Non-Contact, Mechanical Fatigue-Related ACL Injury Prevention Through Extracellular Matrix Crosslink Preservation: A Narrative Review.

Background: Anterior cruciate ligament (ACL) injuries are increasing in young athletes and many are related to non-contact, spontaneous mechanical fatigue-related ruptures. The objective of this narrative review is to identify and synthesize the anatomical, histological, physiological, and biomechanical basis of extracellular matrix (ECM) factors that contribute to ACL injuries and suggest ways to decrease their occurrence. Methods: The primary investigator searched PubMed, Web of Science, and Google Scholar database titles and abstracts using search phrases with Boolean operators: "anterior c

Post-operative nerve injuries after cervical spine surgery.

Although relatively rare, post-operative nerve injuries may occur after cervical spine procedures. The most common post-operative neural disorder is C5 nerve palsy. The risk factors for C5 nerve palsy are male gender, OPLL, and posterior cervical approaches. It generally presents with deltoid and/or biceps weakness, and may present immediately or several days after surgery. Treatment is generally conservative due to transient duration of symptoms, but evaluation of residual compression at C4-5 is essential. PTS (Parsonage-Turner syndrome) is an idiopathic plexopathy generally presenting with s

Injuries associated with traumatic anterior glenohumeral dislocations.

BACKGROUND: A number of shoulder girdle injuries are associated with acute anterior glenohumeral dislocations. In the present study we evaluated the prevalence of neurological deficits, greater tuberosity fractures, and rotator cuff injuries in a population of unselected patients who presented with a traumatic anterior glenohumeral dislocation. METHODS: A prospective trauma database was used to record the demographic details on 3633 consecutive patients (2250 male patients and 1383 female patients with a mean age of 47.6 years) who had sustained a traumatic anterior glenohumeral dislocation b

Open reduction internal fixation with bone grafting for scapular body nonunion: A case report.

Scapular body fractures are rare fractures that represent less than 1 % of all fractures and are typically associated with a high energy mechanism of injury. Traditionally these fractures have been treated non-operatively, resulting in union of the fracture and acceptable patient outcomes. We present a case of symptomatic scapular body fracture nonunion following non-operative management that was treated with open reduction and internal fixation with local autologous bone grafting. Our patient went on to successful union of his fracture as well as drastic improvement in shoulder function, rang

The biomechanical demands of manual scaling on the shoulders & neck of dental hygienists.

The purpose of this study was to evaluate the postural and muscular demands placed on the shoulders and neck of dental hygienists when performing a simulated manual scaling task. Nineteen healthy female dental hygienists performed 30-min of simulated manual scaling on a manikin head in a laboratory setting. Surface electromyography was used to monitor muscle activity from several neck and shoulder muscles, and neck and arm elevation kinematics were evaluated using motion capture. The simulated scaling task resulted in a large range of neck and arm elevation angles and excessive low-level muscu

Brachial Plexus Injury Caused by Indwelling Axillary Venous Pacing Leads.

A 64-year-old male patient underwent cardiac resynchronization therapy (CRT) device implantation via the axillary venous approach. Two weeks later, the patient started complaining of "electric shock-like" pain in the left axillary area. During physical examination, typical pain in the left axillary area was reproduced whenever his left shoulder was passively abducted more than 60 degrees. Fluoroscopic examination showed that the left ventricle (LV) and right atrium (RA) leads were positioned at an acute angle directing towards the left brachial plexus whenever the patient's shoulder was passiv

The reorganization of the motor system in primates after the loss of a limb.

As a result of an extensive search, we were able to obtain a limited number of macaque monkeys, New World squirrel monkeys, and prosimian galagos years after they had received a therapeutic amputation of a forelimb or hindlimb as a treatment for injury. Forelimb or hindlimb regions of primary motor cortex, M1, were stimulated in these primates with microelectrodes using amounts of current just sufficient to evoke a movement. Results were compared to those obtained from normal primates or from M1 contralateral to the intact limb in the same primates. In all cases, more sites in M1 contralateral

Massive rotator cuff tears repaired on top of humeral head by McLaughlin's procedure.

The clinical and radiographic outcomes of McLaughlin's procedure for massive rotator cuff tears were investigated in 25 shoulders, in which the cuff tears were so severe that the tendons were sutured on the top of the humeral head. The mean age at surgery was 62.2 years (range, 39-74 years). The mean follow-up period was 50 months (range, 24-80 months). The University of California, Los Angeles score significantly improved from 10.9 to 31.8 points postoperatively; the postoperative result was classified as excellent in 11 shoulders, good in 11, and poor in 3. Osteoarthritis progressed postoper

Successful salvage of the upper limb after crush injury requiring nine operations: a case report.

