Compartment Syndrome of the Arm After Cable-Wakeboard Accident.

A compartment syndrome is an increased tissue pressure within a closed osteofascial compartment. This compromises blood flow to the muscles and nerves within that compartment, which -if not treated adequately in an early stage-results in permanent tissue and nerve damage. It most frequently occurs in the lower leg, but can also occur elsewhere when muscles are enclosed in tight fascial compartments, such as the forearm and hand. In this report a patient is described who developed an acute compartment syndrome of the arm after a cable-wakeboard accident in which his arm was strangulated. Cable-

Relationship between gait biomechanics and inversion sprains: a prospective study of risk factors.

This prospective study determined gait related risk factors for inversion sprains in 223 physical education students. Static lower leg alignment was determined, and 3D-kinematics combined with plantar pressure profiles were collected. After evaluation, the same sports physician registered all sports injuries during the next 6-18 months. During this period, 21 subjects had an inversion sprain, one of whom had a bilateral sprain. Twenty-two ankles, 12 left and 10 right comprised the inversion sprain group and both feet of 36 non-injured subjects acted as controls. Comparison of the two groups re

Case report: the clinical toxicity of dimethylamine borane.

CONTEXT: Dimethylamine borane (DMAB) is a reducing agent used in nonelectric plating of semiconductors. Exposures are usually through occupational contact. We report here four cases of people who suffered from work-related exposure to DMAB. CASE PRESENTATION: Three patients exposed to DMAB decontaminated immediately by drinking a lot of water; they reported dizziness, nausea, diarrhea 6-8 hr later. The other patient did not decontaminate at once, and he suffered from more severe symptoms, including dizziness, nausea, limb numbness, slurred speech, irritable mood, and ataxia 13 hr later. Magne

The anatomy of the posterolateral aspect of the rabbit knee.

The purpose of this study was to determine the anatomy of the posterolateral aspect of the rabbit knee to serve as a basis for future in vitro and in vivo posterolateral knee biomechanical and injury studies. Twelve nonpaired fresh-frozen New Zealand white rabbit knees were dissected to determine the anatomy of the posterolateral corner. The following main structures were consistently identified in the rabbit posterolateral knee: the gastrocnemius muscles, biceps femoris muscle, popliteus muscle and tendon, fibular collateral ligament, posterior capsule, ligament of Wrisberg, and posterior men

Flow-through use of the osteomusculocutaneous free fibular flap.

A case of a forearm defect resulting from a high-velocity firearm injury was reconstructed by flow-through free transfer of the osteomusculocutaneous fibular flap harvested from the remaining stump of the patient's left leg that was amputated below the knee. The dimensions of the bone defect and damage to anatomic structures of more than one type (including ulnar artery, ulnar bone, and overlying soft tissue) were the reasons for the treatment approach with a composite tissue transfer including vascularized bone. Preoperative radiographic and angiographic examination revealed that the amputati

Improved body composition after 8 wk of electrically stimulated leg cycling in tetraplegic patients.

The practical aspects of utilizing electrically stimulated leg cycling (ESLC) to counteract alterations in body composition were investigated in five tetraplegic subjects with long-standing complete spinal cord injuries (C5-C7). After a 2-wk adaptation period, the subjects performed seven ESLC sessions per week for 8 wk. No adverse reactions were noted in response to the ESLC program. The ESLC sessions were accompanied by higher lactate concentrations compared with arm exercise. Heart rate and blood pressure response revealed clear, but not serious, signs of autonomic dysreflexia in the beginn

Patellofemoral problems after intraarticular anterior cruciate ligament reconstruction.

A series of 226 anterior cruciate ligament (ACL) reconstructions were reviewed to determine the incidence of patellofemoral (PF) problems and the associated prognostic factors. Patients were divided into four groups according to the type of injury (acute or chronic) and operation (through an arthrotomy or arthroscopic assisted). The average follow-up period was 39 months. Overall there was a 5% incidence of PF crepitus with pain and/or swelling, and a further 20% of clear PF crepitus without pain. The change from open surgery and cast to arthroscopic surgery and early motion allowed a decrease

Biomechanical measures during landing and postural stability predict second anterior cruciate ligament injury after anterior cruciate ligament reconstruction and return to sport.

