Lethal immunotoxicity in high-dose systemic AAV therapy.

High-dose systemic gene therapy with adeno-associated virus (AAV) is in clinical trials to treat various inherited diseases. Despite remarkable success in spinal muscular atrophy and promising results in other diseases, fatality has been observed due to liver, kidney, heart, or lung failure. Innate and adaptive immune responses to the vector play a critical role in the toxicity. Host factors also contribute to patient death. This mini-review summarizes clinical findings and calls for concerted efforts from all stakeholders to better understand the mechanisms underlying lethality in AAV gene th…

Clinical relevance of terminal Schwann cells: An overlooked component of the neuromuscular junction.

The terminal Schwann cell (tSC), a type of nonmyelinating Schwann cell, is a significant yet relatively understudied component of the neuromuscular junction. In addition to reviewing the role tSCs play on formation, maintenance, and remodeling of the synapse, we review studies that implicate tSCs in neuromuscular diseases including spinal muscular atrophy, Miller-Fisher syndrome, and amyotrophic lateral sclerosis, among others. We also discuss the importance of these cells on degeneration and regeneration after nerve injury. Knowledge of tSC biology may improve our understanding of disease pat…

Diagnosis and classification of sporadic inclusion body myositis (sIBM).

Sporadic inclusion body myositis (sIBM) is the most common acquired muscle disease in elderly individuals, particularly men. Its prevalence varies among ethnic groups but is estimated at 35 per one million people over 50. Genetic as well as environmental factors and autoimmune processes might both have a role in its pathogenesis. Unlike other inflammatory myopathies, sIBM causes very slowly progressive muscular weakness and atrophy, having a distinctive pattern of muscle involvement and different forms of clinical presentation. In some cases a primary autoimmune disease coexists. Diagnosis is

Genetic axonal neuropathies and neuronopathies of pre-natal and infantile onset.

The infantile-onset axonal neuropathies and neuronopathies are an uncommon and heterogeneous group of conditions causing weakness, wasting, and developmental delay in early childhood. Many are associated with central nervous system or other systemic manifestations and cause early mortality. We review the axonal Charcot-Marie-Tooth subtypes with onset in infancy, spinal muscular atrophy, and related syndromes of early infancy, giant axonal neuropathy, infantile neuroaxonal dystrophy, hereditary motor and sensory neuropathy with agenesis of the corpus callosum, early-onset neuropathies associate…

Megalencephaly, mega corpus callosum, and complete lack of motor development: delineation of a rare syndrome.

Unlike atrophy of the corpus callosum (CC), callosal hypertrophy is a rare neuroimaging finding with only few reported patients. The "megalencephaly, mega CC, and complete lack of motor development" syndrome is morphologically characterized by generalized megalencephaly, a thickened CC, and extensive polymicrogyria causing a pachygyric appearance. We report on the fifth patient showing this rare syndrome, a 3-year-old girl displaying the typical neuroimaging features. Clinically she showed a severely impaired motor, mental, and speech development with marked muscular hypotonia but no dysmorphi

Asymmetrical late onset motor neuropathy associated with a novel mutation in the small heat shock protein HSPB1 (HSP27).

Distal hereditary motor neuropathy, also known as distal spinal muscular atrophy, is characterised by slowly progressive weakness and wasting of the hands and feet and has a heterogeneous genetic basis. One form of distal hereditary motor neuropathy is associated with mutations in the gene for the small heat shock protein HSPB1 (hsp27). Families have been described in which slowly progressive, symmetrical, lower limb predominant motor weakness is usually evident by middle age. Here we report a novel mutation, G84R, in an elderly patient presenting with strikingly asymmetrical weakness. Express…

[Kennedy's disease--case report].

Kennedy's disease or spinal and bulbar muscular atrophy (SBMA) is a rare neurological illness inherited recessively, linked with chromosome X. The first symptoms appear in the adult age and result from degeneration of both motor and sensory neurons. The disease begins differently, thus causing diagnostic mistakes. In our case the symptoms suggested discopathy, therefore the patient was operated on L5-S1 discopathy, which did not improve lower limb muscle strength. This entailed further diagnostic work-up. The EMG (needle) examination showed widespread chronic denervation with reinnervation in …

Overexpressed human survival motor neurone isoforms, SMNDeltaexon7 and SMN+exon7, both form intranuclear gems but differ in cytoplasmic distribution.

Homozygous mutations of the telomeric survival motor neurone gene (SMN1) cause spinal muscular atrophy (SMA). The centromeric copy gene (SMN2) generally skips exon 7 during splicing and fails to compensate for SMN1 deficits, so SMA cells have reduced SMN protein and few nuclear gems. To investigate the role of exon 7 in SMN localisation, cDNAs for full-length SMN and SMNDeltaexon 7 were overexpressed in COS cells, neurones and SMA fibroblasts. Both constructs formed discrete intranuclear bodies colocalising with p80-coilin, but produced more cytoplasmic aggregates in cells overexpressing exon …

Cervical flexion myelopathy associated with cervical spina bifida occulta--case report.

A 16-year-old boy presented with segmental muscular atrophy of the bilateral distal upper extremities. Cervical spine x-ray films showed occult spina bifida from C-1 to T-1 associated with an abnormal long club-like bone located parallel to the epidural space between C-5 and C-7. In neck flexion, the cervical spinal cord was stretched and compressed to the posterior aspect of the vertebral body. Moreover, the dorsally placed abnormal bone migrated ventrally, indenting the dorsal portion of the spinal cord. This is quite an unusual case of so-called "flexion myelopathy," aggravated by the abnor…

[A study of superoxide dismutase in erythrocytes in Duchenne muscular dystrophy].

