Pituitary gigantism caused by growth hormone excess from infancy.

A 2 1/4-year-old boy with pituitary gigantism had a large pituitary macroadenoma. Gross tumor removal has prevented further visual losses, but he has had persistent hypersecretion of growth hormone and prolactin. Treatment with somatostatin analog has decreased both hormone secretion and growth velocity.

Association of Hirschsprung's disease and Müllerian inhibiting substance deficiency.

An infant who presented with Hirschsprung's disease was found to also have the clinical features of persistent Müllerian duct syndrome and on testing to have Müllerian inhibiting substance deficiency. This association has not been previously reported. While this may be a coincidence, recent reports indicate intersex conditions can be associated with Hirschsprung's disease.

Effects of repetitive strains on vertebral end plates in young rats.

A histologic study of vertebral end plates was performed on young rats' tails subjected to intensive passive motion. Seven six-week-old rats had two-hour motion cycles daily, for two months. Histologic changes of proximal caudal vertebrae in the experimental group were compared with those of a control group of seven rats of the same litter which were allowed to grow freely without mechanical stress. The main findings were three striking protrusions of disk tissue into end plates, not observed in the control animals. End plate axial bulges lacking spindle-shaped cells and an increase in chondro

Prior exposure to copper, but not iron, decreases the severity of copper deficiency in growing rats.

The development of Cu deficiency after prior exposure for 8 days to normal and high Cu and to normal and high Fe was examined in weanling male Sprague-Dawley rats. After 28 days of Cu depletion, those with high rather than normal initial Cu stores had less elevation in heart and liver weight; higher hemoglobin and hematocrit (by 17%), Cu,Zn-superoxide dismutase activity in liver (44%), cytochrome c oxidase activity in liver (66%) and heart (19%), and Cu concentration in liver (76%) and heart (37%). In contrast, high initial stores of Fe did not alter Cu status during Cu deprivation. Thus, prio

[Molecular biology of normal and pathologic anti-müllerian hormone].

Anti-Müllerian hormone, responsible for Müllerian regression in male fetuses, is a glycoprotein dimer with two 72 kD subunits. The AMH gene is a small (2,800 bp) gene with 5 exons, localized on the tip of the short arm of chromosome 19, band p 133, and transcribed in a 2,000 kbp mRNA. Persistent Müllerian duct syndrome, a rare form of male pseudohermaphroditism characterized by the presence of uterus and Fallopian tubes in patients with normally virilized genitalia, may result from defective AMH gene or from target-organ insensitivity. Four mutations were identified in the AMH gene, 3 are poin

Simpson-Golabi-Behmel syndrome with severe cardiac arrhythmias.

We report on a family with 2 affected males with the X-linked Simpson-Golabi-Behmel (SGB) syndrome. The propositus was a 33-year-old man with pre- and postnatal overgrowth, "coarse" face with hypertelorism, broad nose, wide mouth, malposition of teeth, submucous cleft, accessory nipples, broad hands with hypoplastic index finger nails, and operated left postaxial hexadactyly. From the age of 26 years he suffered from severe tachyarrhythmias, requiring recurrent defibrillations. The brother of the propositus was macrosomic at birth and had a similar facial appearance. In addition he had a pylor

Perspectives on the pathophysiology of fetal alcohol syndrome.

Fetal alcohol syndrome (FAS) is a major known cause of fetal malformations and mental retardation. Prevention/intervention of FAS can only be achieved with identification of the mechanisms by which alcohol induces birth defects. The purpose of this paper is to discuss the data on possible mechanisms of FAS, and to give a number of suggestions for future research areas.

Precocious or early puberty and growth failure in girls treated for acute lymphoblastic leukaemia.

We have studied 41 children with early or precocious puberty who have been treated for acute lymphoblastic leukaemia with prophylactic cranial irradiation (1,800-2,400 cGy) accompanied by intrathecal methotrexate and systemic chemotherapy. Mean age at radiotherapy was 3.9 years (range 1.7-7.7) in the girls and 4.8 years (range 2.6-7.8) in the boys. Mean age at the onset of puberty was 8.6 years (range 6.7-9.7) in the girls and 9.3 years (range 7.8-10.3) in the boys. Of the 41 children with early puberty (greater than 1.4 SD from the mean) 36 were females and 5 were males. 21 of the 36 girls ha

A child with sclerocornea, short limbs, short stature, and distinct facial appearance.

We describe a boy with sclerocornea, short limbs, short stature and a distinct facial appearance. The resemblance to other reports of affected sibs suggests that this is a newly recognised, autosomal recessive syndrome.

Combined congenital deficiencies of intrinsic factor and R binder.

Coexisting deficiencies of both intrinsic factor (IF) and R binder were identified in an Algerian boy who presented with severe megaloblastic anemia, growth retardation, and neurologic dysfunction with typical features of subacute combined degeneration of the spinal cord. The anemia responded completely to cyanocobalamin and folic acid. IF was absent from gastric juice, but acid secretion and gastric mucosa were normal. R binders were absent from gastric juices as well as from serum, saliva, and polymorphonuclear leukocytes. The patient's father exhibited absence of R binder in his serum with

Psychosocial correlates of short stature and delayed puberty.

Studies relating to the psychosocial aspects of short stature and pubertal delay are reviewed. Although IQ scores show a statistical correlation with stature, significant intellectual, psychological, or academic deficits have not been consistently demonstrated for short children. However, hypopituitary growth failure is associated with poor social adjustment as an adult despite growth hormone therapy. The reasons for this remain unclear. Constitutional pubertal delay in boys can lead to social and academic problems. Treatment with a short course of testosterone can be beneficial in selected ca

Anterior and posterior displacement of the hip after innominate osteotomy.

Ten patients were treated for anterior or posterior displacement of the hip after an innominate osteotomy for congenital dislocation of the hip. All required a repeated open reduction with an additional procedure, either at the same time as reduction or as a second stage. Stable reduction was achieved in eight cases, but in two the initial attempt failed and the operation had to be repeated. The clinical features of hips with this complication are described and the technique of the salvage operation is discussed.

Calcifying discopathy in infancy in the cervical spine: evaluation of vertebral alterations over a period of time.

Calcifying discopathy in infancy involving in the cervical spine has already been observed and described by many authors, as a well-defined clinico-radiological syndrome with a benign course. The clinical picture is composed of: pain and functional limitation, sometimes with a stiff neck, more rarely slight fever, increase of the erythrocyte sedimentation rate or leukocytosis. The radiographic picture consists of the association of morphological and structural alterations of vertebral bodies adjacent to one or more disc calcifications usually centrally sited, sometimes associated with anterior

The early identification and classification of growth disturbances of the proximal end of the femur.

An analysis of the serial radiographs of sixty-eight patients who were treated for congenital dislocation of the hip revealed that a growth disturbance of the proximal end of the femur that was caused by partial or complete physeal closure developed in thirty-three of these patients. The disturbances were related to the character of the metaphyseal growth-disturbance lines and were subsequently classified according to the site and extent of physeal closure. In order to study this problem, we divided the physis of the proximal end of the femur into two contiguous sections: a medial and a latera