Emergency treatment of amputation is one of the most frequently used therapeutic methods for patients with severe upper limb crush injury with a mangled extremity severity score (MESS) of more than 7. With the development of advanced surgical repair techniques and reconstructive technology, cases that once required amputation can now be salvaged with appropriate management, and some limb functions may also be reserved. A patient with a severe upper limb crush injury with a MESS score of 10 was treated in our hospital. The limb was salvaged after 9 surgeries over 10 months. The follow-up visits

Factors associated with persistent shoulder pain following arthroscopic rotator cuff repair: a scoping review.

BACKGROUND: Rotator cuff (RC) tears are among the most common and disabling musculoskeletal conditions of the shoulder. When conservative management fails, arthroscopic rotator cuff repair (ARCR) is performed. Some patients, however, report persistent shoulder pain following the surgical procedure. To improve patient outcomes, it is essential to identify factors associated with persistent pain following ARCR, which is the aim of this scoping review. METHODS: The scoping review followed the Joanna Briggs Institute (JBI) methodology and Preferred Reporting Items for Systematic Reviews and Meta-

Providing safe and effective rehabilitation by assessing supraspinatus muscle elasticity using ultrasound real-time tissue elastography after rotator cuff repair: A case series.

INTRODUCTION: Although surgical techniques have advanced to avoid a postoperative re-tear after rotator cuff surgery, it remains unclear how to directly evaluate the risk of a re-tear. OBJECTIVE: To describe how muscle elasticity with real-time tissue elastography could be used to avoid re-tear in individual cases after rotator cuff repair. CASE DESCRIPTION: This case series included four patients, two per tear size (small and large/massive), with contrasting changes in muscle elasticity of the supraspinatus muscle. All patients underwent primary arthroscopic or open rotator cuff repair. The

The influence of excessive ipsilateral trunk tilt on upper-extremity throwing mechanics: a newly characterized parameter for biomechanical evaluation in high school and professional pitchers.

BACKGROUND: Although contralateral trunk tilt has been recognized, the phenomenon of excessive ipsilateral trunk tilt, often observed during earlier portions of the pitch, has not been explored as a potential correlate with throwing-arm kinetics among baseball pitchers. The purpose of this study was to evaluate kinetic and kinematic parameters among high school and professional pitchers related to excessive ipsilateral and contralateral trunk tilt. METHODS: Professional and high school pitchers were assessed with a 3-dimensional motion capture system (480 Hz) while pitching. Pitchers were gro

Editorial Commentary: Clinical Improvements of Superior Capsular Reconstruction Are Not Due to Maintained Dynamic Acromiohumeral Distance.

Biomechanical data often help inform clinical decision-making in orthopaedic surgery. However, there are times when the biomechanical data and clinical data do not align. This is seen in cases of statistical but clinically irrelevant differences. This is also seen at times in cases of demonstrated clinical improvements with a poorly understood mechanism. Superior capsular reconstruction has gained significant traction since the original description in 2012. It has taken a central role in the armamentarium of treatment options of irreparable rotator cuff tears. There is robust published literat

Efficacy of radial extracorporeal shockwave therapy in rehabilitation following arthroscopic rotator cuff repair: A STROBE compliant study.

Rotator cuff tear is a common cause of shoulder pain and disability. Arthroscopic rotator cuff repair (ARCR) is performed to treat a torn tendon. Postoperative joint immobilization is essential, but it is a problem that needs to be addressed in the rehabilitation process. This study aimed to evaluate the effects of radial extracorporeal shock wave therapy (rESWT) in patients who underwent ARCR and required active movement after the immobilization period. This study was an open-label, prospective, single-arm trial of 30 inpatients aged >18 years who underwent ARCR. A total of 6 rESWT sessions,

Editorial Commentary: Superior Capsular Reconstruction: Indications and Proper Technique Results in Good Outcomes but Reports of Complications.

Superior capsular reconstruction is a minimally invasive option to treat massive irreparable rotator cuff tears. In the appropriately selected patient, available data suggest that while the procedure generally results in improved function, there is a not insignificant risk of complications. Moreover, the rate of complications is likely underestimated given that outcomes are typically published by those with significant technical expertise. The literature supports improved outcomes in patients without significant degenerative change (less than Hamada 3) along with an intact or repairable subsca
