The intelligibility of sentences in quiet and in noise in aged listeners.

The monaural speech-reception threshold (SRT) for sentences was investigated in quiet and under four noise conditions for 80 male subjects (age 60-90) and 30 female subjects (age 71-89). The noise levels used were 28, 43, 58, and 73 dBA. The noise had the long-term average spectrum of speech. It is shown that a model developed by Plomp [J. Acoust. Soc. Am. 63, 533-549 (1978)], which interprets any hearing loss for speech (SHL) as a combination of a loss of class A (attenuation of both speech and noise) and a loss of class D (distortion of the sound signals), enables one to make an accurate des

Generalization of perceptual adaptation to second-language speech in a single-talker training paradigm: Predictors remain elusive.

First-language (L1) listeners typically show talker general adaptation to second-language (L2) speech after exposure to multiple L2 talkers. However, cross-talker generalization following single L2 talker exposure is inconsistent. We tested two hypotheses regarding generalization following single-talker training. If intelligibility drives generalization, then a high-intelligibility training talker should facilitate cross-talker generalization. However, if similarity drives generalization, then test performance should vary depending on acoustic similarity between training and test talkers. Twen

Audiovisual perception of noise vocoded speech in dyslexic and non-dyslexic adults: the role of low-frequency visual modulations.

Visual speech inputs can enhance auditory speech information, particularly in noisy or degraded conditions. The natural statistics of audiovisual speech highlight the temporal correspondence between visual and auditory prosody, with lip, jaw, cheek and head movements conveying information about the speech envelope. Low-frequency spatial and temporal modulations in the 2-7 Hz range are of particular importance. Dyslexic individuals have specific problems in perceiving speech envelope cues. In the current study, we used an audiovisual noise-vocoded speech task to investigate the contribution of

The Use of the Voice Trainer App for Vocal Control in People with a Degenerative Ataxia: A Pilot Intervention Study.

Dysarthria is disabling in persons with degenerative ataxia. There is limited evidence for speech therapy interventions. In this pilot study, we used the Voice trainer app, which was originally developed for patients with Parkinson's disease, as a feedback tool for vocal control. We hypothesized that patients with ataxic dysarthria would benefit from the Voice trainer app to better control their loudness and pitch, resulting in a lower speaking rate and better intelligibility. This intervention study consisted of five therapy sessions of 30 min within 3 weeks using the principles of the Pitch

Adaptation of the HINT (hearing in noise test) for adult Canadian Francophone populations.

The HINT provides an efficient and reliable method of assessing speech intelligibility in quiet and in noise by using an adaptive strategy to measure speech reception thresholds for sentences, thus avoiding ceiling and floor effects that plague traditional measures performed at fixed presentation levels A strong need for such a test within the Canadian Francophone population, led us to develop a French version of the HINT. Here we describe the development of this test. The Canadian French version is composed of 240-recorded sentences, equated for intelligibility, and cast into 12 phonemically …

[Phoniatric aspects of reconstructive laryngectomy].

Reconstructive laryngectomy has been performed at the E.N.T. Clinic in Ferrara for some time. After surgery the patients undergo speech therapy and phoniatric treatment for a period of time varying from 2 to 6 months. In order to better evaluate the vocal quality obtained after such reeducation the vocal emissions of 25 patients were examined and recorded. The recorded material, made up of prolonged vowels and 6 phonetically balanced sentences, was then evaluated by a panel of 7 "trained" listeners. The evaluation score-card proposed by Woiers in 1977 was used in taking the data. This not only

NAL-NL2 empirical adjustments.

NAL-NL1, the first procedure from the National Acoustic Laboratories (NAL) for prescribing nonlinear gain, was a purely theoretically derived formula aimed at maximizing speech intelligibility for any input level of speech while keeping the overall loudness of speech at or below normal loudness. The formula was obtained through an optimization process in which speech intelligibility and loudness were predicted from selected models. Using updated models and applying some revisions to the derivation process, a theoretically derived NAL-NL2 formula was obtained in a similar way. Further adjustmen…

A prospective study of speech and voice rehabilitation after total laryngectomy with the low-resistance Groningen, Nijdam and Provox voice prostheses.

In a prospective study the Groningen, Nijdam, and Provox voice prostheses were evaluated with respect to speech and voice rehabilitation. At approximately 1, 4, and 10 months after operation, patients were submitted to a standardized speaking task to evaluate phonatory skills (phrase length, phonation duration, dynamics on tone, dynamics on sentence, speech rate and availability of sound), speech quality (fluency and overall intelligibility), voice quality and stoma technique (stoma noise). Tracheoesophageal speech rehabilitation proved to be successful in 94-100% of patients, as measured at a

Implementing two treatment approaches to childhood dysarthria.

The paucity of evidence and detail in the literature regarding speech treatment for children with dysarthria due to cerebral palsy (CP) renders it difficult for researchers to replicate studies and make further inroads into this area in need of exploration. Furthermore, for speech-language pathologists (SLPs) wishing to follow treatments that the literature indicates have promise, little guidance is available on the details of the treatments that yielded the positive results. The present article details the implementation of two treatment approaches in speech treatment research for children wi

[Dysarthria across Parkinson's disease progression. Natural history of its components: dysphonia, dysprosody and dysarthria].

Dysarthria refers to a collective name for a group of neurologic motor speech disorders, resulting from central and/or peripheral nervous system abnormalities. Speech alteration in Parkinson's disease, so-called hypokinetic dysarthria, presents with prosodic insufficiency, related to a monotony of pitch and intensity, a reduction of accentuation, variable speech rate and possible phoneme imprecision. In most cases, voice is harsh and breathy. This symptom can affect both voice and speech quality, as well as prosody and intelligibility. As a consequence, many patients complain about speech impa

The Effect of Short Pulse Width Settings on the Therapeutic Window in Subthalamic Nucleus Deep Brain Stimulation for Parkinson's disease.

BACKGROUND: Subthalamic nucleus deep brain stimulation (STN-DBS) is an established treatment for selected Parkinson's disease (PD) patients, but therapy is often limited by side effects. Previous studies indicate an inverse relationship of the therapeutic window (TW) to pulse width (PW) settings down to 60μs, but there is limited data available on the effect of shorter PWs. OBJECTIVE: To define the TW of STN-DBS in PD at PW of 30μs (PW30) relative to standard PW settings at 60μs (PW60), and to compare speed of gait and speech intelligibility on the two PW conditions. METHODS: Monopolar revie…

Speech perception of infants with high familial risk for dyslexia differ at the age of 6 months.

As yet relatively little is known of the earliest signs of dyslexia. We present evidence showing that the speech perception of 6-month-old infants from dyslexic families differs significantly from that of infants from control families with normal reading parents; the former group needed a significantly longer duration to categorize speech sounds as long. The same difference appeared in their dyslexic parents. This study shows that differences in categorizing speech sounds according to duration, which is crucial to intelligibility in Finnish, are a factor associated with familial risk for dysle

The Relationship Between Apraxia of Speech and Oral Apraxia: Association or Dissociation?

Acquired apraxia of speech (AOS) is a motor speech disorder that affects the implementation of articulatory gestures and the fluency and intelligibility of speech. Oral apraxia (OA) is an impairment of nonspeech volitional movement. Although many speakers with AOS also display difficulties with volitional nonspeech oral movements, the relationship between the 2 conditions is unclear. This study explored the relationship between speech and volitional nonspeech oral movement impairment in a sample of 50 participants with AOS. We examined levels of association and dissociation between speech and
