Maintaining Osseointegration: Understanding, preventing, and treating Risk Factors for Peri-implant Diseases.

The dental implant is a predictable way of restoring fully and partially edentulous patients and has shown high success rate for managing a broad range of clinical conditions. Improving implant surfaces has revolutionized the osseointegration of implants and changed the focus of implant research from osseointegration more toward the risk factors associated with the failure of implants in the long-term.

Clinical outcomes of an osteotome technique and simultaneous placement of Neoss implants in the posterior maxilla.

BACKGROUND: Insufficient bone volume often hamper placement of dental implants in the posterior maxilla. PURPOSE: The aim of the present clinical study was to evaluate retrospectively the clinical outcome of implant placement in the resorbed posterior maxilla using an osteotome technique without adding any grafting material. MATERIALS AND METHODS: Twenty patients with 5 to 9 mm of residual alveolar bone height in the posterior maxilla received twenty-nine implants (Neoss Ltd., Harrogate, UK) using an osteotomy technique without bone grafts. Intraoral radiographs were taken before and after i

Soft Tissue Reconstruction After Total Ankle Arthroplasty.

BACKGROUND: Wound complications following total ankle replacement (TAR) may potentially lead to devastating consequences. Soft tissue coverage of the prosthesis and tendons with a flap potentially prevent a catastrophic cascade leading to infection and implant failure. The aim of this study was to investigate the success and complications of flaps following soft tissue defects as a result of total ankle arthroplasty. METHODS: We retrospectively identified a consecutive series of 1001 primary TARs performed between January 1998 and December 2014 whose data were prospectively collected. We then

Bone stress and interfacial sliding analysis of implant designs on an immediately loaded maxillary implant: a non-linear finite element study.

OBJECTIVES: This study investigated the surrounding bone stress and the implant-bone interfacial sliding of implant designs and implant sizes of immediately loaded implant with maxillary sinus augmentation by using three-dimensional (3D) non-linear finite element (FE) analysis. METHODS: Twenty-four FE models including four implant designs (cylindrical, threaded, stepped and step-thread implants) and three implant dimensions (standard, long and wide threaded implants) with a bonded and three levels of frictional contact of implant-bone interfaces were analyzed. The maxillary model was construc

Evaluation of 31 zygomatic implants and 74 regular dental implants used in 16 patients for prosthetic reconstruction of the atrophic maxilla with cross-arch fixed bridges.

BACKGROUND: The use of a specially designed implant to be anchored in the zygomatic body has been proposed as an alternative to bone grafting in the prosthetic rehabilitation of the severely resorbed maxilla. However, few studies have evaluated the long-term stability and soft tissue conditions of zygomatic implants. PURPOSE: The aim of this retrospective study was to evaluate the clinical performance of zygomatic implants when used for prosthetic reconstruction of atrophic maxillae. MATERIALS AND METHODS: Sixteen patients consecutively treated with 31 zygomatic implants and 74 additional de

Preliminary report of immediately loaded Altiva Natural Tooth Replacement dental implants.

BACKGROUND: Osseointegration may be described as a direct contact between living bone and an alloplastic implant. It is thought that a period of undisturbed healing is required to ensure osseointegration. The length of the healing period is dependent on a number of factors, such as implant material, surface configuration, site preparation technique, bone quality, healing capacity of the osteotomy, and implant design. Elimination of the healing period offers distinct advantages in terms of cost of therapy and convenience to patients. Recently, a new one-piece implant design was proposed that pr

Secondarily stabilized dental implants: an alternative to sinus grafting--early clinical results of 63 consecutively placed implants.

PURPOSE: A standard dental implant modified with an apical cross-screw was studied clinically in the posterior maxilla in a trial of 63 sinus-penetrating implants. PATIENTS AND METHODS: Twenty-five patients who had cross-screw implants placed in the atrophic posterior maxilla were followed for up to 3 years. RESULTS: Of 63 implants placed, 5 implants failed over a 4-year study period for a 92.1% success rate. Despite the implant body penetrating into the sinus cavity, no long-term complication resulted. CONCLUSION: Threaded dental implants that were stabilized secondarily with a cross-screw

Novel implant design improves implant survival in multirooted extraction sites: a preclinical pilot study.

