INTRODUCTION: Brachial plexus injury is a rare but serious complication of thoracic surgery performed in the lateral position. Improper positioning of the upper limb or neck can increase the risk of nerve damage. Risk factors include excessive abduction of the upper limb and hyperextension of the neck. Preventive measures-such as preoperative simulation of surgical positioning, regular intraoperative reassessment of limb and neck alignment, and close collaboration among surgeons, anesthesiologists, and nursing staff-are critical for ensuring patient safety.

Mangled limb injuries, typically caused by high-energy trauma, present significant challenges in surgical decision-making, particularly when considering limb salvage versus primary amputation. While common mechanisms include vehicular or industrial accidents, unconventional etiologies are rarely documented. We report a unique case of a 43-year-old female who sustained a mangled left lower limb injury following a gearbox explosion in a Porsche Cayenne. The injury resulted in a near-complete amputation at the distal third of the leg, with extensive contamination by oily and metallic debris. Radi

The distal fibula plays a key role in ankle stability by forming the ankle mortise and serving as the attachment site for the lateral ligament complex. Traumatic loss of the distal fibula is extremely rare and is associated with ankle instability, valgus deformity, and eventual arthritis if not addressed. We report the case of a 34-year-old female who sustained a high-energy open injury with complete loss of the distal fibula, disruption of the lateral ligament complex, and a comminuted distal tibial fracture following a road traffic accident. Initial management included emergency debridement,

Distal leg and ankle defects with exposed bone are difficult to reconstruct, particularly after contaminated open injuries. Free tissue transfer is unavailable in many resource-limited hospitals, making reliable pedicled flaps essential. We report three adult men with Gustilo-Anderson type IIIB distal leg or ankle injuries treated with a distally based peroneus brevis muscle flap at a Sudanese teaching hospital. In the first patient, a 10×5 cm distal tibial defect after motorcycle trauma was covered immediately after debridement and external fixation, followed by skin grafting on postoperative

INTRODUCTION: Traumatic hemipelvectomy is a rare operation, often performed for injuries caused by a high force mechanism such as a traffic or industrial accident, and often fatal. Most often performed emergently, here we report a traumatic hemipelvectomy performed on a pediatric patient one month from initial injury.
CASE PRESENTATION: A 7-year-old healthy male was crushed between two motor vehicles while riding his bicycle. He was taken emergently to the nearest trauma center and underwent external fixation of his pelvis, exploratory laparotomy, abdominal packing, diverting colostomy, vesic

INTRODUCTION: Alzheimer's disease (AD) is a progressive neurodegenerative disease and the leading cause of dementia. Recent research highlights meningeal lymphatics as key regulators in neurological diseases, suggesting that enhancing their drainage function could be a potential therapeutic strategy for AD. Our proof-of-concept study demonstrated that cranial bone transport can improve meningeal lymphatic drainage function and promote ischemic stroke recovery.
METHODS: This study defined cranial bone maneuver (CBM) technique. After osteotomy, a small circular bone flap was made and attached t

BACKGROUND: This study aimed to investigate the anatomical effect of radial rotational deformity and reduced radial bow on the forearm range of motion (ROM) to improve outcomes in severe forearm diaphyseal fractures and malunion.
METHODS: Twenty upper limbs of 10 fresh-frozen cadavers were amputated at the humerus and distally at the carpometacarpal joint and examined. Soft tissues, except the interosseous membrane, articular capsule, and pronator quadratus muscle, were removed. The deformity was modeled by cutting at the maximal bow of the radial diaphysis and fixing it with an external fixa

INTRODUCTION: Pressure ulcers are common complications in immobilized patients, typically occurring over bony prominences. However, in rare instances, particularly in high-risk individuals with hematologic disorders such as sickle cell disease, ulcers may develop at unusual anatomical sites due to external medical devices. This case highlights a rare presentation of a vulvar pressure ulcer caused by prolonged application of a Thomas splint in a patient with homozygous sickle cell disease (HbSS).
CASE REPORT: A 24-year-old woman with HbSS was admitted with a closed distal femoral shaft fractur

BACKGROUND: Multilevel fractures are breaks of a single bone at multiple levels, often as a combination of diaphyseal and metaphyseal sections. They are usually caused by the effect of strong direct force. Combined injuries of a bone are fractures accompanied by joint distortion. This results in ligament ruptures or avulsions together with a shaft fracture. Surgical treatment is always challenging and requires a differentiated approach.
OBJECTIVE: The aim of this review article is to highlight the diagnostic and surgical procedures for multilevel fractures. Particular attention is paid to the

