Relationship Between Manual Wheelchair Using Skills and Upper Limb Musculoskeletal Disorders in Patients With Spinal Cord Injury: A Musculoskeletal Ultrasonography Study.

OBJECTIVES: The aims of this study are to investigate the relationship between wheelchair skills and upper limb pathologies detected by ultrasound and to identify independent predictors of joint pathologies. DESIGN: Forty-five patients who were paraplegic because of spinal cord injury were included. Demographic and clinical data were evaluated. The Spinal Cord Independence Measure III and Wheelchair Skills Test Questionnaire was administered. Shoulder, elbow, and wrist joints of both upper limbs were evaluated with ultrasound. RESULTS: Nineteen patients (42.2%) had musculoskeletal pain in th…

Lacertus Fibrosus Syndrome: A Case Report.

Lacertus fibrosus syndrome is described as compression of the median nerve, which takes place beneath a layer of ligamentous tissue (lacertus fibrosus, also known as bicipital aponeurosis) slightly beyond the elbow joint. Both sexes can develop lacertus fibrosus syndrome, most often after the age of 35. The possible risk factors are repetition of movements, overwork, and manual work while the forearm is pronated. Lacertus fibrosus syndrome presents a distinct diagnostic challenge because it is a somewhat unknown and non-documented disease. Its symptoms are often mistaken for those of carpal tu…

The Effects of Botulinum Toxin Injections on Spasticity and Motor Performance in Chronic Stroke with Spastic Hemiplegia.

Spastic muscles are weak muscles. It is known that muscle weakness is linked to poor motor performance. Botulinum neurotoxin (BoNT) injectio … udy. Spasticity assessment (Modified Ashworth Scale (MAS) and reflex torque) and muscle strength of elbow flexors, as well as motor performance assessment (force variability of submaximal elbow flexion) were performed within one week before (pre-injection) and 3~4 weeks (3-wk) after BoNT injections. As expected, BoNT injections reduced the MAS score and …

Increased brain activation during motor imagery suggests central abnormality in Neonatal Brachial Plexus Palsy.

Neonatal Brachial Plexus Palsy (NBPP) may lead to permanent impairment of arm function. As NBPP occurs when central motor programs develop, these may be ill-formed. We studied elbow flexion and motor imagery with fMRI to search for abnormal motor programming. We compared the cortical activity of adults with conservatively treated NBPP to that of healthy individuals stratified for hand dominance, using fMRI BOLD tasks of elbow flexion and motor imagery of flexion. Additionally, resting-state networks and regional gray matter volume were studied. Sixteen adult NBPP patients (seven men; median ag…

Acute effects of direct inhibitory pressure over the biceps brachii myotendinous junction on skeletal muscle activation and force output.

Force (F) reduction is reported with myotendinous junction (MTJ) manipulation. Autogenic inhibition reflex (AIR) activation is supposed to be the main mechanism. Still, its role remains unclear. The study aimed at assessing the effects of MTJ direct inhibitory pressure (DIP) on neuromuscular activation and F in the elbow flexor (agonist) and extensor (antagonist) muscles. After maximum voluntary contraction (MVC) assessment, thirty-five participants randomly performed submaximal contractions at 20, 40, 60, and 80% MVC. Electromyographic (EMG), mechanomyographic (MMG), and F signals were record…

Ability of electrical stimulation therapy to improve the effectiveness of robotic training for paretic upper limbs in patients with stroke.

We investigated whether untriggered neuromuscular electrical stimulation (NMES) can increase the effectiveness of shoulder and elbow robotic training in patients with hemiparesis. Thirty subacute stroke patients were randomly equally allocated to robot only (RO) and robot and electrical stimulation (RE) groups. During training, shoulder and elbow movements were trained by operating the robotic arm with the paretic arm, and the robotic device helped to move the arm. In the RE group, the anterior deltoid and triceps brachii muscles were electrically stimulated at sub-motor threshold intensity. T…

Fractures of the radial head and neck.