BACKGROUND: Athletes who return to sport participation after anterior cruciate ligament reconstruction (ACLR) have a higher risk of a second anterior cruciate ligament injury (either reinjury or contralateral injury) compared with non-anterior cruciate ligament-injured athletes. HYPOTHESES: Prospective measures of neuromuscular control and postural stability after ACLR will predict relative increased risk for a second anterior cruciate ligament injury. STUDY DESIGN: Cohort study (prognosis); Level of evidence, 2. METHODS: Fifty-six athletes underwent a prospective biomechanical screening af

Walking Stability and Kinematic Variability Following Motor Fatigue Induced by Incline Treadmill Walking.

Detecting motor fatigue during rigorous activities is essential for preventing injuries, falls, and over-exertion. While research has focused on developing fatigue indices using motion capture or wearable sensors, the method of inducing fatigue can impact movement patterns differently. This study compared the effects of whole-body motor fatigue induced by incline treadmill walking with localized fatigue induced by leg presses and isolated ankle movements, as investigated in our prior study. Twenty healthy young participants walked at 1.25 m/s for 5 min before (PRE) and after (POST) motor fatig

Ultrasound elastography to guide compression therapy for gastrocnemius trigger points in runners.

Myofascial pain syndrome (MPS) is one of the most common musculoskeletal pain disorders affecting athletes. This is an important clinical issue because MPS has a negative impact on athlete achievement as with sporting injuries. We screened 44 amateur runners who ran ≥ 50 km/week and reported running-related lower-leg pain. Thirty runners with gastrocnemius myofascial trigger points (MTrPs; 21 men, 9 women; mean ages 36 and 33 years) underwent B-mode ultrasonography and strain elastography. Ischemic compression was applied to identified MTrPs, with sonographic reassessment after each cycle and

Ultrasound Imaging of Ankle Retinacula: A Comprehensive Review.

The retinacula of the ankle are specialized anatomical structures characterized by localized thickenings of the crural fascia that envelop the deep components of the lower leg, ankle and foot. The ankle retinacula include the extensor retinacula, the peroneal retinacula and flexor retinaculum. Despite their potential to explain persistent and unexplained pain following an injury, these structures are often overlooked or incorrectly diagnosed. Hence, this comprehensive review was performed aiming to investigate the use and the methodology of US imaging to assess ankle retinacula. The search was

The Impact of Differential Knee Laxity on Brain Activation During Passive Knee Joint Loading.

Although higher anterior knee laxity is an established risk factor of ACL injury, underlying mechanisms are uncertain. While decreased proprioception and altered movement patterns in individuals with anterior knee laxity have been identified, the potential impact of higher laxity on brain activity is not well understood. Thus, the purpose of this study is to identify the impact of different magnitudes of knee laxity on brain function during anterior knee joint loading. Twenty-seven healthy and active female college students without any previous severe lower leg injuries volunteered for this st…

Four-day delay "metachronous" polymicrobial necrotizing fasciitis of lower limbs: a unique case report.

Necrotizing fasciitis is a rare soft tissue infection that requires immediate medical attention to prevent its fulminant development that can lead to amputation or death of the patient. In most of reported cases of multifocal necrotizing fasciitis, injuries appear synchronously within hours from the initial diagnosis. It is the only third reported case with metachronous lesions, and the first that involves both S. pyogenes and S. aureus. Early diagnosis and multidisciplinary treatment is mandatory to prevent fatal outcomes. We present the case of a 58-year-old Caucasian man who developed necro

External Focus Reduces Accuracy and Increases Antagonist Muscle Activation in Novice Adolescent Soccer Players.

Instep kick is one of the most effective kicking techniques in soccer. Lower extremity muscles and joints play a crucial role during instep kick. However, external (EF) and internal focus and their effect on the muscles are still ambiguous. In this study, 13 male adolescent soccer players were included and aimed to hit the targets in internal and EF conditions. Lower extremity muscle activations were measured with surface electromyography, and kinematics were measured with a high-speed video camera. Muscle activations and movement latencies were analyzed in four different phases (backswing, le

Management of a Complex Lower Limb Open Fracture in a Teenage Patient: A Case Report.

The challenges of managing Gustilo IIIB tibial fractures (ie, high energy trauma with a contaminated wound >10 cm in length, severe comminution ["crumbling"] or segmental fractures, and periosteal stripping) in children are unique in part because no clear guidelines exist and the injuries may cause short-term and long-term complications. Repeated wound debridement and secondary reconstruction are required in approximately 20% of these cases in both adults and children. A 13-year-old girl presented with severe polytrauma including an open Gustilo type IIIB fracture of the left lower leg. The pa