BACKGROUND: The primary aim is to evaluate clinical, radiographic, and histologic parameters of novel implants with "three roots" design that were inserted into fresh multirooted extraction sockets. A secondary aim is to compare this new implant to standard root-form dental implants. METHODS: Immediate implantation of novel or standard design 6 × 6-mm implants was performed bilaterally into multirooted sockets in mandibles of mini-pigs. Twelve weeks later, clinical, radiographic, stability, histomorphometric, and microcomputed tomography (micro-CT) analyses were performed. RESULTS: Survival

Within-subject comparisons of implant-supported mandibular prostheses: choice of prosthesis.

Although previous studies have demonstrated that implant-supported prostheses are more satisfactory and efficient for edentulous patients than are conventional prostheses, until now no investigation has directly compared different types of implant-supported prostheses. We carried out a within-subject cross-over clinical trial with fixed and long-bar removable implant-supported mandibular prostheses. Fifteen subjects were randomly divided into two groups. One group received the fixed prosthesis first, while the other first received the removable. After a two-month adaptation period, psychometri

Angled Screw Channel: An Alternative to Cemented Single-Implant Restorations--Three Clinical Examples.

This article presents three cases of single labially tilted implants restored with screw-retained single crowns. Individualized abutments with an angled screw channel were used to avoid an unesthetic vestibular access channel. This individualized abutment allows the dentist and dental technician to use the screw-retained restorations where a cemented reconstruction would otherwise have been needed.

Influence of the implant cervical topography on the crestal bone resorption and immediate implant survival.

The aim of the study was the investigation of the survival rate of immediate implants in addition to the evaluation of the level of the alveolar bone around the neck region of immediately placed implants of different macrodesigns. To address the question whether the "biological", highly polished area of the implant neck is more associated with crestal resorpiton than the rough neck region, the influence of the surface characters of the implant cervical region was studied. The survival rate of 129 implants from 52 patients was evaluated on different time points after immediate implantation. The

Swine Pudendal Nerve as a Model for Neuromodulation Studies to Restore Lower Urinary Tract Dysfunction.

Lower urinary tract dysfunction, such as incontinence or urinary retention, is one of the leading consequences of neurological diseases. This significantly impacts the quality of life for those affected, with implications extending not only to humans but also to clinical veterinary care. Having motor and sensory fibers, the pudendal nerve is an optimal candidate for neuromodulation therapies using bidirectional intraneural prostheses, paving the way towards the restoration of a more physiological urination cycle: bladder state can be detected from recorded neural signals, then an electrical cu

Esthetic and Functional Rehabilitation: Retreatment of Anterior Fixed Dental Prothesis With Biologically Oriented Preparation Technique and Digital Workflow.

OBJECTIVE: This clinical case describes a multidisciplinary retreatment of a patient with anterior fixed dental prostheses (FDPs) using minimally invasive restorations and a biologically oriented preparation technique (BOPT). CLINICAL CONSIDERATIONS: A 56-year-old female patient, treated 30 years ago with a metal-ceramic FDP due to dental agenesis, presented a misfit prosthesis at the gingival margin, black spaces, and food retention at the pontics. Notably, tooth number 2.6 was absent, and she exhibited a left crossbite. Her chief complaint was the compromised esthetics of her restorations.

Short partial doffs of release/relock sockets may effectively stabilize limb fluid volume in prosthesis users with transtibial amputation.

BACKGROUND: A challenge often faced by people with lower extremity amputation is management of prosthetic socket fit due to changes in fluid volume within their residual limb. Prior research suggests that intermittently doffing the prosthetic socket may help stabilize daily residual limb fluid volume. METHODS: To assess the effects of partial doff duration on residual limb fluid volume retention, participants with transtibial amputation were tested by walking on a treadmill in a controlled, laboratory setting under three conditions. An automated system to release the locking pin and enlarge t

Transfemoral interface considerations: A clinical consensus practice guideline.

BACKGROUND: Although there have been a number of studies exploring the impact and efficacy of transfemoral prosthetic components such as knee and foot mechanisms, the empirical evidence surrounding transfemoral prosthetic interface considerations is limited. This constitutes a substantial void for practicing clinicians seeking to apply best practices for patients who use transfemoral prostheses. Recent years have seen increased production and availability of clinical practice guidelines germane to prosthetic rehabilitation. In those areas where empirical evidence is lacking, consensus clinical