In orders to search for possible events in the pathogenesis of Duchenne muscular dystrophy (DMD), the activity of erythrocyte superoxide dismutase has been determined in thirty-seven patients and fifteen carriers of DMD, comparison to fourty-five age-matched normal controls, seventeen cases of infantile, Kugelberg-Welander type spinal muscle atrophy. The results showed that superoxide dismutase activity were elevated especially in early periods of disease in DMD but not in other groups of patients. These findings suggest that alterations of SOD do not constitute the primary mechanism of induci

[Electromyographic study of 50 cases of Werdnig-Hoffmann disease].

An electromyographic study was performed in 50 cases of severe infantile spinal muscular atrophy (type I). 164 muscles and 93 nerves (ulnar and posterior tibialis) were tested. The thresholds of stimulation of the motor nerves were increased. Motor action potential was absent in 30% of the investigated nerves, motor nerve conduction velocity was slow in 44%, H reflex was absent in 86%. Proprioceptive nerve conduction velocity was slow in some cases. Using the long-time (50-100 msec) direct longitudinal technique of muscle stimulation (provoked fibrillation), denervation could be proved in 98% …

Challenges and Advances in Gene Therapy Approaches for Neurodegenerative Disorders.

INTRODUCTION: The recent approval of Spinraza (nusinersen), an antisense oligonucleotide, by U.S. Food and Drug Administration to treat patients with spinal muscular atrophy, has reignited interests of researchers in designing and testing new gene therapy approaches to treat neurological disorders, in particular, to curb neurodegenerative diseases of the central nervous system which represent an ever-increasing public health burden to today's society. CONCLUSION: This review highlights several key factors to be taken into consideration to design successful preclinical and clinical gene therap…

Noninvasive airway approaches for acute neuromuscular respiratory failure in emergency departments.

Emerging evidence advocates for noninvasive ventilation (NIV) combined with mechanical in-exsufflation (MIE) as a first-line approach for acute respiratory failure (ARF) in patients with neuromuscular disorders (NMD). To date, most NIV studies of ARF in NMD patients have been performed in intensive care units or in hospital settings. However, the utility of using combined NIV/MIE in the emergency department (ED) settings is unclear. We report on the implementation of NIV/MIE in two children with type II spinal muscular atrophy who presented to the ED with ARF. This is the first report on the f…

[Post-polio syndrome. Part I. The "legacy" of forgotten disease, challenges for professionals and polio survivors].

The outcome of paralytic polio was believed to be a stable neurological state. Now, it is established that polio has an additional, slowly progressive phase, called post-polio syndrome (PPS) that develops 30-40 years after the acute poliomyelitis in 25-80% of paralytic and about 40% of nonparalytic polio survivors. The clinical symptoms are nonspecific and usually include muscle weakness, fatigue and muscle or joint pain. Some patients suffer from muscular atrophy, respiratory insufficiency, dysphagia, sleep disturbances or cold intolerance. The etiopathogenesis of PPS is unclear and many fact…

Anatomic features of the paramedian muscle-splitting approaches to the lumbar spine.

BACKGROUND: Intermuscular approaches can expose the lumbar spine and minimize muscular trauma and injury. The segmental anatomy of the posterior lumbar musculature allows surgical access through separation of muscle groups and fascicles and provides one to develop intermuscular working channels while preserving the integrity of the muscles and their function. In addition, preservation of the accompanying neurovascular bundles minimizes blood loss, tissue atrophy, and pain. With these approaches, a variety of procedures for decompression, discectomy, interbody fusion, or pedicle screw fixation

[When death is an act of care: refusing life support for children].

This paper analyzes a court case involving parents' refusal of life support measures for an 8-month-old infant with spinal muscular atrophy type I, a fatal degenerative genetic disease. The parents filed for a court injunction to ensure that the infant would not be submitted to mechanical ventilation in case of respiratory distress. The Brazilian courts recognized the parents' right to refuse life support measures, and the infant died a week after the ruling. The parents' request was to guarantee their right to avoid medical procedures that would not alter the infant's clinical prognosis. The …

Unrip is a component of SMN complexes active in snRNP assembly.

A macromolecular complex containing survival of motor neurons (SMN), the spinal muscular atrophy protein, and Gemin2-7 interacts with Sm proteins and snRNAs to carry out the assembly of these components into spliceosomal small nuclear ribonucleoproteins (snRNPs). Here we report the characterization of unr-interacting protein (unrip), a GH-WD protein of unknown function, as a component of the SMN complex that interacts directly with Gemin6 and Gemin7. Unrip also binds a subset of Sm proteins, and unrip-containing SMN complexes are necessary and sufficient to mediate the assembly of spliceosomal…

Skeletal Muscle Mitochondria Dysfunction in Genetic Neuromuscular Disorders with Cardiac Phenotype.

Mitochondrial dysfunction is considered the major contributor to skeletal muscle wasting in different conditions. Genetically determined neuromuscular disorders occur as a result of mutations in the structural proteins of striated muscle cells and therefore are often combined with cardiac phenotype, which most often manifests as a cardiomyopathy. The specific roles played by mitochondria and mitochondrial energetic metabolism in skeletal muscle under muscle-wasting conditions in cardiomyopathies have not yet been investigated in detail, and this aspect of genetic muscle diseases remains poorly