The majority of simple fractures of the radial head are stable, even when displaced 2 mm. Articular fragmentation and comminution can be seen in stable fracture patterns and are not absolute indications for operative treatment. Preservation and/or restoration of radiocapitellar contact is critical to coronal plane and longitudinal stability of the elbow and forearm. Partial and complete articular fractures of the radial head should be differentiated. Important fracture characteristics impacting treatment include fragment number, fragment size (percentage of articular disc), fragment comminutio…

Shoulder and neck muscle activities during typing with articulating forearm support at different heights.

Use of forearm support is known to reduce physical stress of computer users, but research about how to properly position the forearm support is insufficient. This study was aimed to determine whether the height of forearm support influences muscular loads during typing. Twenty four subjects performed a typing task with a pair of articulating forearm support at three different heights as well as without any support, while shoulder, neck and forearm muscle activities and posture data were recorded. Typing with the support at resting elbow height produced significantly (p < 0.05) lower shoulder a…

Results of c5 root grafting to the musculocutaneous nerve using pedicled, vascularized ulnar nerve grafts.

PURPOSE: Vascularized nerve grafts are indicated for the repair of large nerve defects. In brachial plexus injuries, the poor prognosis for functional hand reconstruction when the lower roots are avulsed makes the ulnar nerve a potential donor for vascularized nerve grafts. We report on the results we obtained with reconstruction of elbow flexion using long pedicled ulnar nerve grafts that connected the C5 root to the musculocutaneous nerve. METHODS: We prospectively studied 8 young adults with complete brachial plexus palsy with avulsion of the lower roots, who had surgical repair an average…

Reliability of upper and lower limb three-dimensional kinematics in children with hemiplegia.

The repeatability of both 3D kinematic measurements of arm movement during simple upper limb tasks and lower limb movement during gait analysis was evaluated in 10 children with hemiplegic cerebral palsy. All tasks were completed on two separate occasions, 1 week apart. The 3D lower limb gait analysis showed high levels of repeatability in the sagittal plane measures, with mean coefficient of multiple correlations (CMCs) greater than 0.92 between sessions. Transverse and frontal plane measures had mean CMCs greater than 0.7 between sessions. A 3D analysis of shoulder and elbow flexion/extensio…

The effect of wrist guard use on upper-extremity injuries in snowboarders.

The objective of this investigation was to determine the effect of wrist guard use on all upper-extremity injuries in snowboarders. This mat … f wrist guard use among snowboarders with hand, wrist, or forearm injuries was 1.6%; for those with elbow, upper arm, or shoulder injuries, it was 6.3%; and for controls, it was 3.9%. Thus, wrist guard use reduced the risk of hand, wrist, or forearm injury by 85% (adjusted odds ratio = 0.15, 95% confidence interval: 0.05, 0.45). However, the adjusted od …

Biceps-to-triceps transfer in tetraplegic patients: report of the medial routing technique and follow-up of three cases.

We reviewed 4 biceps-to-triceps transfers for active elbow extension in 3 tetraplegic patients using a medial routing technique. The biceps-to-triceps transfer to regain active elbow extension in tetraplegic patients is an alternative to the more commonly described deltoid-to-triceps transfer. Before surgery, all 3 patients had absent triceps function and active biceps, brachialis, and supinator function. Postoperative results were assessed by a modified University of Minnesota Functional Improvement questionnaire and by follow-up evaluation of range of motion and muscle strength. All 3 patien…

Huckstep intramedullary nailing for non-union of humeral shaft fractures.

BACKGROUND: Various techniques of internal fixation for non-union of humeral shaft fractures have been attempted, leading to union rates of between 50 and 90% with persisting non-union causing pain and disability. Some of these techniques have led to shoulder and elbow dysfunction. METHODS: Eight patients treated with Huckstep nail fixation for humeral shaft non-union were reviewed and the rate of union was determined. Elbow and shoulder function were assessed as well as the presence of pain or disability. RESULTS: Union was achieved in seven of the eight patients (87.5%). Four patients had …