The management of adult spinal deformity (ASD) has evolved dramatically over the past century, transitioning from external bracing and in situ fusion to complex, technology-driven surgical interventions. This review traces the historical development of spinal deformity correction and highlights contemporary enabling technologies that are redefining the surgical landscape. Advances in stereoradiographic imaging now allow for precise, low-dose three-dimensional assessment of spinopelvic parameters and segmental bone density, facilitating individualized surgical planning. INTRODUCTION: Hypophosphatemic rickets is a rare X-linked genetic disorder clinically characterized by bone deformities, especially localized in the lower limbs, such as varus and valgus knees. Diagnosis is based on clinical observation and biochemical parameters. Treatment is pharmacological, however, if the skeletal deformities are severe, it is necessary a surgical correction.
PRESENTATION OF CASES: We report two patients with hypophosphatemic rickets and severe varus knee. Both patients were surgically treated by bilateral femoral and tibial osteotomy, and stabilized with a uniplanar exte

HYPOTHESIS: In bone conduction implantation, the position of the implant influences the audiological benefit of the patient.
BACKGROUND: One way of treating hearing loss is the implantation of bone conduction implants (BCIs), which effectively transmit vibrations through the skull bone to the cochlea given that the implant transducer is securely fixated. Laboratory research on the efficacy of bone conduction sound transmission found that a closer proximity of the transducer to the ipsilateral cochlea yields significantly higher cochlear promontory vibrations and hence, higher stimulation effi

BACKGROUND: Direct skeletal fixation, a surgical technique enabling the attachment of an external prosthesis directly to the bone through a percutaneous implant, offers an enticing solution for patients with lower limb amputations facing socket-related issues. However, understanding of its impact on musculoskeletal function remains limited.
METHODS: This study compares pre- and 1-year post-osseointegration surgery outcomes, focusing on patient-reported measures and musculoskeletal system function during level-ground walking. Two participants with unilateral transfemoral amputations and two pa

In the field of orthopedics, surgeons have long been facing the challenge of loosening of external fixation screws due to inherent material characteristics. Despite Polyetheretherketone (PEEK) being employed as an orthopedic implant material for many years, its bio-inert nature often hinders bone healing due to the limited bioactivity, which restricts its clinical applications. Herein, a new type of orthopedic implant (Sr-SPK) was developed by introducing strontium (Sr)-doped mesoporous bioactive glass (Sr-MBG) onto the surface of PEEK implants through a simple and feasible method. In vitro ex

Total hand degloving injuries present a significant challenge in treatment, with no consensus yet on the optimal choice of flap for coverage. Functional reconstruction necessitates careful consideration of syndactyly release, particularly after flap coverage. A 64-year-old woman sustained a total hand degloving injury and underwent extensive reconstructive surgery. Initially, a free superficial circumflex iliac perforator flap was applied to the thumb, whereas pedicled groin and superficial inferior epigastric artery flaps were used for the remaining fingers. However, partial necrosis at the e

Traditional temporary cardiac pacemakers (TCPs), which employ transcutaneous leads and external wired power systems are battery-dependent and generally non-absorbable with rigidity, thereby necessitating surgical retrieval after therapy and resulting in potentially severe complications. Wireless and bioresorbable transient pacemakers have, hence, emerged recently, though hitting a bottleneck of unfavorable tissue-device bonding interface subject to mismatched mechanical modulus, low adhesive strength, inferior electrical performances, and infection risks. Here, to address such crux, we develop

A 17-month-old domestic short-hair cat was referred due to a non-union in the left tibia. The initial repair, conducted 3 months prior at another animal hospital, involved an intramedullary (IM) pin and wire to address a comminuted fracture. Unfortunately, the wire knot caused a skin tract, resulting in osteomyelitis. Although the wire knot was removed at that hospital, the draining tract persisted, continuously discharging exudate. Upon evaluation, the first surgery was reassessed and revised, involving the removal of the IM pin and the application of external skeletal fixation alongside an a

Osteomyelitis is commonly caused by pathogens like Staphylococcus aureus, but rare organisms such as Corynebacterium minutissimum, typically associated with superficial skin infections, can also be implicated. Recognizing these atypical pathogens presents diagnostic and therapeutic challenges, especially in the presence of orthopedic hardware. We conducted a literature review yielding 25 studies and encompassing 797 patient cases, which highlights the emerging role of Corynebacterium species in osteomyelitis, particularly following trauma or surgical interventions.
