity. The neurophysiological and pulse oximetry monitoring of the lower extremities are extremely sensitive for an early detection of thrombotic occlusions and vascular complications.

Multi-channel motor evoked potential monitoring during anterior cervical discectomy and fusion.

OBJECTIVES: Anterior cervical discectomy and fusion (ACDF) surgery is the most common surgical procedure for the cervical spine with low complication rate. Despite the potential prognostic benefit, intraoperative neurophysiological monitoring (IONM), a method for detecting impending neurological compromise, is not routinely used in ACDF surgery. The present study aimed to identify the potential benefits of monitoring multi-channel motor evoked potentials (MEPs) during ACDF surgery. METHODS: We retrospectively reviewed 200 consecutive patients who received IONM with multi-channel MEPs and soma…

The Intersegmental Pudendal Reflex: Evidence of human sacral-to-thoracolumbar connectivity for intraoperative monitoring.

OBJECTIVE: To introduce the Intersegmental Pudendal Reflex (IPR) and evaluate its potential as a surrogate functional marker for S2-S4 integrity and for extended suprasegmental levels during Intraoperative Neurophysiological Monitoring (IOM). METHODS: Observational retrospective study involving twenty patients undergoing spinal surgery. Pudendal nerve stimulation elicited sacral and suprasacral responses. Reflexive responses were recorded simultaneously in the external anal sphincter (bulbocavernosus reflex, BCR) and the abdominal oblique muscles (IPR). RESULTS: The IPR consists of a short-l…

Traditional growing rod lengthening without intraoperative neuromonitoring: a 20-year institutional analysis demonstrating neurologic safety and cost savings.

PURPOSE: To evaluate the neurologic safety and economic implications of performing traditional growing rod (TGR) lengthening procedures for early-onset scoliosis (EOS) without intraoperative neuromonitoring (IONM). Although IONM is widely adopted for deformity correction, its necessity during routine distraction lengthening procedures remains uncertain. METHODS: A retrospective cohort study was conducted of pediatric EOS patients (< 18 years) who underwent TGR lengthening procedures between 2000 and 2020 at a single tertiary children's hospital. Inclusion required documented neurologic assess

Direct (D)-Wave Monitoring Enhancement With Subdural Electrode Placement: A Case Series.

PURPOSE: Direct-wave (D-wave) neuromonitoring is a direct measure of corticospinal tract integrity that detects potential injury during spinal cord surgery. Epidural placement of electrodes used for D-wave measurements can result in high electrical impedances resulting in substantial signal noise that can compromise signal interpretation. Subdural electrode placement may offer a solution. METHODS: Medical records for consecutive patients with epidural and subdural D-wave monitoring were reviewed. Demographic and clinical information including preoperative and postoperative motor strength were

Improving intraoperative evoked potentials at short latency by a novel neuro-stimulation technology with delayed return discharge.

OBJECTIVE: The intraoperative monitoring of cranial nerve function records evoked responses at latencies of a few milliseconds. Unfortunately, these responses may be masked by the electrical artifact of the stimulation pulse. In electrical stimulation, the return discharge of the stimulation pulse significantly contributes to the width of the electrical artifact. METHODS: We have generated stimulation pulses with an ISIS Neurostimulator (inomed Medizintechnik GmbH) providing a novel stimulation artifact reduction technique. It delays the return discharge of the stimulating pulse beyond the la

Training Courses in Laryngeal Nerve Monitoring in Thyroid and Parathyroid Surgery- The INMSG Consensus Statement.

Intraoperative neural monitoring (IONM) is now an integral aspect of thyroid surgery in many centers. Interest in IONM and the number of institutions that perform monitored thyroidectomies have increased throughout the world in recent years. For surgeons considering the introduction of IONM in their practice, specific training in IONM devices and procedures can substantially shorten the learning curve. The International Neural Monitoring Study Group (INMSG) has been at the forefront of IONM technology and procedural adoption since the introduction of neural monitoring in thyroid and parathyroi

Use of postoperative neurophysiological testing to help guide management in a case of delayed neurological injury.

Bimodal intraoperative neuromonitoring (IONM), combining transcranial motor-evoked potentials (tcMEP) and somatosensory-evoked potentials (SSEP), enables real-time detection and prevention of spinal cord injury during pediatric spinal deformity correction. Although rare, false-positive and false-negative signal alerts have been reported. However, no previously published accounts have described the use of postoperative neurophysiological testing to both identify new-onset neurological injury and guide reintervention. Here, we describe the case of an 18-year-old young man with achondroplasia, th

Anesthetic Considerations of Intraoperative Neuromonitoring in Thyroidectomy.

BACKGROUND: Intraoperative neuromonitoring (IONM) might reduce the incidence of injury to the recurrent laryngeal nerve (RLN) during thyroidectomy. Although dislocation of endotracheal tube surface electrodes can lead to false-positive IONM results (loss of signal), the risk factors for dislocation and the effects of muscle relaxants are unclear. Therefore, to identify factors that affect IONM results, we examined the frequency and risk factors for tube dislocation after cervical extension before surgery, the effect of sugammadex administration, and the correlation between IONM results and pos

An electroneurography-based assay for identifying injured nerve segment during surgery: design and in vivo application in the rat.

OBJECTIVE: Nerve injury is the main reason for nerve reconstruction surgery, during which the surgeon must determine the location of the injured nerve segment, resect it, and reconnect the remaining healthy nerve stump ends within a limited time. Given this importance, an assay needed to determine the exact location of the injured nerve segment, but no tool has yet fulfilled this need so that a visual inspection of the nerve is still the primary method of identifying the injured segment. APPROACH: We designed a flexible multi-electrode array sensor that records the electroneurographic signal

Resection of a Superior Mediastinal Mature Teratoma Using Intraoperative Neural Monitoring.

A 33-year-old woman presented with a right cervical mass. Contrast computed tomography showed a multilocular tumor with a clear border and heterogeneous contents including fat and calcification. The tumor was located adjacent to the vagus and recurrent nerves. To avoid injury of these nerves, we resected the tumor through a median sternotomy and right cervical lateral incision. Intraoperative neural monitoring was performed using an NIM TriVantage EMG tube (Medtronic, Minneapolis, MN). After the surgery, no neuropathy such as hoarseness was recognized. Pathological diagnosis showed a benign ma

Impact of continuous intraoperative vagus stimulation on intraoperative decision making in favor of or against bilateral surgery in benign goiter.

The advent of continuous vagus stimulation (CVS), eliminating lag time between nerve preparation with potential trauma and stimulation, has transformed the intraoperative surgical strategy in thyroid surgery. Continuous intraoperative nerve monitoring empowers the surgeon to be optimally aware of traction-related injury to the recurrent laryngeal nerve (RLN). Electromyographic precursor lesions, called combined events, prompt surgeons to cease harmful surgical maneuvers and release the nerve before damage to the nerve is established. Complete RLN recovery, defined as restitution of the nerve a
